Abstract

The invention relates to a device in the form of a tube (6) comprising two cuffs, which can be used in the medical field in order to control the airways and, in this way, to enable ventilatory support and which is known as a laryngeal catheter or a glottic mask. In particular, the inventive device can be used to overcome the disadvantages of the laryngeal mask associated with leaks and the movement thereof. The invention consists of a tube (6) which resembles an intubation catheter comprising a proximal hole and a distal hole and two cuffs, namely a first 20 ml air cuff (4) which is positioned approximately 1 cm from the distal end (2) and a second 100 ml air cuff (5) which is positioned approximately 1 cm behind the first cuff. The distal end (2) is positioned beneath the laryngoscope directly behind the vocal cords (3), giving rise to the name laryngeal catheter (1). The first cuff is then positioned directly in front of the vocal cords (4) and inflated with 20 ml of air. Subsequently, the second cuff (5) is inflated with approximately 100 ml of air, such that it is housed and fixed in the oropharynx. In this way, the first cuff (4) is maintained under pressure on the glottis in order to guarantee the tightness thereof, thereby giving rise to the name glottic mask.

Description

SONDE D'INTUBATION MUNIE D'UN MASQUE LARYNGIEN À DEUX BALLONETSINTUBATION PROBE HAVING A LARYNGIAN MASK WITH TWO BALLOONS
The technical field of the invention is medical and relates more specifically to the control of the airways.
Ce contrôle est réalisé classiquement par plusieurs dispositifs :This control is carried out conventionally by several devices:
- l'intubation trachéale qui permet un bon contrôle des voies aériennes, mais nécessite de traverser les cordes vocales puis une partie de la trachée laquelle est douloureuse et nécessite un très bon relâchement.- tracheal intubation which allows a good control of the airways, but requires crossing the vocal cords and a part of the trachea which is painful and requires a very good relaxation.
Comparativement au masque laryngé ce dispositif est plus étanche avec beaucoup moins de fuites et moins de risque d'inhalation.- the laryngeal mask: its insertion requires a lighter general anesthesia, but its disadvantages are the failures of the technique during the installation, the leaks and its secondary displacement. The device according to the invention overcomes the disadvantages of the two techniques without completely replacing them. It is indeed an intubation limited to part of the larynx (1) avoiding the crossing of the painful trachea. Its pose requires a general anesthesia less deep than for endotracheal intubation. Compared to the laryngeal mask this device is more watertight with much less leakage and less risk of inhalation.
It is a tube (6) resembling an intubation tube with a proximal orifice and a distal orifice but having two balloons, a first (4) of twenty milliliters of air located about one centimeter from the distal end ( 2) and a second (5) of one hundred milliliters of air located behind the first (4) at about one centimeter. Patient under general or comatose anesthesia, laryngoscopy and local anesthesia of the glottis by Xylocaine 5% (7 to 10 instillations) and then placement of the distal end (2) of the device at the level of the larynx (1) just after or between the vocal cords (3). Inflate the first balloon (4) with twenty milliliters of air and remove the laryngoscope by exerting light pressure with the hand on the device in the caudal direction to keep it in place. Inflate the second balloon (5) with one hundred milliliters of air, thus allowing the device to be fixed at the level of the oropharynx and to complete the sealing of the first balloon (4) around the glottis. pushing it towards the larynx (1).
The device can make it possible to provide ventilatory assistance in general anesthesia, especially for acts of short to medium duration. It could be useful in difficult intubations or often only a part of the glottis is visible. The device could provide ventilatory support during the realization of the percutaneous tracheotomy without hindering it.

REVENDICATIONS
1) Device for ensuring the control of the airways allowing ventilatory assistance by performing intubation limited to a part of the larynx (1) avoiding the crossing of the trachea, characterized in that it comprises a tube (6) forming a probe of intubation having a proximal orifice and a distal orifice and having two balloons, a first of twenty milliliters of air (4) located about one centimeter from the distal end (2) and a second (5) of one hundred milliliters of air located behind the first at about a centimeter; the distal end (2) is maintained just after the vocal cords (3) thanks to the two balloons (4, 5), the second balloon (5) once inflated fixing the device on the one hand on the level of the oropharynx and secondly maintain a pressure on the inflated first balloon (4) which will exert pressure on the glottis to seal and thereby form the "glottic mask" of the device.
