Abstract

Bone screws (100) and inserts (300) disposed therein or thereon are provided for the delivery of medicants to bone. The bone screws (100) provide a method to strengthen or repair damaged bones, or to promote bony fusion, and in conjunction with the inserts, provide a method of delivering a continuous or regulated amount of medicant to a desired location within the bone such as a fracture interface. In one embodiment, the inserts (300) are customized in size, shape, or configuration after the bone screw (100) has been inserted into the patient.

Claims

WHAT IS CLAIMED IS: 1. A device for delivering a substance to a bone, the device comprising: (a) a bone screw comprising two ends connected by a shaft, wherein the shaft is threaded along at least a portion of its exterior and caπnulated along at least a portion of its length; (b) one or more bone-screw fenestrations disposed along the cannulated portion of the bone-screw shaft; (c) an insert disposed inside the bone-screw cannulation, wherein the insert is cannulated along at least a portion of its length; and (d) one or more insert fenestrations disposed along the cannulated portion of the insert.
2. The device of claim 1 wherein the insert blocks at least one bone-screw fenestration.
3. The device of claim 1 wherein the bone screw compήses a material selected from the group consisting of titanium and its alloys, tantalum and its alloys, nickel-cadmium and its alloys, steel and its alloys., plastics, absorbable materials, resorbable materials, polyamino acids, polylactide, polyglycolide, hydroxylapatite, and tricalciumphosphate.
4. The device of claim 1 wherein the insert comprises a material selected from the group consisting of titanium and its alloys, tantalum and its alloys, nickel-cadmium and its alloys, steel and its alloys, plastics, absorbable materials, resorbable materials, polyamino acids, polylactide, polyglycolide, hydroxylapatite, and tricalciumphosphate.
5. The device of claim 1 further comprising a substance reservoir attached to at least one end of the bone screw or insert,
6. The device of claim 5 wherein the reservoir is implanted subcutaneously,

7. The device of claim 1 further comprising a pump attached to at least one end of the bone screw or insert.
8. The device of claim 7 wherein the pump is implanted subcutaneously.
9. The device of claim 1 wherein the bone screw further comprises an internal substance reservoir.
10. The device of claim 1 wherein the insert further comprises an internal substance reservoir.
11. A method of administering a substance to a bone, the method comprising introducing the device of claim 1 into a bone and introducing the substance into the insert cannulation.
12. The method of claim 11 comprising: (a) disposing the insert inside the bone-screw cannulation in a first alignment such that the insert blocks one or ore bone-screw fenestrations; (b) introducing the device into a bone; and (c) subsequently realigning ihe insert such that one or more of the blocked bone-screw fenestrations become unblocked.
13. The method of claim 11 comprising: (a) introducing the bone screw into a bone; (b) subsequently mtroducing the insert into the bone-screw cannulation.
14. A device for delivering a substance to a bone, the device comprising: (a) a bone screw comprising two ends connected by a shaft, wherein the shaft is cannulated along at least a portion of its length; b) one or more bone-screw fenestrations disposed along the cannulated portion of the bone-screw shaft; and
30 (c) an insert disposed inside the bone-screw cannulation, wherein the insert is cannulated along at least a portion of its length, and further wherein at least a portion of the insert comprises a porous material that is permeable to the substance to be delivered to the bone.

The device of claim 14 wherein the bone screw comprises a material selected from the group consisting of titanium and its alloys, tantalum and its alloys, nickel-cadmium and its alloys, steel and its alloys, plastics, absorbable materials, resorbable materials, polyamino acids, polylactide, polyglycolide, hydroxylapatite, and tricalciumphosphate.
16. The device of claim 14 wherein the insert comprises a material selected from the group consisting of titanium and its alloys, tantalum and its alloys, nickel-cadmium and its alloys, steel and its alloys, plastics, absorbable materials, resorbable materials, polyamino acids, polylaclide, polyglycolide, hydroxylapatite, and tricalciumphosphate.
17. The device of claim 14 further comprising a substance reservoir attached to at least one end of the bone screw or insert.
18. The device of claim 17 wherein the reservoir is implanted subcutaneously.
19. The device of claim 1 further comprising a pump attached to at least one end of the bone screw or insert.
20. The device of claim 1 wherein the pump is implanted subcutaneously.
21. The device of claim 1 wherein, the bone screw further comprises an internal substance reservoir.
22. The device of claim 14 wherein the insert further comprises an internal substance reservoir.
23. A method of administering a substance to a bone, the method comprising introducing the device of claim 14 into a bone and introducing the substance into tire insert cannulation.
24. A device for delivering a substance to a bone, the device comprising: (a) a bone screw comprising two ends connected by a shaft, wherein the shaft is cannulated along at least a portion of its length; (b) one or more bone-screw fenestrations disposed along the cannulated portion of the bone-screw shaft; (c) an insert disposed inside the bone-screw cannulation, wherein the insert is cannulated along at least a portion of its length; and (d) at least one expandable balloon disposed around a portion of the insert.
25. The device of claim 24, comprising a first balloon disposed around a first portion of the insert, a second balloon disposed around a second portion of the insert and at least one insert fenestration disposed between the first and second balloons.

The device of claim 24 wherein the bone screw comprises a material selected from the group consistmg of titanium and its alloys, tantalum and its alloys, nickel-cadmium and its alloys, steel and its alloys, plastics, absorbable materials, resorbable materials, polyamino acids, polylactide, polyglycolide, hydroxylapatite, and tricalciumphosphate.
27. The device of claim 24 wherein the insert comprises a material selected from the group consisting of titanium and its alloys, tantalum and its alloys, nickel-cadmium and its alloys, steel and its alloys, plastics, absorbable materials, resorbable materials, polyamino acids, polylactide, polyglycolide, hydroxylapatite, and tricalciumphosphate.

28. The device of claim 24 further comprising a substance reservoir attached to at least one end of the bone screw or insert.
29. The device of claim 28 wherein the reservoir is implanted subcutaneously.
30. The device of claim 24 further comprising a pump attached to at least one end of the bone screw or insert.
31. The device of claim 30 wherein the pump is implanted subcutaneously.
32. The device of claim 24 wherein the bone screw further comprises an internal substance reservoir.
33. The device of claim 24 wherein the insert further comprises an internal substance reservoir.
34. A method of administering a substance to a bone, the method comprising introducing the device of claim 24 into a bone and introducing the substance into the insert cannulation.
35. The method of claim 34 comprising: (a) introducing the device into a bone with the at least one balloon in a deflated state; and (b) subsequently inflating the at least one balloon.
36. The method of claim 34 comprising: (a) introducing the bone screw into a bone; (b) subsequently introducing the insert into the bone-screw cannulation.
37. A device for delivering a substance to a bone, the device comprising:
33 (a) a bone screw comprising two ends connected by a shaft, the shaft comprising a thread around at least a portion of its exterior surface; and (b) an insert comprising a cannulation along at least a portion of its length, the insert attached along the exterior surface of the bone screw.
38. The device of claim 37 wherein the external surface defines a groove i tming along at least a portion of its length and the insert is adapted to fit into the groove.
39. The device of claim 37 wherein the insert comprises one or more insert fenestrations disposed along the cannulated portion of the insert.
40. The device of claim 37 wherein the insert is permeable lo the substance to be delivered to the bone.
41. The device of claim 37 lurther comprising a substance reservoir attached to at least one end of the insert.
42. The device of claim 37 wherein the insert further comprises an internal substance reservoir.
4 . A method of administering a substance to a bone, the method comprising introducing the device of claim 37 into a bone and introducing the substance into the insert cannulation.
