- Die Erfindung betrifft chirurgische Systeme und insbesondere Verfahren und eine Vorrichtung zum Befestigen von Gewebe am Knochen. Die Erfindung findet beispielsweise bei der Rotatorenmanschettenreparatur Anwendung.
- Für Sehnen und andere Weichteile ist ein Reißen oder ein Abriss vom Knochen nicht ungewöhnlich. Athleten beispielsweise erleiden oft "Rotatorenmanschetten"-Abrisse, wobei sich der Tendo supraspinatus vom Humerus trennt und zu Schmerzen und zum Verlust der Fähigkeit führt, den Arm zu heben und nach außen zu drehen. Die vollständige Abtrennung kann auftreten, wenn die Schulter ein schweres Trauma erleidet. Typischerweise beginnt das Reißen, insbesondere bei älteren Patienten, allerdings als kleine Läsion.
- Zur Reparatur einer gerissenen Rotatorenmanschette müssen die Chirurgen den Tendo supraspinatus wieder am Humerus befestigen. Traditionsgemäß wurde dies bisher durch das schräge Bohren von mehreren Löchern durch die größere Tuberositas, d. h. die knochige Struktur auf der Außenseite des Humerus-Köpfchens, erledigt. Anschließend werden durch diese Löcher oder durch transossäre Tunnel Nahtmaterialien gezogen, um die Sehne zu befestigen.
- Obwohl die Verwendung solcher Nahtmaterialien zur Verankerung der Sehne als "goldener Standard" bezeichnet wurde, wurde gezeigt, dass die Nahtmaterialien durch den Knochen an sich wandern, insbesondere entlang des oberen Randes der Öffnungen, die die Enden der transossären Tunnel am seitlichen Kortex der größeren Tuberositas festlegen. Dies ist besonders bei älteren Patienten auffällig, deren Knochenstrukturen schwächer und darum gegenüber einer Degradation unter der Kraft der Nahtmaterialien weniger widerstandsfähig zu sein pflegen. Eine Stand-der-Technik-Lösung bestand bisher darin, die Ränder der transossären Tunnel durch Befestigen plättchenartiger Elemente zu erhöhen, siehe Gerber et al., "Mechanical Strength of Repairs of the Rotator Cuff", Journal of Bone Joint Surgery, Bd. 76, Nr. 3, S. 371–380 (Mai 1994).

- Die Nachteile der Erhöhung der größeren Tuberositas durch Plättchen sind die Schwierigkeiten in der Positionierung und Befestigung der Plättchen. Obwohl ein endoskopischer Eingriff in der Regel bevorzugt ist, macht es die Verwendung der Erhöhungsplättchen notwendig, dass die Rotatorenmanschettenreparatur durch eine offene Operation vorgenommen wird. Ferner müssen die Plättchen mit einem chirurgischen Klebstoff am Knochen befestigt werden, um das Risiko ihrer seitlichen Wanderung im Falle eines Nahtbruchs zu umgehen.
- DE-A-31 46 634 offenbart ein System zum Zusammenhalten von Knochenteilen, so dass sie zusammenwachsen, beispielsweise nach einer Trennung durch Verletzung oder Operation. Auf der gesamten Länge der Tunnel in jedem Knochenteil werden Elemente platziert, wobei die Elemente abgerundete Ausgänge aufweisen, so dass ein Gurtdraht die Elemente ohne scharfe Krümmung verlassen kann.
- WO-A-92/10149 offenbart eine prothetische Vorrichtung zur Positionierung innerhalb eines Blindknochenkanals und zur Aufnahme eines Ligaments oder einer Sehne.
- Die vorliegende Erfindung ist eine Öse nach Anspruch 1 zum Befestigen eines Weichteils an einem Knochen, der mit einem transossären Tunnel versehen ist. Fakultative Merkmale sind in den Unteransprüchen dargelegt.
- Eine Aufgabe der Erfindung besteht in der Bereitstellung von verbesserten chirurgischen Systemen und insbesondere einer Vorrichtung zur Befestigung von Gewebe am Knochen.
- Eine weitere Aufgabe der Erfindung besteht in der Bereitstellung einer verbesserten Vorrichtung zur Rotatorenmanschettenreparatur.
- Eine damit zusammenhängende Aufgabe der Erfindung besteht in der Bereitstellung einer Öse zum Verstärken der Ränder von transossären Tunneln gegen die Kraft von Nahtmaterialien.
- Die Anwendung der Erfindung zur Verhinderung der Wanderung des Nahtmaterials durch den Knochen am Ende der Öffnung umfasst das Anordnen einer Öse in einem Ende der Knochenöffnung und das Hindurchziehen des Nahtmaterials durch die Öse sowie durch die Öffnung in dem Knochen, um das weiche Gewebe mit dem Nahtmaterial zu befestigen.
- Die erfindungsgemäße Öse kann bei einem Verfahren, wie vorstehend beschrieben, zur Verwendung bei der Befestigung des Tendo spinatus am Humerusknochen durch Anordnen einer Öse in einem Ende eines transossären Tunnels und Hindurchziehen des Nahtmaterials durch die Öse und den Tunnel, um die Sehne am Humerus zu befestigen, verwendet werden.
- Die Knochenöffnung kann vor der Anordnung der Öse expandiert werden.
- Die Öse kann einen geflaschten Kopf einschließen, um zu verhindern, dass die Öse zu weit in den Tunnel gepresst wird.
- Ein Teil des Insertionselements besitzt einen Außendurchmesser, der größer ist als ein Innendurchmesser von mindestens einem Teil des Kanals in dem expandierbaren Element, so dass die Insertion des Insertionselements in das expandierbare Element bewirkt, dass das expandierbare Element unter Erhalt eines Presssitzes mit der Knochenöffnung expandiert. Die Verbesserung umfasst weiterhin das Hindurchziehen des Nahtmaterials durch einen Kanal des Insertionselements sowie durch die Öffnung in dem Knochen, um das Weichteil mit dem Nahtmaterial zu befestigen.
- Das expandierbare Element kann im Wesentlichen zylindrisch sein und besitzt einen axialen Kanal, der zwischen seinem proximalen und seinem distalen Ende verläuft. Nach dieser Ausführungsform ist das Insertionselement gleichfalls verlängert und weist einen axialen Kanal auf, der zwischen seinem proximalen und seinem distalen Ende verläuft.
- Der Schritt des Anordnens des expandierbaren Elements der erfindungsgemäßen Öse im Ende der Knochenöffnung umfasst das Anordnen dieses Elements im Ende eines transossären Tunnels im Humerus.
- Ein Platzierungselement kann an dem expandierbaren Element fragil befestigt sein, derart, dass die Kraftausübung auf das Insertionselement die fragile Verknüpfung zwischen diesen Elementen zerbricht, nachdem diese Kraft bewirkt hat, dass das Insertionselement das expandierbare Element expandiert.
- Die Vorteile der Erfindung umfassen die Möglichkeit der Installierung der erfindungsgemäßen verstärkenden Öse über einen endoskopischen sowie über einen offenen chirurgischen Eingriff.
- Diese und weitere Aspekte der Erfindung gehen aus den Zeichnungen und aus der folgenden Beschreibung hervor.
- Kurze Beschreibung der Zeichnungen
- Ein vollständigeres Verständnis der Erfindung kann unter Bezugnahme auf die Zeichnungen erhalten werden. Es zeigen:

3A bis3E und die4A bis4C eine erfindungsgemäße Öse und ihre Verwendung bei der erfindungsgemäßen Rotatorenmanschettenreparatur. - Ausführliche Beschreibung der erläuterten Ausführungsformen

1A beschreibt eine teilweise abgerissene Rotatorenmanschette. In der Erläuterung liegt das kugelförmige Köpfchen10 von Humerus12 in der Gelenkpfanne14 , die durch die Scapula16 gebildet wird. Der Muskulus supraspinatus18 quert die Spina scapulae (nicht gezeigt) und läuft zu einer Sehne19 aus (in der Zeichnung undifferenziert), deren Ende normalerweise vollständig in einer Gelenkfläche der größeren Tuberositas22 inseriert ist. Ein Teilabriss der Rotatorenmanschette ergibt sich, wenn sich ein Teil24 der Sehne19 von der größeren Tuberositas22 ablöst, wie in der Erläuterung gezeigt. -
1B erläutert die Rotatorenmanschette von1A , die nach der bisherigen operativen Technik repariert wurde, wobei die Sehne19 durch ein Nahtmaterial26 , das einen transossären Tunnel28 passiert, der schräg durch die größere Tuberositas22 gebohrt wurde, wieder am Humerus befestigt wird. Ein weiteres Verständnis der bisherigen Operationstechnik kann unter Bezugnahme auf Gerber et al., "Mechanical Strength of Repairs of the Rotator Cuff", Journal of Bone and Joint Surgery, Bd. 76-B, Nr. 3 (Mai 1994), S. 371 et seq. erreicht werden. - Obwohl
1B die Verwendung von nur einem einzigen transossären Tunnel und einem einzigen Nahtmaterial zeigt, werden typischerweise mehrere Tunnel und Nahtmaterialien verwendet, wie in der zuvor genannten Stand-der-Technik-Druckschrift diskutiert. Das Nahtmaterial kann jeweils oberhalb (d. h. oberhalb der größeren Tuberositas) zu einer Schleife verknotet, durch die Sehne hindurch gezogen und an sich selbst festgebunden werden. Alternativ können Paare von nebeneinander liegenden Nahtmaterialien zwischen den Austrittslöchern am seitlichen Kortex miteinander verknotet werden. -
2 erläutert eine modifizierte Nahttechnik unter Verwendung einer erfindungsgemäßen Öse. Die Modifikation umfasst die Verwendung einer Knochen-"Öse"32 zur Verstärkung der Öffnung30 am Ende des transossären Tunnels28 am seitlichen Kortex der größeren Tuberositas22 . Dies verhindert die Wanderung des Nahtmaterials26 durch den oberen Rand der Öffnung30 . Es wurde festgestellt, dass sich eine solche Wanderung durch wiederholtes Belasten des Nahtmaterials während des Gebrauchs der Schulter ergibt. - Die Öse
32 umfasst einen Zylinder oder eine andere derartige massive geometrische Form (z. B. Parallelepiped), die zur Einbettung in den Humerus12 und insbesondere die größere Tuberositas22 , wie gezeigt, geeignet ist. Die Öse32 umfasst eine Kanüle34 , die mit dem transossären Tunnel28 ausgerichtet ist, durch den das Nahtmaterial26 hindurch läuft. Die Öse32 umfasst vorzugsweise auch einen Kopf36 , der über die Knochenoberfläche emporragt. Dieser Kopf36 ist geflanscht, um zu verhindern, dass die Öse32 zu weit in die Öffnung30 hineingetrieben wird, und insbesondere, um zu verhindern, dass der Kopf während der Platzierung als Ergebnis von Kräften, die durch das Nahtmaterial26 ausgeübt werden, unter die Knochenoberfläche getrieben wird. - Die Öse
32 wird in den transossären Tunnel28 geschraubt oder durch einen Presssitz, chirurgisches Epoxy oder einfach durch die durch das Nahtmaterial26 ausgeübte Kraft gehalten. In Fällen, wobei die Öse32 es erfordern, kann das Ende30 des Tunnels28 vergrößert sein, um das Einbetten der Öse in den Knochen zu erleichtern. - Die Öse
32 besteht aus einem biokompatiblen Material, d. h. einem Material, das chemisch und biologisch inert ist, wie implantationsgeeignetes Polyethylen mit hoher Dichte, Polyethylen mit niedriger Dichte (PE 6010 und PE 2030) und Polypropylen (13R9A und 23M2: alle hergestellt von Rexene, Dallas, Texas). Die Öse32 kann auch aus einem Metall bestehen, wie chirurgischer implantationsgeeigneter Edelstahl. - Bei einer bevorzugten Ausführungsform ist die erfindungsgemäße Öse
40 in einer Weise aufgebaut, die dem Knochenanker des Typs entspricht, der in US-A-5,725,529, veröffentlicht am 10. März 1998, beschrieben ist. Eine solche Öse ist in den3A bis3E erläutert. - Unter Bezugnahme auf
3A umfasst die Öse40 im Allgemeinen ein verlängertes Insertionselement42 und ein in etwa zylindrisches expandierbares Element44 . In seinem unexpandierten Zustand kann das expandierbare Element44 in einem Knochenloch45 platziert werden, d. h. das Ende30 des transossären Tunnels28 oder eine vergrößerte, am Ende30 des Tunnels28 in den Knochen gebohrte Öffnung. Das expandierbare Element44 umfasst einen axialen Kanal46 , der von Ende zu Ende verläuft, zum Aufnehmen des Insertionselements und zum Hindurchtreten lassen eines transossären Nahtmaterials. - Wie in der Zeichnung gezeigt, ist der äußere Durchmesser von mindestens einem Teil des Insertionselements
42 größer als der innere Durchmesser von mindestens einem Teil des axialen Kanals46 des expandierbaren Elements44 . Wenn demnach das Insertionselement42 in den axialen Kanal46 eingeführt wird, übt der breitere Teil des Insertionselements42 auf die Wände des expandierbaren Elements44 eine Kraft orthogonal zu Kanal46 aus. Diese nach außen gerichtete Kraft lässt das expandierbare Element44 gegen die Wände der Knochenhöhle45 expandieren und fixiert das Insertionselement42 in dem expandierbaren Elements44 und das expandierbare Element44 in einem festen Presssitz in der Öffnung45 . - Das expandierbare Element
44 umfasst eine äußere Oberfläche zum befestigten Eingriff in eine innere Oberfläche des transossären Tunnels. Die äußere Oberfläche kann glatt sein oder kann, wie gezeigt, mit einer Vielzahl von Rillen48 versehen sein. Insbesondere umfasst eine bevorzugte Konfiguration eine Vielzahl von ringförmigen Rillen zum Eingreifen in Unregelmäßigkeiten in der Knochenöffnungswand, wenn sich das expandierbare Element44 deformiert und sich während und nach der Expansion an die Knochenöffnungswand anpasst. - Das expandierbare Element
44 umfasst auch einen am Ende angebrachten geflanschten Kopf49 , um zu verhindern, dass das Element44 bei der Platzierung oder als Ergebnis von Kräften, die durch das transossäre Nahtmaterial ausgeübt werden, zu weit in den transossären Tunnel hineingetrieben wird. Der Kopf49 besitzt vorzugsweise eine abgerundete äußere Oberfläche zur besseren Verteilung der Belastungen, die durch das Nahtmaterial auf ihm lasten. - Das Insertionselement
42 ist eine im Wesentlichen verlängerte Form mit einer äußeren Oberfläche mit einer oder mehreren Auskragungen50 zum Eingriff mit der inneren Oberfläche des axialen Kanals46 des expandierbaren Elements44 . Die Auskragungen50 umfassen Teile mit äußeren Durchmessern, die, wie gezeigt, größer sind als der innere Durchmesser des Kanals46 , so dass sie das expandierbare Element bei Insertion expandieren. - Das Insertionselement
42 umfasst einen Kanal52 , der entlang der Länge des Elements von Ende zu Ende verläuft. Der Kanal ist so bemessen, dass der Durchtritt eines Nahtmaterials26 des zur transossären Nahtreparatur der Rotatorenmanschette verwendeten Typs möglich ist. - Die Öse
40 wird unter Verwendung einer Platzierungsvorrichtung des Typs, der in der zuvor genannten US-A-5,725,529 offenbart ist, in den transossären Tunnel28 ,45 eingebettet. Die Vorrichtung umfasst ein zylindrisches Halteelement54 , das das expandierbare Element während der Platzierung über eine fragile Kopplung55 hält. Wie gezeigt, hält das Halteelement das Insertionselement42 auch in Ausrichtung mit dem Ende des expandierbare Elements44 . Die Platzierungsvorrichtung umfasst ferner einen Plunger56 , der auf das Insertionselement42 Kraft ausübt und es in das fragil gehaltene expandierbare Element44 presst und bewirkt, dass das Halteelement nach beendeter Insertion von dem expandierbaren Element44 abbricht. - Ein weiteres Verständnis von Aufbau und Arbeitsweise eines Knochenankers, auf dem die Öse
40 beruht, kann unter Bezugnahme auf die zuvor genannte US-A-5,725,529 erreicht werden. - Eine nicht erfindungsgemäße chirurgische Technik verläuft nach den herkömmlichen chirurgischen Techniken zur Rotatorenmanschetten-Nähreparatur. Nach dem Bohren des transossären Tunnels
28 wird allerdings die Öse40 platziert, um die Knochenhöhle45 und insbesondere die Öffnung30 am Ende des transossären Tunnels auf dem seitlichen Kortex der größeren Tuberositas22 zu verstärken. Insbesondere ist, wie in3A gezeigt, die Öse40 im Ende der Knochenhöhle45 angeordnet. Wie in der3B gezeigt, wird der Plunger56 verwendet, um das Insertionselement42 in das expandierbare Element44 zu pressen, wobei das Insertionselement42 in dem expandierbaren Elements44 und das expandierbare Element44 in einem festen Presssitz in der Öffnung45 fixiert wird. Wie in3C gezeigt, bewirkt die anhaltende Kraft des Plungers56 nach vollständiger Insertion des Insertionselements42 , dass sich das Halteelement54 von dem expandierbare Element44 an der fragilen Kopplung55 löst. Nach Platzierung der Öse wird die Rotatorenmanschetten-Nähoperation auf regulärem Weg beendet, wobei das transossäre Nahtmaterial26 durch die Öse40 und insbesondere durch die Kanäle46 und52 hindurch gezogen wird, wie in4D gezeigt. Die unter Verwendung der Öse40 beendete Reparatur ist in3E gezeigt. - Es existieren Ösen, die anders sind als erfindungsgemäß in den
3A –3E gezeigt. Diese sind beispielsweise in den4 –7 gezeigt. - Unter Bezugnahme auf die
4A –4C ist eine zweiteilige Gewinde-Öse58 mit einem Insertionselement60 und einem expandierbaren Element62 gezeigt. Die Öse wird auf die vorstehend beschriebene Weise verwendet, allerdings wird das Insertionselement60 in das Expanderelement62 geschraubt (statt gepresst), nachdem letzteres in der Knochenhöhle platziert worden ist. Wie in den4B und4C gezeigt, pressen beim Vorschub des Insertionselements in das Expanderelement die Nutflächen63 an dem Insertionselement60 die Flügel64 an dem Expanderelement62 in die Wände der Knochenhöhle. Die expandierten Flügel wirken als Anker und verhindern, dass sich die zusammengebaute Einheit (wie in4C gezeigt) in der Höhle bewegt. Das Expanderelement umfasst einen Flanschkopfbereich, der verhindert, dass der Zusammenbau durch die Kraft des Nahtmaterials (nicht gezeigt) weiter in die Knochenhöhle hineingetrieben wird. Wie vorstehend, sind sowohl das Insertionselement60 als auch das Expanderelement62 kanüliert, um das Nahtmaterial hindurch treten zu lassen. - Vorstehend beschrieben ist eine Vorrichtung, die die vorstehend dargelegten Aufgaben erfüllt. Die Fachwelt weiß, dass die erläuterten Ausführungsformen nur beispielhaft gezeigt und beschrieben sind. Beispielsweise kann die erfindungsgemäße Öse, zusätzlich zu der Rotatorenmanschettenreparatur, in Verbindung mit anderen Weichteil-an-Knochen-Befestigungen unter Verwendung von transossären Tunneln günstigerweise eingesetzt werden, wie (als nicht einschränkendes Beispiel) bei der distalen Bizepssehnereparatur, Scaphoid-Lunatum-Ligamentreparatur und dorsale Kapsulodesis, DRUJ-Arthroplastie, RCL/UCL-Rekonstitution des Daumens, RCL/UCL-Rekonstitution der Finger, Reparatur der zentralen Schlitzes für die Boutonniere-Deformität der Hand, Reparatur der terminalen Sehne bei Mailet-Deformität der Hand, Reparatur von avulsiertem oder lazeriertem FDP/FDS, Reparatur von ATF/CG-Ligamenten zur Korrektur der lateralen Ligament-Instabilität von Ferse/Fuß, Achillessehnen-Reparatur nach Abriss, zweigeteilte Ligamentreparatur zur Korrektur der Avulsion des anterio-lateralen Kalkaneus.
Claims (5)
- Öse (
40 ,58 ) zur Verhinderung der Migration von Nahtmaterial (26 ) über den Rand einer Öffnung (30 ) in einem transossären Tunnel (28 ), wobei die Öse (40 ,58 ) aufweist: ein expandierbares Element (44 ,62 ) mit einem sich hindurch erstreckenden Kanal; und ein Insertionselement (42 ,60 ), das einen sich hindurch erstreckenden Kanal einschließt, wobei mindestens ein Teil des Insertionselements (42 ,60 ) einen äußeren Durchmesser einschließt, der größer ist als ein Innendurchmesser des Kanals von mindestens einem Teil des expandierbaren Elements, derart, dass die Einführung des Insertionselements (42 ,60 ) in das expandierbare Element (44 ,62 ) die Expansion des expandierbaren Elements (44 ,62 ) unter Erhalt eines Presssitzes mit der Öffnung bewirkt. - Öse nach Anspruch 1, wobei das Insertionselement (
42 ,60 ) einen Flanschkopf umfasst. - System, das eine Öse nach Anspruch 1 und eine Einlagerungsvorrichtung (
54 ) zum Einbringen eines expandierbaren Elements in das Ende der Knochenöffnung (30 ) umfasst. - System nach Anspruch 3, wobei das expandierbare Element (
44 ) mit der Einlagerungsvorrichtung (54 ) fragil verknüpft ist. - System nach Anspruch 4, wobei zum Einführen des Insertionselements (
42 ) in das expandierbare Element (44 ) die Einlagerungsvorrichtung (54 ) zur Anwendung von Kraft auf das Insertionselement (42 ) ausgelegt ist.
