Abstract

What is described is a hip prosthesis stem (1), having a cavity (2) for the insertion of a movable neck. The stem (1) is provided with a gripping member (3) and with means (10, 11) for its reversible connection by complementary means (7) to the said gripping member (3), in such a way that the stem (1) can be manipulated without any touching of the outer surface (1e), while being unable to be translated or rotated with respect to the said gripping member (3).

Hip prosthesis stem provided with a gripping member connected to it.
The present invention relates to the technical field of the design and production of hip prosthesis stems, in other words of tapered metal members which are inserted into the femur and which are each provided with a cavity into which is inserted the movable neck which supports a spherical head that replaces the head of the femur.
As is known, the success of an operation for fitting a hip prosthesis depends not only on the correct design of the various components but also on the extent to which each part remains securely fixed without the possibility of rotational and translatory movement or slipping.
In particular, it is very important that the aforementioned stem is implanted in a reliably permanent way into the femur to which it is fitted. At the present time, this is achieved by means of what is known as the "primary" adhesive effect imparted by the forced joint between the stem and the femur, obtained by forcefully inserting the stem, which as stated has a tapered shape, into the femur. Use is also made of what is known as the "secondary" adhesion, provided by the incorporation of the outer surface of the stem into the layer of bone tissue which is in contact with it and which regenerates after the operation. To increase the effectiveness of the aforesaid secondary adhesion, the said outer surface of the stem is treated, or an additional layer is applied to it, in such a way that the surface directly in contact with the bone has a degree of roughness and/or considerable porosity. The cells of the regenerated bone tissue thus penetrate between the said roughnesses or into the said pores, creating a multiplicity of small protuberances which remain firmly fixed to the stem and thus prevent any movement .
The methods used at the present time include the application of hydroxyapatite, or a thin layer of porous titanium, as well as the sanding of the outer surface of the stem.
Whatever type of treatment is used, the roughness and/or porosity is extremely shallow, and if an area of the treated surface is touched before the stem is introduced into the femoral cavity, with the application of a layer of grease, blood or physiological tissue, even if this layer is very thin, then the said surface, as will be understood, becomes unable to perform the aforesaid function.
Stems produced at the present time are removed from their sterile wrapping during the operation, and their necessary handling, which may be complicated, very frequently gives rise to the problem mentioned above. If pliers or tongs are used to grip a stem in order to avoid this problem, then, because of the tapered shape of the stem, it is necessary to grip the inside of the aforementioned cavity for the insertion of the movable neck, and this gives rise to a risk of causing abrasion, dents or scratches, which, even if extremely small, may compromise the adhesion of the complicated joint between the stem and the movable neck.
To overcome all the drawbacks described above, the inventor of the solution proposed in this application has devised a stem which can be provided in advance with a convenient gripping member connected to it in an easily reversible way, with the aid of suitable connecting means formed on the said stem, before it is enclosed in its sterile wrapping. The stem can thus be removed from its wrapping by gripping the said gripping member, and can be manipulated until it has been fitted into the femur, after which the said gripping member, preferably of the disposable type, can be removed.
As described more fully below, the said means formed on the stem for connection to the gripping member can be designed in such a way that, on completion of the connection between them, the stem and the gripping member form a single unit, without the possibility of movement and/or rotation with respect to each other.
The object of the present invention is therefore a hip prosthesis stem as described in the attached Claim 1. A more detailed description will now be given of a preferred example of embodiment of the stem according to the invention, with additional reference to the attached drawings, in which:
- Figure 1 is a side view of the said example of embodiment of a hip prosthesis stem according to the invention with a separate gripping member which is subsequently connected to it; - Figure 2 is a front view of the example of embodiment of Figure 1, showing only a part of the gripping member.
With reference to the aforesaid Figures 1 and 2, these show how a hip prosthesis stem 1 according to the invention has the said cavity 2 provided with an opening 2a for the insertion of a movable neck (not shown) . Near this opening 2a, and in particular in the part of the outer surface le of the stem 1 surrounding this cavity 2, there are formed two recesses 10 and 11 parallel and opposite to each other in such a way that a relatively thin ridge 9 is formed between them, and can be clamped between the two jaws 5 and 6 of forceps 3, preferably of plastics material, provided with elastic return means consisting of a simple flexion or torsion spring 4 of a known type.
It should be noted that, by making the forceps in a suitable way according to known methods, its jaws can be made to bend and act as an opposing spring, without any need to fit an additional member having this function (this case is not shown in the drawings) .
The forceps 3 is shown in solid lines in the situation in which it is kept open by overcoming the opposition of the spring 4: the drawing shows how a projection 7, cylindrical in this case, is formed on the inner surface of the terminal portion 5t of one 5 of the jaws 5, 6, for connection in a complementary way to a cylindrical through hole 8 formed at the base of one of the said two recesses 10 and 11 (namely the recess numbered 10 in the drawing) .
If the forceps 3, opened as described, is advanced in such a way that its two jaws 5 and 6 enclose the said ridge 9, and the action of the spring 4 ceases to be opposed, the forceps 3 closes, and its cylindrical projection 7 penetrates into the complementary recess 8 described above, thus preventing any relative translatory movement between the forceps and the stem 1. (The forceps 3 is shown in broken lines in this connected state.)
To prevent any relative rotation between them, the inventor suggests that the said recesses 10 and 11 with the edges 12 (Figure 2) be shaped so that they house in a complementary way the said terminal portions 5t and 6t of the jaws 5 and 6 of the forceps 3. Thus the profiles of the said terminal portions adhere to the edges of the said recesses 10 and 11, and, for obvious geometrical reasons, cannot rotate in any way with respect to the forceps 3.
This method can be used to form a hip prosthesis stem which, being connected reversibly to a gripping member in such a way that it cannot carry out rotational or translatory movements with respect to it, can be manipulated without being touched on its outer surface. In this way the inventor's objective is achieved.
A further note should be added: the creation of the said recesses in the proximity of the opening of the cavity described above creates connecting edges between the edges and the bases of the said cavities, which, together with the ridge 9, form reliable elements for the application of one of the types intermediate members used to transmit to the stem the kinetic energy of a mallet with which the said stem is forcefully inserted into the femur .

Claims

Claims
1. Hip prosthesis stem (1), having a cavity (2) for the insertion of a movable neck, characterized in that it is provided with a gripping member (3) and with means (10, 11) for its reversible connection by complementary means (7) to the said gripping member (3), in such a way that the stem (1) can be manipulated without any touching of the outer surface (le) , while being unable to be translated or rotated with respect to the said gripping member (3) .

Stem according to Claim 1, in which the said gripping member consists of a forceps (3) with elastic return means (4), provided with two jaws (5 and 6), the inner surface of the terminal portion (5t) of at least one of these jaws having at least one recess or a projection (7) which can be connected in a complementary way to at least one projection or recess (8) formed on the outer surface of the stem (1) in the proximity of the opening (2a) of the said cavity (2) .

Stem according to Claim 2, in which the said at least one recess (8) or projection of the stem (1) is formed on a ridge (9) having parallel sides enclosed between two parallel and opposite recesses (10, 11) formed in the area of the outer surface (le) of the stem on which the said opening (2a) is located, these recesses being capable of containing on opposite sides the terminal portions (5t and 6t) of the jaws (5 and 6) of the said forceps (4).

Stem according to Claim 3, in which the edges (12) of the said two recesses (10 and 11) are shaped so that they house in a complementary way the terminal parts (5t and 6t) of the said jaws (5 and 6) , thus preventing the rotation of these jaws with respect to the stem (1) when this is connected to the said forceps (3)
