Description
- Die vorliegende Erfindung bezieht sich grundsätzlich auf das Zurvertügungstellen einer Führung zur Benutzung während der Durchführung von Wirbelchirurgie, im Zusammenhang mit Systemen welche Bilder während der medizinischen und chirurgischen Prozeduren nutzen und herstellen, wobei die Bilder beim Durchführen der Prozeduren und Indizieren der relativen Position von verschiedenen Körperteilen und chirurgischen Instrumenten helfen. Im Besonderen bezieht sich die Erfindung auf eine Führung, und ein System welches die Führung nutzt, zum Durchführen von Wirbelchirurgie, um einen geschützten Zugang über eine Wirbelscheibe und in einen angrenzenden Wirbel zum Präparieren eines Implantatsraums zur Verfügung zu stellen, so dass ein vordefinierter Durchgang und eine Platzierung eines künstlichen Implantats/Knochendübels in den Implantatsraum sichergestellt wird.
- Hintergrund der Erfindung
- Verschiedene Instrumente und Methoden wurden zum Präparieren angrenzender Wirbel eines Rückgrates zum Einsetzen eines Knochendübels oder eines künstlichen Implantats bisher benutzt. Diese Instrumente und Methoden erlauben es Scheibenmaterial zwischen angrenzenden Wirbeln zu entfernen und von einem Abschnitt jedes Wirbels zu entfernen um einen Implantatsraum oder eine Öffnung zu schaffen. Bohrhülle, rohrtörmiges Glied, Futteral, Arbeitskanal, und Schutzvorrichtung sind einige wenige Namen welche für eine schützende Führung zum Führen von Bohrern und anderen scheiben- und knochenentfernenden Vorrichtungen in das Rückgrat zum Formen eines Implantatsraums zum Aufnehmen eines Knochendübels, oder eines künstlichen Implantats verwendet werden. Bestimmte Führungen haben geschärtte Enden oder umfassen Zähne zum Kontaktieren der Wirbel nach dem Aufbringen einer Auftreffkraft. Die Führung kann auch so positioniert werden, dass sie sich durch den gesamten Scheibenraum hindurch erstreckt und in die angrenzenden Wirbeln hineingesetzt wird, um einen geschützten Zugang zu den Wirbeln während des Prozesses des Herausformens des Implantatsraums zur Verfügung zu stellen. Der Implantatsraum erstreckt sich durch den gesamten Scheibenraum und ragt in jeden der angrenzenden Wirbel entlang zweier sich gegenü berstehender herausgeschnittener Bögen hinein. Ein Beispiel für die Prozedur zum Bohren von Löchern über einen Scheibenraum und die dazu gehörige Instrumentation ist in dem US-Patent 5,484,437 von "Michelson" beschrieben.
- Das Nutzen von zylindrischen Implantaten ist wünschenswert, da der Chirurg die aufnehmende Stelle durch das Bohren eines zylindrischen Loches über den Scheibenraum und in die angrenzenden Wirbel einen hineinpräparieren kann. Die kurvige Oberfläche der zylindrischen Löcher, welche in die Wirbel hineingebohrt wurden, stellen die Möglichkeit einer engen Passung (engen Kongruenz) dann zur Verfügung, wenn das zylindrische Loch mit einem Implantat ausgefüllt wird, welches korrespondierende zylindrische Abschnitte eines passenden Durchmessers aufweist.
- Typischerweise werden gewindebehaftete künstliche Implantate, wie z.B. das Implantat, welches in der US-Patentschrift 5,015,214 von Michelson offenbart ist, in den Implantatsraum zwischen den angrenzenden Wirbeln direkt eingesetzt, um direkt zu partizipieren und in die nachfolgende Fusion aufgenommen zu werden. Gewindebehaftete Knochendübel, wie z.B. durch Viche gelehrt, können auch in den Implantatsraum mit dem Ziel des Überbrückens der Öffnung platziert werden und mit dem Ziel des Inkorporierens in die Fusion zwischen den Wirbeln eingebracht werden. Des weiteren können künstliche Implantate des "push-in-Typs", wie z.B. in der US-Patentschrift 5,593,409 von Michelson offenbart, und dem Inhaber der vorliegenden Anmeldung überschrieben, in den Implantatsraum eingebracht werden, welche durch die vorab beschriebenen Instrumente und Methoden geformt werden.
- Egal ob das Implantat oder der Dübel in den Implantatsraum hineingestoßen oder geschraubt wird, versucht der Chirurg das Futteral zum Führen der Formation des Implantatsraums so zu orientieren, dass ungefähr dieselbe Menge von Material von jedem Wirbel entfernt wird. Sobald das Implantat oder der Dübel implantiert ist, kann ein Röntgenbildschirm verwendet werden um beim Feststellen, ob eine angemessene Platzierung stattgefunden hat, zu assistieren.
- Eine Anzahl von differenten Typen an chirurgischen Navigationssystemen sind im Stand der Technik beschrieben worden, welche das Indizieren der relativen Position des medizinischen Instruments und der Körperteile, welche in den medizinischen und chirurgi schen Prozeduren verwendet werden, inkludiert. Zum Beispiel ist hier die US-Patentschrift Nr. 5,383,454 von Bucholz: PCT-Anmeldungsnummer PCT/US94/04530 (Veröffentlichungsnummer WO 94/24933) von Bucholz: und die PCT-Anmeldung mit der Nummer PCT/US95/12894 (Veröffentlichungsnummer WO 96/11624) von Bucholz et al. zu nennen, welche Systeme zum Nutzen während medizinischer und chirurgischer Prozeduren unter Nutzung von Scans mittels eines Scanners vor der Durchführung der Prozedur offenbaren. Chirurgische Navigationssystem inkludieren typischerweise auch Verfolgungsmittel, wie z.B. eine LED-Anordnung auf dem Körperteil, Emittern auf den medizinischen Instrumenten, einem Analog-Digitalumsetzer zum Verfolgen der Positionen des Körperteils und der Instrumente, und ein Display (Anzeige) für die Position eines Instruments, welches in der medizinische Prozedur relativ zu einem Körperteil verwendet wird.
- Prozeduren zum Präparieren eines Implantatsraums in dem Rückgrat umfassen einige bestimmte besondere Herausforderungen die an spezifische Level im Inneren der Wirbelsäule gerichtet sind. Zum Beispiel kann das Durchführen einer Prozedur an L4-5 des Rückgrates folgende Aufgaben stellen: 1) Während eines Posterioransatzes, ist das Entfernen von bedeutenden Muskeln und das Ziehen bedeutender Muskeln Voraussetzung um den Scheibenraum L4-5 zu erreichen. Signifikantes postoperatives Trauma beim Patienten kann resultieren. 2) Während eines Anterioransatzes mit entweder einem transperitonealen oder einem retroperitonealen, einem offenen oder einem laparoskopischen Ansatz um den Scheibenraum L4-5 (Bandscheibe L4-5) zu erreichen, liegen die großen Gefäße fast direkt auf der Vorderseite des Wirbelkanals an diesem Level. Sezierung und Manipulation dieser Gefäße kann zeitintensiv und schwierig sein.
- Der Anmelder erkannte, dass ein retroperitonealer Schrägansatz zu dem L-5 Scheibenraum (Bandscheibe) eine bevorzugte chirurgische Lösung darstellen kann. Das Muskelzerteilen und Ziehen eines Posterioransatzes ist dabei nicht unabdingbar, da der schräge Ansatz direkt anterior von den Psoasmuskeln stattfindet, welche lateral entlang der Wirbelsäule positioniert sind. Der schräge Ansatz erlaubt auch einen leicht hinteren Ansatz von den großen Gefäßen, welche anterior von der Wirbelsäule liegen, wodurch das Risiko der Verletzung der großen Gefäße reduziert wird.
- Beim Betrachten des Rückgrates anhand lediglich eines lateralen Fluorobildes, würde der Wirbelchirurg, über diesen schrägen Ansatz, eine Indizierung der Tiefe und der Ausrichtung der Instrumente in limitierter Weise feststellen, wie diese auf posterior laterale Weise über den Scheibenraum in Richtung des ausgehenden Nervenkanals sich bewegen.
- Im Lichte des Vorhergesagten, wäre es in der Kunst für ein System und eine Methode zur Platzierung einer Führung in den Wirbel segensreich, welche Führungshilfe dem Chirurg zur Verfügung stellt um die Platzierung der Führung beim Formen des Implantatsraums zu verbessern, sowohl die Nutzung bei der Determinierung der Tiefe des Einsetzens der Instrumente, welche durch die Führung geschoben werden, als auch das Platzieren der Implantate und Dübel in dem Implantatsraum zu verbessern.
- Zusammenfassung der Erfindung
- Die vorliegende Erfindung bezieht sich auf eine Führung, ein System und eine Methode zum Nutzen einer Führung beim Durchführen von Wirbelchirurgie, und zwar zum Präparieren eines Implantatsraums über einer Wirbelscheibe und angrenzende Wirbeln. Um genauer zu sein, ist es eine Aufgabe der vorliegenden Erfindung eine Apparatur und ein Verfahren zum Setzen einer Führung in die Wirbel unter Nutzung eines bildgeleiteten Chirurgieverfahrens unter Verbesserung der Platzierung der Führung, durch welche der Implantatsraums ausgeformt ist und dabei eine angemessene Platzierung des Implantats oder Dübels nach sich zieht, zu ermöglichen.
- Um diese Aufgabe und andere Vorteile in Übereinstimmung mit der Aufgabe der Erfindung durchführen zu können, umfasst die Erfindung eine Führung zum Durchführen einer Wirbelchirurgie zum Präparieren eines Implantatsraums über einer Wirbelscheibe und angrenzender Wirbeln, wie hierin ausgeführt und ausführlich beschrieben. Die Führung wird in Verbindung mit einem computerkontrollierten Chirurgienavigationssystem verwendet, und zwar unter Verwendung eines Energiedetektionsfeldes zum Verfolgen der Positionen der Führung in drei Dimensionen relativ zu einem bekannten Referenzpunkt. Die Führung umfasst einen Körper zum Zurvertügungstellen eines geschützten Zuganges, zum Präparieren des Implantatsraums über die spinale Scheibe (Bandscheibe) und in die angrenzenden Wirbel. Der Körper hat eine Passage die derart adaptiert ist, dass sie eine Knochenentfernungsvorrichtung zum Ausformen des Implantatsraums durch den Körper aufnimmt. Wenigstens ein Emitterfeld ist an dem Körper zur Nutzung während der Identifikation des Ortes der Führung relativ zu den angrenzenden Wirbeln angebracht.
- Die Führung kann auch zusätzlicherweise eine scheibendurchdringende Extension umfassen, welche sich von dem Körper entfernt, und zwar zum Einsetzen in den Scheibenraum zwischen angrenzenden Wirbeln und zum Tragen oder Abstützen gegen Endplatten der angrenzenden Wirbel. Ein bevorzugter Körper umfasst eine hohle Röhre welche ein Ende aufweist, welches Extensionen zum Penetrieren des Rückgrates umfasst. Ein Emitterfeld ist vorzugsweise in jeder Extension der Führung angebracht und umfasst vorzugsweise wenigstens eine LED. Bevorzugte Felder umfassen ein elektrisch mit Energie versehbares Energieemissionsfeld, bekannt als aktiver Emitter, als auch eine reflektierende Oberfläche, welche Signale reflektiert, bekannt als passiver Emitter.
- Zusätzlich umfasst die Erfindung auch ein System zur Nutzung bei der Präparation eines Implantatraums im Rahmen einer Wirbelchirurgie über einer spinalen Scheibe und angrenzenden Wirbeln. Das System umfasst die oben beschriebene Führung und ein computerkontrolliertes Chirurgienavigationssystem unter Verwendung eines Energiedetektionsfeldes um die Positionen der Führung in einem dreidimensionalen Raum, relativ zu einem bekannten Referenzpunkt zu verfolgen.
- Zusätzlich kann die Erfindung auch in einer Methode zum Durchführen von spinaler Chirurgie mit einer Führung verwendet werden, welche verwendet wird um einen geschützten Zugang über eine Bandscheibe und in angrenzende Wirbeln zum Präparieren eines Implantatsraums zur Verfügung zu stellen. Die Methode umfasst die Schritte des Anbringens eines Referenzfeldes an den Wirbel um den es geht; Registrierung des Ortes des Wirbels mit einem computerkontrollierten Chirurgienavigationssystems; Kontaktierung eines Endes der Führung mit wenigstens einem Emitterfeld, welches an den angrenzenden Wirbel angebracht ist; Verwendung eines Chirurgienavigationssystems mit einem Computercontroller und einem Analog-Digitalumwandlerfeld zum Kommunizieren mit dem Energieemitter der Führung; Positionierung der Führung in einem dreidimensionalen Raum relativ zu einem bekannten Referenzpunktsystem; und Ausformen des Implantatsraums durch die Führung über den Scheibenraum und in Implantatsraums durch die Führung über den Scheibenraum und in einen Abschnitt von jedem angrenzenden Wirbeln.
- In einem weiteren Aspekt, umfasst die Methode den Schritt des Generierens eines Displays in der Position der Führung. Der Schritt zum Generieren zeigt vorzugsweise den Ort der Führung relativ zu den angrenzenden Wirbeln um eine Sicht auf die axiale Orientierung der Führung relativ zu jedem Wirbel und/oder relativ zu den angrenzenden Wirbeln zur Verfügung zu stellen. Die Orientierung der die Scheiben durchdringenden Extensionen kann auch dargestellt sein. Die Stelle der Spitze eines Instruments, welche durch die Führung gesetzt werden kann, kann auch dargestellt werden. Ein Identifizieren der Stelle der Instrumentenspitze, welche durch die Führung hindurchgelangt, erlaubt das Verfolgen der Werkzeugeinsatztiefe. Der Generierungsschritt kann auch den Ort der Führung relativ zu den angrenzenden Wirbeln anzeigen, um eine Schnittansicht der Führung relativ zu einer anterior oder posterior Sicht der angrenzenden Wirbeln zur Verfügung zu stellen.
- Die Methode kann auch den Schritt des Emittierens eines Signals von einem Emitter, welcher an der Führung angebracht ist, welche on einem Apparat repräsentativ aufgenommen wird und das Signal auf einem visuellen Display wiederzeigt, umfassen. Eine bevorzugte Methode der vorliegenden Erfindung umfasst auch den Schritt des Implantierens eines künstlichen Implantats, eines Knochendübels, oder anderer Typen von Knochen in dem Implantatsraum. Die Methode kann auch den Schritt des Verfolgens eines künstlichen Implantats oder Knochendübeleinsatzes über ein Emitterfeld, welches angebracht an das Einsetzgerät ist, umfassen. Das Einsatzgerät oder der Einsetzer ist vorzugsweise so konfiguriert, dass er durch die Führung passt. Das computerkontrollierte Chirurgienavigationssystem kann auch so konfiguriert sein, dass es das Einsatzgerät darstellt, wenn es durch die Führung gelangt und noch weiter bevorzugt, kann auch sowohl das Einsatzgerät als auch das angebrachte künstliche Implantat oder der Knochendübel basierend auf den geometrischen Konfigurationen des Einsatzgerätes und des Implantats oder Knochendübels, wie es daran angebracht ist, darstellen.
- Die Aufgaben der vorliegenden Erfindung sind es, einem Chirurgen eine Führung zur Verfügung zu stellen, und ein System zum Verfolgen der Führung in Verbindung mit einem chirurgischen Navigationssystem auf eine Weise zur Verfügung zu stellen, so dass ein Patient an den Wirbeln behandelt werden kann, und dabei sichergestellt ist, dass die Führung zu dem Rückgrat beim Ausformen eines Implantatsraums angemessen orientiert ist.
- Zusätzliche Aufgaben und Vorteile der Erfindung werden zum Teil in der nachfolgenden Beschreibung genauer erläutert werden und werden zum Teil auch durch das Lesen der Beschreibung offensichtlich werden, oder können durch das Praktizieren der Erfindung erlernt werden. Die Aufgaben und Vorteile der Erfindung werden durch die Elemente und Kombinationen, wie sie im Besonderen durch die Beschreibung ausgestellt werden, realisiert und erzielt.
- Es sollte verständlich sein, dass sowohl die vorhergehende allgemeine Beschreibung und die nachfolgende detaillierte Beschreibung lediglich exemplarisch und erklärend und nicht die Erfindung beschränkend sind.

- Im Folgenden wird nun auf eine bevorzugte Ausführungsform Bezug genommen, wobei ein Beispiel dessen in den beiliegenden Zeichnungen illustriert wird. Wo auch immer möglich, werden dieselben Referenzzeichen für dieselben oder ähnlichen Bauteile in den Zeichnungen verwendet. Das nachfolgende Beispiel ist als rein exemplarisch für die Erfindung gedacht.
- Wie grundsätzlich in der PCT/US95/12894 beschrieben, ist ein typisches Chirurgienavigationssystem in
1A gezeigt. Ein computerassistiertes Bildführungschirurgiesystem, grundsätzlich mit Nummer 10 indiziert, generiert ein Bild zum Darstellen auf einem Monitor106 , welches die Position von einem oder mehreren Körperelementen, wie z.B. spinalen Elementen, welche fix in einem stabilisierenden Rahmen gehalten werden oder einer Vorrichtung wie z.B. einem spinalen Chirurgierahmen125 , wie er häufig in der Wirbelchirurgie verwendet wird, darstellt. Eine Referenzbogen120 der Verfolgungsmittel, wie z.B. LED-Emitter122 enthält, ist an dem stacheligen wirbeligen Prozess durch eine Klammer oder andere verbindende Vorrichtungen angebracht. Das Bild ist von einem Bilddatenset generiert, grundsätzlich vor der Operation durch einen CAT-Scanner z.B. generiert, wobei das Bild Referenzpunkte für wenigstens ein Körperelement, wie z.B. ein Wirbelelement, aufweist. Die Referenzpunkte des spezifischen Körperelements haben fixe beabstandete Relationen zu dem spezifischen Körperelement. Das System umfasst eine Apparatur, wie z.B. eine Analog-Digital-Umwandeleinheit oder eine andere Positionssensiereinheit (PSU) wie z.B. eine Sensorteldanordnung110 auf der Stütze120 zur Identifizierung in der relativen Position von jedem der Referenzpunkte, welche dargestellt werden müssen über die Verfolgung der Position der Emitter122 auf dem Bogen120 , während der Prozedur. Das System umfasst auch einen Prozessor114 , wie z.B. einen PC oder andere geeignete Workstationprozessoren in Verbindung mit Controller108 zur Modifizierung des Bilddatensatzes in Übereinstimmung mit den identifizierten relativen Positionen von jedem der Referenzpunkte während der Prozedur, so wie sie durch das Feld110 identifiziert sind. Der Prozessor114 kann dann, z.B., einen verschobenen Bilddatensatz generieren, welcher die Position der Körperelemente während der Prozedur für die Darstellung auf dem Monitor106 repräsentiert. Alles in allem ist die Bedienung eines Chirurgienavigationssystems im Stand der Technik ausreichend gut bekannt und es besteht keine Notwendigkeit dieses hier noch genauer zu beschreiben. - Eine Ausführungsform der vorliegenden Erfindung ermöglicht das Verfolgen ohne Nutzen einer Klammer oder anderer verbindender Vorrichtungen zum Halten eines Emitters an dem Wirbelkörper unter Nutzung von dynamischer Referenzierung (wie z.B. Fluoroskopreferenzierung, magnetischer Resonanzbildintraoperativerzeugung und Ultraschallregistrierung). All diese Optionen zeigen Echtzeitverschiebungen und Manipulationen des Rückgrates.
- In Übereinstimmung mit der bevorzugten Ausführungsform der vorliegenden Erfindung, unter Bezugnahme auf die
2 und3 , umfasst die Erfindung die Führung130 zur Nutzung bei der Durchführung von Wirbelchirurgie zum Präparieren eines Implantatsraumes über eine spinale Scheibe Bandscheibe) und angrenzende Wirbel hinweg. Die Führung130 wird in Verbindung mit einem computerkontrollierten Chirurgienavigationssystem10 verwendet, und zwar unter Verwendung eines Energiedetektionsfeldes110 zum Verfolgen der Positionen der Führung130 in einem dreidimensionalen Raum relativ zu einem bekannten Referenzpunkt. Die Führung130 umfasst den Körper132 zum Ver fügungstellen eines geschützten Zuganges zum Präparieren des Implantatsraums über die spinale Scheibe (Bandscheibe) und in die angrenzenden Wirbel hinein. Der Körper132 hat eine Passage die derart ausgestattet ist, dass sie eine Knochenentfernungsvorrichtung aufnehmen kann, wie z.B. in3 gezeigt, zum Ausformen des Implantatsraums durch den Körper132 . Wenigstens ein Emitterfeld130 ist an dem Körper132 zur Nutzung bei der Identifizierung der Stelle der Führung130 relativ zu den angrenzenden Wirbeln angebracht.10 zeigt eine alternative Führung130 mit scheibenpenetrierenden Extensionen134 und daran angebrachtem Emitterfeld190 . - Eine bevorzugte Knochenentfernungsvorrichtung umfasst eine Reibahle
300 mit einer LED310 wie in den3 ,6A und6B gezeigt. Ein visuelles Display der Reibahle300 kann auch zum Zeigen der Tiefe des Einsetzens der Reibahle300 in das Rückgrat, relativ zu den bekannten Koordinaten der Führung130 und des Körpers, in welchen die Reibahle300 penetriert, generiert werden. - Andere Vorrichtungen zum Nutzen beim Durchführen von Wirbelchirurgie über den Scheibenraum werden in den
9 ,11A ,11B und12 gezeigt. Die9 zeigt einen Distraktor410 mit angebrachtem T-Griff430 und Emitter190 . Die11A zeigt einen Zapfen400 welcher vorzugsweise derart ausgestattet ist, dass er an den T-Griff430 wie in11B gezeigt angeschlossen werden kann. Der T-Griff430 umfasst einen Emitter190 der daran, zur Nutzung bei der Identifizierung der Stelle des Zapfens400 , relativ zu den angrenzenden Wirbeln, in welche er eingesetzt ist, genutzt wird. Die12 zeigt einen Einsetzer420 mit angebrachtem T-Griff430 und Emitterfeld190 . Der Emitter190 auf dem Distraktor410 , der Zapfen400 , und der Einsetzer420 , werden jeweils genutzt um den Ort für jede Werkzeugeinheit in dem dreidimensionalen Raum relativ zu einem bekannten Referenzpunkt derart zu identifizieren, so dass jedes Werkzeug individuell auf dem visuellen Display gezeigt werden kann, welches mit dem computerkontrollierten Chirurgienavigationssystem10 assoziiert ist. - Die Führung
130 kann zusätzlicherweise auch eine scheibendurchdringende Extension umfassen, welche sich von dem Körper132 zum Einsetzen in den Scheibenraum zwischen den angrenzenden Wirbeln erstreckt und zum Tragen gegen die Endplatten der angrenzenden Wirbel ausgestaltet ist. Ein bevorzugter Körper132 umfasst eine hohle Röhre mit umfassenden Extensionen134 zum Durchdringen des Rückgrates. Ein be vorzugtes Emitterfeld110 auf der Führung sind lichtemittierende Dioden (LEDs), aber es können auch jegliche andere energieausstrahlende Vorrichtungen oder Verfolgungsmittel, wie sie im Stand der Technik bekannt sind und zur Verfolgung durch ein korrespondierendes Detektorfeld geeignet sind, genutzt werden. Ein bevorzugtes Emitterfeld umfasst drei LEDs zur Nutzung bei der Determinierung der Stelle und Rotation eines Werkzeugs im Raum in allen Richtungen. Im Sinne der Illustration und nicht der Limitierung kann die Verfolgungsvorrichtung Signale aktiv generieren, wie z.B. mit akustischen, magnetischen, elektromagnetischen, radiologischen und mikropulsierten Radarsystemen oder passiv, wie z.B. über reflektierende Oberflächen. - Zusätzlich umfasst die Erfindung ein System zur Nutzung in der Durchführung von Wirbelchirurgie zum Präparieren eines Implantatsraums über eine Bandscheibe und angrenzende Wirbel. Das System umfasst die vorab beschriebene Führung
130 und ein computerkontrolliertes Chirurgienavigationssystem10 unter Verwendung eines Energiedetektionsfeldes110 zum Verfolgen der Positionen der Führung130 in einem dreidimensionalen Raum relativ zu einem bekannten Referenzpunkt in einem Körper, welcher auf ähnliche Weise durch ein Sensorfeld110 verfolgt wird. - Die Führung
130 und das computerkontrollierte Chirurgienavigationssystem10 versetzen den Wirbelchirurgen in die Möglichkeit die angemessene Implantierung eines Implantats oder Dübels sicherzustellen, und zwar durch das Formen eines angemessen orientierten Implantatsraums durch die Führung130 und in das Rückgrat, sowie auch das Beobachten der Tiefe der Erfindung von Instrumenten, die durch die Führung130 gelangen. Als Ergebnis ist dabei sichergestellt, dass die Führung130 vor dem Ausformen des Implantatsraums angemessen ausgerichtet wird. Die4A zeigt eine Querschnittsansicht eines spinalen Segmentes200 mit einem Implantat oder Dübel202 in einer Kontaktierung der vorbestimmten Endplatte204 . Typischerweise tritt die vorbestimmte Endplatte204 Kontaktierung dann auf, wenn ungefähr gleiche Mengen vom Knochen von jeder angrenzenden Endplatte204 der angrenzenden Wirbel entfernt werden und das Implantat oder der Dübel202 axial von anterior A nach posterior P oder umgedreht ausgerichtet ist. - Eine Querschnittsansicht eines Wirbelsegments eines Implantants oder Dübels
202 in einer grundsätzlich weniger gewünschten Position wird in4B gezeigt. Das Implantat oder der Dübel202 , wie er in4B dargestellt ist, ist in einem Implantatsraum mit winkliger sagittaler Trajektorie implantiert. Im Kontrast dazu, ist in5B eine grundsätzlich bevorzugte vorbestimmte sagitalle Ausrichtung des Implantats oder Dübels202 in Bezug auf die Wirbel dargestellt. -
5A zeigt eine Querschnittsansicht eines Implantats oder Dübels202 , die über einen axialen Abschnitt eines Wirbels projiziert ist und zeigt eine vorbestimmte axiale Ausrichtung des Implantats oder Dübels202 mit dem Wirbel von posterior nach anterior (von vorne kommend nach hinten gehend).5C zeigt eine Querschnittsansicht eines Implantats oder Dübels202 , welche zwischen angrenzenden Wirbeln positioniert ist und zeigt eine vorbestimmte coronale Ausrichtung des Implantats oder Dübels202 zu dem Wirbel. Die angemessene Platzierung der Führung130 vor dem Ausformen des Implantatsraums setzt das Implantat202 in die Lage in einer vorbestimmten axialen, sagittalen und coronalen Ausrichtung, wie in5A ,5B und5C gezeigt, zu platzieren. - Sobald der Chirurg den Wirbelkörper mit dem computerkontrollierten Chirurgienavigationssystem
10 registriert, kann die Stelle der oben offenbarten Führung130 auf einem Computersystem relativ zu dem Körper angeschaut werden, so dass der Wirbelchirurg in Echtzeit den Ort des Endes der Führung130 in einer axialen, coronalen und sagittalen Ansicht, sowohl als auch in einer 3-D-Rekonstruktion sehen. Der Chirurg wird auch in der Lage sein die Trajektorie der Führung130 relativ zu dem Körper in allen drei Stellen zu sehen. Diese Technik verbessert deutlich den Prozess für den Chirurgen zum Positionieren der Führung130 , beim Ausformen eines Implantatsraums und optimiert dadurch die Positionierung des Implantats oder Dübels der eingesetzt werden soll. - In Übereinstimmung mit den Anzeigebildschirmen, wie sie in den
6A ,6B ,7 und8 dargestellt sind, ist, sobald ein Referenzfeld an den Patienten angebracht ist und registriert ist, die Software im Navigationssystem10 in der Lage die folgende Information dem Chirurgen zur Verfügung zu stellen. Die Stelle der Führung130 wird relativ zu der Anatomie des Patienten gezeigt. Die rotatorische Position der Führung130 wird auch indiziert, damit der Chirurg die Orientierung der Schutzextensionen134 relativ zu den angrenzenden Wirbeln des Rückgrates beobachten kann. - Im Besonderen zeigen die
6A und6B eine Reibahle300 , die durch die Führung130 hindurchgeschoben ist und die Tiefe der Penetrierung der Reibahle300 . Die6A zeigt eine Reibahle300 unmittelbar vor dem Entfernen von Material von den Wirbelendplatten. Die6B zeigt eine Reibahle300 mit einer größeren Tiefe zum Einsetzen und nach dem Entfernen von Material von den Wirbelendplatten. Die Endposition der Reibahle300 bleibt auf dem Anzeigebildschirm als Bezug für den Chirurgen. Der Durchmesser der Reibahle300 ist vorzugsweise in den Computer eingegeben oder aus einem vorprogrammierten Menü selektiert worden. -
7 zeigt den Zapfen400 wie er durch die Führung130 eingesetzt wird. Die Endposition des Zapfens400 bleibt auf dem Bildschirm als Referenz für den Chirurgen. Der Durchmesser des Zapfens400 ist in den Computer eingegeben oder aus einem vorprogrammierten Menü selektiert worden. -
8 zeigt ein Display welches die Stelle und die Tiefe des Implantats500 nach Einsetzens durch die Führung130 repräsentiert. Die Endposition des Implantats500 bleibt auf dem Bildschirm zur Referenz für den Chirurgen. Sowohl die Spitze als auch das Ende des Implantats500 ist auf dem Bildschirm sichtbar. Der Durchmesser und die Länge des Implantats500 ist vorzugsweise in den Computer eingegeben oder aus einem programmierten Menü selektiert worden. - Durch das Eingeben der Dimensionen des Implantats
500 in den Computer oder durch das Selektieren einer Implantatsgröße aus einem programmierten Menü und durch das Vorfinden der geometrischen Konfiguration des Einsetzers420 in den Computer, kann das Display das Implantat in Relation zu den angrenzenden Wirbeln im Wissen der Position des Einsetzers und dadurch Wissen der Position eines fest angebrachten Implantats500 zeigen. Auf diese Weise kann man das Implantat500 verfolgen oder jede andere Vorrichtung bekannter Dimension, welche an ein Werkzeug bekannter Dimension angeschlossen ist, wobei daran ein Emitterfeld angebracht ist. Durch das Vorsehen eines Emitterfeldes an dem Werkzeug bekannter Dimension und dem Verfolgen der Position davon, kann die Position einer anderen Vorrichtung bekannter Dimension, welche fest an dem Werkzeug angebracht ist, sicher festgestellt werden. Dadurch kann das Implantat oder eine andere Vorrichtung auf dem Navigationssystemmonitor ohne zwin gende Notwendigkeit für einen Separationsemitter an der angebrachten Vorrichtung dargestellt werden. - Nachdem die bevorzugte Ausführungsform der Führung in dem vorliegenden System beschrieben wurde, wird nun die Methode des Nutzens des Apparates zum Ausführung der Erfindung näher beschrieben werden. Das Bedienen eines Chirurgienavigationssystems ist grundsätzlich wohlbekannt und in der Druckschrift PCT/US95/12894 beschrieben. Der erste Schritt ist die Stelle des Wirbels unter Nutzung der computergestützten Bildführungschirurgienavigationsmethoden zu identifizieren.
- Ein Referenzfeld
120 ist fest an den Wirbeln, die zu untersuchen sind, befestigt. Die Anbringungsmittel können Klammern, Schrauben, oder andere Mittel sein. Das Referenzfeld kann über einen spinalen Prozess, wie z.B. in einem Posteriortall oder dem Anteriorkörper in einem Anteriorfall sein. Das Feld kann percutan mit einer minimal invasiven Technik angebracht werden. Sobald das Referenzfeld angebracht ist, muss der Körper, der untersucht werden soll, zu den Bilddaten in dem computerkontrollierten Chirurgienavigationssystem registriert werden. Dies kann auf unterschiedlichste Weisen erreicht werden. In einem offenen oder endoskopischen Fall, können Punkte auf dem Knochen physikalisch mit einem verfolgten Pointer (tracked pointer) berührt werden. Für eine percutane Technik, können Punkte auf einem Vergleichsfeld, welches an dem Körper vor dem Scan angebracht wurde, berührt werden, oder Punkte auf fluoroskopischen oder ultraschallgenerierten Bildern können identifiziert werden. Die Erfindung kann auch mit einer anderen Registrationstechnik hergestellt werden. Die Erfindung kann auch mit anderen Chirurgietechniken (d.h. posterior, anterior, laparoskopischen, anterior endoskopischen, etc.) genutzt werden. - Sobald die Referenz
120 auf dem Patienten platziert ist und mit dem Computersystem anatomisch registriert ist, kann die Führung130 kann im Raum relativ zu dem Rückgrat in dem Chirurgienavigationssystem ohne weitere chirurgische Exponierung des Rückgrates verfolgt werden. Die Position der Führung130 wird durch den Nutzer über das Drauftreten auf das Fußpedal116 , über das das Emitterfeld190 mit Energie versorgt wird, determiniert. Die Emitter195 generieren Infrarotsignale, welche über ein Kameraanalog-Digital-Wandlerfeld110 aufgenommen werden und unter Nutzung der Triangulationsmethode zur Bestimmung der Position der Führung130 genutzt werden. Die relati ve Position des Körperteils, wie z.B. über den Wirbelprozess in ähnlicher Weise determiniert, wird durch die Nutzung von ähnlichen Emittern122 , welche an dem Referenzrahmen120 in mechanischer Zusammenwirkung mit dem Rückensegment befestigt wurden bestimmt. Wie im Stand der Technik wohlbekannt und in der Druckschrift PCT/US95/12894 beschrieben, basierend auf der relativen Position des Wirbelsegments unter Führung130 (wie z.B, durch das Berühren eines bekannten Referenzpunktes) würde der Computer einen präoperativen Scan illustrieren – wie z.B. die entsprechende CATscanscheibe (CATscan slice) – und zwar auf dem Bildschirm des Monitors106 , welcher die Position der Führung130 und des Wirbelsegmentes indizieren würde. Es sollte auch verstanden sein, dass sowohl passive als auch aktive Verfolgungstechniken durchgehend genutzt werden können. - Nachdem die Wirbelelemente in dem Wirbel registriert wurden, kann eine Führung angemessen so ausgerichtet werden, dass die Scheibe und der Knochen durch die Führung entfernt werden können, so dass ein angemessen orientierter Implantatsraum geformt wird, in welchen ein künstliches Implantat oder ein Knochendübel eingesetzt werden kann.
- Zusätzlich kann die Erfindung auch eine Methode zum Durchführen von Wirbelchirurgie mit einer Führung umfassen, welche genutzt wird um geschützten Zugang über eine Bandscheibe und die angrenzenden Wirbel zum Präparieren eines Implantatsraums herzustellen. Diese Methode umfasst die folgenden Schritte: Kontaktieren des einen Endes der Führung mit wenigstens einem elektrisch mit Energie versorgbaren Energieemitterfeldes, welches an dem angrenzenden Wirbel angebracht ist; Verwenden eines Chirurgienavigationssystems mit einem Computercontroller und einen Analog-Digital-Wandlerfeld zum Kommunizieren mit dem Energieemitter der Führung: Positionieren der Führung im dreidimensionalen Raum relativ zu einem bekannten Referenzpunkt; und Ausformen des Implantatsraums durch die Führung über den Scheibenraum und in einen Abschnitt von jedem angrenzenden Wirbel.
- In einem anderen Aspekt umfasst die Methode das Generieren eines Displays von der Position der Führung. Der generierende Schritt zeigt vorzugsweise die Stelle der Führung relativ zu den angrenzenden Wirbeln, um eine Sicht der axialen Orientierung der Führung relativ zu jedem Wirbel und/oder relativ zu den angrenzenden Wirbeln zur Ver fügung zu stellen. Der generierende Schritt kann auch die Stelle der Führung relativ zu den angrenzenden Wirbeln darstellen, um eine Schnittansicht der Führung relativ zu einer anterior oder posterior Ansicht der angrenzenden Wirbeln zur Verfügung zu stellen. Der herstellende oder generierende Schritt kann auch die Stelle der Führung relativ zu den angrenzenden Wirbeln anzeigen, und zwar um eine Querschnittsansicht der Führung in der sagittalen und coronalen Ebene oder in den Ebenen relativ zu den gegenwärtigen Trajektorie der Führung.
- Die Methode kann auch den Schritt des Emittierens eines Signals von einem Emitter umfassen, welcher an die Führung angebracht ist, wobei das Signal von einem Apparat empfangen wird und auf repräsentative Weise das Signal auf einem visuellen Display indiziert dargestellt wird. Eine bevorzugte Methode der vorliegenden Erfindung umfasst auch den Schritt des Implantierens entweder eines künstlichen Implantats oder eines Knochendübels in den Implantatsraum.
- Andere Ausführungsformen der Erfindung werden für den Fachmann nach Studium der Schrift und Anwendung der Erfindung hierdurch offenbart. Es ist auch gewollt, dass die Patentschrift und die Beispiele ausschließlich als beispielhaft gesehen werden.
Claims (38)
- Eine Führung (
130 ) in Kombination mit einem Werkzeug zur Benutzung bei der Durchführung spinaler Chirurgie zum Präparieren eines Implantatraums über angrenzende Wirbel, wobei besagte Führung (130 ) zur Benutzung in einem computerkontrollierten Chirurgienavigationssystem (10 ) unter Verwendung einer Positionierverfolgungsvorrichtung (110 ) zum Verfolgen der Positionen von besagter Führung (130 ) in dreidimensionaler Ausrichtung relativ zu einem bekannten Referenzpunkt vorgesehen ist, wobei die Führung (130 ) folgendes umfasst: einen Körper (132 ) zum zur Verfügung stellen von einem Zugang zu den angrenzenden Wirbeln, um den Implantierraum vorzubereiten, wobei besagter Körper (132 ) einen Durchgang zum Aufnehmen eines Werkzeugs durch besagten Körper (132 ) zum Ausformen des Implantierraums aufweist; und eine erste Verfolgungsvorrichtung (190 ), die an dem Körper (132 ) angeschlossen ist, um bei der Identifizierung des Ortes der besagten Führung (130 ) relativ zu den angrenzenden Wirbeln genutzt zu werden, dadurch gekennzeichnet, dass besagte Führung derart bemessen ist, dass sie durch besagte Führung gelangen kann, wobei besagtes Werkzeug eine zweite Verfolgungsvorrichtung, angeschlossen daran, zur Identifizierung des Ortes von besagtem Werkzeug relativ zu den angrenzenden Wirbeln aufweist, wobei besagtes Werkzeug derart beschaffen ist, dass es entweder ein künstliches Implantat oder einen Knochendübel (202 ) zum Einsetzen in den Implantierraum hält. - Die Führung (
130 ) aus Anspruch 1, wobei besagte erste Verfolgungsvorrichtung (190 ) zumindest eine LED (310 ) umfasst. - Die Führung (
130 ) nach Anspruch 1, wobei besagte erste Verfolgungsvorrichtung (190 ) eine elektrisch unter Energie versetzbare erste Energieverfolgungsvorrichtung (190 ) ist. - Die Führung (
130 ) nach Anspruch 1, wobei die besagte erste Verfolgungsvorrichtung (190 ) reflektierende Signale imitiert. - Die Führung (
130 ) nach Anspruch 1, wobei besagter Emitter sich in Richtung der Seite des Körpers (132 ) erstreckt, um eine Sichtlinie von besagter Verfolgungsrichtung zu der Position der Verfolgungsvorrichtung (110 ) des Navigationssystems (10 ) zur Verfügung zu stellen. - Die Führung (
130 ) nach Anspruch 1, wobei besagter Körper (132 ) eine hohle Röhre umfasst. - Die Führung (
130 ) nach Anspruch 6, wobei besagte hohle Röhre einen rechteckigen Querschnitt aufweist. - Die Führung (
130 ) nach Anspruch 1, wobei besagter Körper (132 ) ein Ende mit umfassten Erstreckungen zum Penetrieren der Wirbelsäule inkludiert. - Die Führung (
130 ) nach Anspruch 1, die des Weiteren eine scheibenpenetrierende Verlängerung (134 ) umfasst, welche sich von dem Körper (132 ) zum Einsetzen in den scheibenförmigen Raum zwischen den angrenzenden Wirbeln erstreckt und zum Tragen gegen die Endplatten (204 ) der angrenzenden Wirbel sich erstreckt. - Die Führung (
130 ) nach Anspruch 1, die des Weiteren Stifte zum Kontaktieren der angrenzenden Wirbel umfasst, so dass eine Verschiebung der angrenzenden Wirbel während der Präparation des Implantierraums, verhindert wird. - Die Führung nach Anspruch 1, wobei besagte erste Verfolgungsvorrichtung als eine dynamische Referenz während der Chirurgie verwendet wird.
- Die Führung nach Anspruch 1, wobei besagte Führung zum Kontaktieren auch im Sinne von Klemmen und Durchdringen der angrenzenden Wirbel während der Benutzung der Präparation des Implantierraumes benutzbar ist.
- Die Führung nach Anspruch 1, wobei das Werkzeug aus einer Gruppe von Werkzeugen wie einem Distraktor, einem Dorn, einem Einsetzer und wie einer Reibahle selektiert wird.
- Die Führung (
130 ) nach Anspruch 1, wobei die besagte erste Verfolgungsvorrichtung zum Identifizieren der Orientierung von besagter Führung (130 ) benutzbar ist und besagte zweite Verfolgungsvorrichtung zum Identifizieren der Tiefe von besagtem Werkzeug benutzbar ist. - Die Führung (
130 ) nach Anspruch 1, wobei besagte erste Verfolgungsvorrichtung eine aktive, Verfolgungsvorrichtung aus einer Gruppe wie akustischen, magnetischen, elektromagnetischen, radiologischen, lichtemittierenden Vorrichtungen und mikropulsierenden Radarsystemen selektiert wird. - Die Führung (
130 ) nach Anspruch 1, wobei besagte erste Vertolgungsvorrichtung eine passive Vertolgungsvorrichtung mit reflektierenden Oberflächen ist. - Ein System zur Benutzung während der Durchführung von spinaler Chirurgie zum Präparieren eines Implantierraumes über einer spinalen Scheibe und angrenzenden Wirbeln, wobei besagtes System folgendes umfasst: eine Führung (
130 ) zum Vorsehen von einem Zugang zum Präparieren des Implantatraums über einer spinalen Scheibe und in die angrenzenden Wirbel, wobei besagte Führung (130 ) eine Passage zum Hindurchführen und Aufnehmen eines Werkzeugs durch besagte Führung (130 ) zum Formen des Implantatraums aufweist, wobei an besagte Führung (130 ) eine erste Verfolgungsvorrichtung (190 ) angeschlossen ist, und zwar zur Nutzung während der Identifizierung des Ortes von besagter Führung (130 ) relativ zu den angrenzenden Wirbeln; und ein computerkontrolliertes Chirurgienavigationssystem (10 ) unter Verwendung eines Energiedetektionsfeldes zum Positionsverfolgen von besagter Führung (130 ) in einem dreidimensionalen Raum relativ zu einem bekannten Referenzpunkt umfasst wird, und dadurch gekennzeichnet, dass besagtes Werkzeug zur Passage durch besagte Führung genutzt wird, wobei besagtes Werkzeug eine zweite Verfolgungsvorrichtung angeschlossen daran zur Nutzung der Identifzierung des Ortes von besagtem Werkzeug relativ zu den angrenzenden Wirbeln umfasst, wobei besagtes Werkzeug zum Halten eines künstlichen Implantats oder eines Knochendübels zum Einsetzen in den Implantatsraum ausgestaltet ist. - Das System nach Anspruch 17, wobei besagte erste Verfolgungsvorrichtung (
190 ) wenigstens ein LED (310 ) umfasst. - Das System nach Anspruch 17, wobei besagte erste Verfolgungsvorrichtung (
190 ) eine elektrisch mit Energie versorgbare Energieverfolgungsvorrichtung (190 ) umfasst. - Das System nach Anspruch 17, wobei die besagte erste Vertolgungsvorrichtung (
190 ) reflektierende Signale emittiert. - Das System nach Anspruch 17, wobei die besagte erste Verfolgungsvorrichtung (
190 ) sich derart zu einer Seite der Führung erstreckt, dass eine Sichtlinie von der ersten Verfolgungsvorrichtung (190 ) zu der Position der Verfolgungsvorrichtung (110 ) des Navigationssystems (10 ) vorgehalten wird. - Das System nach Anspruch 17, wobei die besagte Führung (
130 ) eine hohle Röhre umfasst. - Das System nach Anspruch 17, wobei die besagte hohle Röhre einen rechteckigen Querschnitt aufweist.
- Das System nach Anspruch 17, wobei die besagte Führung (
130 ) ein Ende; umfasst, welches Verlängerungen zum Penetrieren des Rückgrats umfasst. - Das System nach Anspruch 17, das des Weiteren eine scheibenpenetrierende Verlängerung (
134 ) umfasst, welche sich von der Führung (130 ) zum Einsetzen in den scheibenförmigen Raum zwischen den angrenzenden Wirbeln und zum Tragen gegen die Endplatten (204 ) der angrenzenden Wirbel sich erstreckt. - Das System nach Anspruch 22, das des Weiteren Stifte zum Kontaktieren der angrenzenden Wirbel zum Verhindern von Verschiebungen zwischen den angrenzenden Wirbel während der Präparation des Implantierraums umfasst.
- Das System nach Anspruch 17, wobei die besagte zweite Verfolgungsvorrichtung zum Identifizieren der Tiefe der Penetration von besagtem Werkzeug durch die Führung (
130 ) genutzt wird. - Das System nach Anspruch 17, wobei besagtes computerkontrolliertes Chirurgienavigationssystem (
10 ) die Position von besagter Führung in einem dreidimensionalen Raum relativ zu einem bekannten Referenzpunkt verfolgt. - System nach Anspruch 28, wobei das besagte computerkontrollierte Chirurgienavigationssystem (
10 ) ein Display der Position von besagter Führung generiert. - Das System nach Anspruch 29, wobei das besagte computerkontrollierte Chirurgienavigationssystem (
10 ) entweder das künstliche Implantat oder aber den Knochendübel an dem einen Ende von besagtem Werkzeug basierend auf den geometrischen Konfigurationen von besagtem Werkzeug und von entweder dem künstlichen Implantat oder aber dem Knochendübel angebracht an besagtem Werkzeug, welche als geometrische Konfigurationen in das computerkontrollierte System eingegeben sind, darstellt. - Das System nach Anspruch 17, wobei besagte Führung mit besagter erster Verfolgungsvorrichtung als dynamischer Referenz während der Chirurgie verwendet wird.
- Das System nach Anspruch 17, wobei die besagte Führung (
130 ) zur auch klemmenden Kontaktierung und Durchdringung der angrenzenden Wirbel zur Nutzung während der Operation des Implantatraumes nutzbar ist. - Das System nach Anspruch 17, wobei besagtes Werkzeug aus einer Gruppe selektiert wird, wie einem Distraktor, einem Dorn, einem Einsetzer und einer Reibahle.
- Das System nach Anspruch 17, das des Weiteren ein Display umfasst, welches derart nutzbar ist, dass es die Position des Werkzeugs relativ zu den angrenzenden Wirbeln darstellt, um eine Sicht auf die axiale Ausrichtung von besagtem Werkzeug relativ zu den angrenzenden Wirbeln zu ermöglichen.
- Das System nach Anspruch 34, wobei das Display des Weiteren zum Generieren von Darstellungen der örtlichen Position der besagten Führung (
130 ) relativ zu den Wirbeln nutzbar ist, um eine Querschnittsansicht von besagter Führung (130 ) relativ zu einer vorderen und einer hinteren Ansicht der angrenzenden Wirbel zu ermöglichen. - Das System nach Anspruch 17, wobei besagte erste Verfolgungsvorrichtung zur Identifizierung der Orientierung der besagten Führung (
130 ) nutzbar ist und die besagte zweite Führungsvorrichtung zum Identifizieren der Tiefe von besagtem Werkzeug nutzbar ist. - Das System nach Anspruch 17, wobei die besagte erste Vertolgungsvorrichtung eine aktive Verfolgungsvorrichtung ist, welche aus einer Gruppe wie akustischen, magnetischen, elektromagnetischen, radiologischen, lichtemittierenden Vorrichtungen und mikropulsierenden Radarsystemen selektiert wird.
- Das System nach Anspruch 17, wobei die besagte erste Vertolgungsvorrichtung eine passive Verfolgungsvorrichtung mit reflektierenden Oberflächen ist.
