Description
- Diese Erfindung betrifft ein zementloses prothetisches Lagerelement.
- Gegenwärtig begrenzt Verschleiß der wohlbekannten Polyethylen-Hüftpfannenkomponente in Hüften, der tibialen Komponente in Knien und anderer prothetischer Lagereinsätze die Lebensdauer dieser künstlichen Gelenke. Normalerweise umfassen orthopädische Vorrichtungen zur Gelenkkonstruktion und Gelenkrekonstruktion Polyethylenlager auf Metall oder Keramik oder Metall auf Metall für die Artikulation. Das Polyethylenlager wird häufig in einer Metallschale festgehalten, zum Beispiel bei Hüftkonstruktionen, oder einer flachen Schale bei Kniekonstruktionen. Zweckmäßigerweise werden aus Polyethylen mit ultrahohem Molekulargewicht hergestellte Lagerauskleidungen durch einen Rastsitz in der Metallunterlage befestigt.
- Die mögliche Verwendung von Elastomeren in Lagern, um eine Fluidfilmschmierung bei niedriger Geschwindigkeit zu fördern, sowie mit einem Schmiermittel geringer Viskosität, wie in einem rekonstruierten Gelenk, ist in Laborstudien veröffentlicht worden, die zeigen, dass solche "nachgiebigen" Lager verglichen mit üblichen Polyethylen-gegen-Metall-Lagern einen niedrigeren Reibungskoeffizienten liefern. Zum Beispiel in der
EP 0 698 382 , derEP 0 876 820 und derEP 0 876 821 wurde gezeigt, dass weiche Polyurethan-Sorten geeignete Materialien für nachgiebige Lager sind. - Wegen der Fluidfilmschmierung ist es weit weniger wahrscheinlich, dass sich die beiden Lageroberflächen während des Gebrauchs berühren, und somit ist der Verschleiß geringer.
- Aus einer Reihe von Gründen ist es jedoch schwierig, dieses Konzept anzuwenden. Eine Schwierigkeit liegt darin, ein Polyurethan oder ein anderes Elastomer auszuwählen, das vom menschlichen Körper nicht zersetzt werden wird, so dass man bei der Auswahl von nachgiebigem Lagermaterial große Sorgfalt walten lassen muss. Wie oben erwähnt, werden Polyethylenlager gewöhnlich durch physikalische Mittel in zum Beispiel einer Metallschale festgehalten, jedoch ist es nicht möglich, diesen Ansatz bei einem "nachgiebigen" Lager zu nutzen, weil, da seine Steifigkeit so viel geringer ist als die des Metalls, es möglich wäre, dass es aus der Schale extrudiert wird oder leicht in Bezug zur Schale bewegt wird, wodurch Schäden hervorgerufen werden. Auch würde eine geringere Steifigkeit zu einer größeren Grenzflächenbelastung führen.
- Wegen der obigen Schwierigkeiten haben die Anmelder mehrere verschiedene Ansätze ausprobiert. Anfänglich wurde zur Erleichterung des Gebrauchs und für ein konventionelles Erscheinungsbild versucht, eine weiche Polyurethan-Sorte auf einer steiferen Polyurethan-Unterlage mit dieser zu verbinden, um ein ähnliches Konstrukt wie bereits zuvor existente Polyethylenkomponenten zu bilden. Dieses Konstrukt könnte dann in Abhängigkeit von der Präferenz des Chirurgen und der klinischen Einschätzung in einer Metallschale montiert oder einzementiert werden.
- Die vorliegende Erfindung betrifft daher ein prothetisches Lagerelement, das ein elastomeres "nachgiebiges" Lagermaterial verwendet.
- Die
EP 0 698 382 beschreibt ein Lagerelement gemäß dem Oberbegriff des beigefügten Anspruchs 1, das eine aus einem starren Polymermaterial hergestellte Unterlage und eine weiche elastomere Polyurethanmaterial-Auskleidung aufweist. - Lagerelemente dieser Art können durch ein Formverfahren hergestellt werden, und dieses wird ebenfalls beschrieben.
- Ähnlich beschreibt auch die
EP 0 552 949 ein Verfahren zur Herstellung einer Hüftpfanne von besonderer Konfiguration, und bei der die innere Lagerkomponente und die Unterlage zusammen geformt werden, um eine einzige Komponente zu bilden, wobei die Unterlage steifer als die innere Lagerkomponente ist. - Die Unterlage umfasst einen teilsphärischen Hauptteil und zwei unabhängige Arme, die daraus überstehen und die voneinander getrennt sind.
- Man hat gefunden, dass die Verwendung einer Formtechnik, um solche prothetischen Lagerelemente herzustellen, mit Vorteil ausgenutzt werden kann, um Elemente zu erzeugen, die überlegene Eigenschaften besitzen und die keine maschinelle Bearbeitung auf der äußeren Oberfläche der Unterlage erforderlich machen.
- Es ist bekannt, zum Beispiel aus der
EP 0 297 789 , Lagerelemente dieser Art, die durch Zement an Ort und Stelle gehalten werden sollen, mit konzentrischen Umfangsnuten und radialen Nuten zu versehen, um zur Stabilisierung und Befestigung der Schalenversteifung an der Zementschicht beizutragen, wenn die Pfanne in die Acetabulumhöhle eingesetzt wird. Es ist verhältnismäßig einfach, solche Nuten durch maschinelle Bearbeitung herzustellen. - Die WO-99/03429 zeigt eine Hüftpfanne, die mit radial verlaufenden und überstehenden asymmetrischen Stützrippen versehen ist, und die bei den bevorzugten Anordnungen anfänglich von der Pfanne getrennt sind und anschließend an ihr befestigt werden. Die Intention dieser Konstruktion besteht darin, dass die Rippen für eine Anpassung an die speziellen Knochendefekte positioniert werden können. Die Rippen stehen über den äußeren Rand der Pfanne über und sind daher anfällig für eine Beschädigung, während die Pfanne eingesetzt wird. Außerdem kann mit dieser Konstruktion die Präparation des Acetabulums kompliziert sein. Weil die Rippen über den äußeren Rand der Pfanne überstehen, sind sie anfällig und neigen unter Belastung dazu, sich von der Pfanne zu lösen, speziell weil sie gemäß dieser Konstruktion so hergestellt werden können, dass sie sich unter Einschlaginstrumenten lösen. Die Arbeit und das Geschick, die beim Befestigen der Rippen an der Pfanne erforderlich sind, machen diese Konstruktion auch teuer in der Herstellung und bei der Operation.
- Die vorliegende Erfindung ist dazu bestimmt, ein zementloses prothetisches Lagerelement bereit zu stellen, das geformt worden ist und das sich ohne Zement in eine Acetabulumhöhle einsetzen lässt, wobei die äußere Oberfläche der Unterlage mit Mitteln zum Unterstützen einer Stabilisierung und des Einwachsens von Knochen versehen ist. Somit ist das Lagerelement gemäß der vorliegenden Erfindung preiswerter und leichter herzustellen und sorgt für eine bessere Positionierung und ein besseres Einsetzen im Knochen.
- Gemäß der vorliegenden Erfindung umfasst ein zementloses prothetisches Lagerelement eine aus einem starren Polymermaterial geformte Unterlage, die an eine Lagerauskleidung angeformt ist und diese abstützt, welche aus einem elastomeren Polyurethanmaterial hergestellt ist und welche eine Lageroberfläche aufweist, wobei die Unterlage einen teilsphärischen Hauptteil und zwei unabhängige Arme umfasst, die daraus überstehen und die voneinander getrennt sind, wobei die Unterlage mit einer äußeren Oberfläche geformt ist, die einen Rand und geformte erhabene Eingriffsmerkmale aufweist, welche mindestens zwei im Abstand voneinander angeordnete, nach oben überstehende Eingriffsrippen einschließen, die senkrecht zur Oberfläche sind und die sich vom Rand der äußeren Oberfläche des teilsphärischen Hauptteils weg in parallelen chordalen Richtungen nach innen zu den unabhängigen Armen hin erstrecken.
- Bei einer bevorzugten Ausführungsform können die Eingriffsmerkmale auch überstehende Strake bzw. Grate einschließen.
- Die Eingriffsmerkmale können von im Wesentlichen dreieckigem Querschnitt sein.
- Die Erfindung kann bei Lagerelementen, bei denen die äußere Oberfläche ihrer Unterlage von einer beliebigen gewünschten Form sein kann, für einen Eingriff mit dem betreffenden Knochen angewandt werden, und wenn das Element in Form einer Hüftpfanne oder ähnlichen Vorrichtung vorliegt, kann die äußere Oberfläche der Unterlage im Wesentlichen teilsphärisch sein, und die überstehenden Grate bzw. Strake können sich in radialen Richtungen erstrecken.
- Es wird ersichtlich, dass die Zeit und die Kosten, die mit dem Versuch verbunden sind, eine solche Oberfläche mit der genannten Art von Merkmalen und solchen Rippen maschinell zu bearbeiten, äußerst kostspielig und zeitaufwendig ist, während sie durch Anwendung der Formtechnik des Anmelders relativ leicht gebildet werden können.
- Die in der
EP 0 698 382 angegebenen Informationen können bei der vorliegenden Erfindung benutzt werden, und somit kann die Unterlage aus einem "starren" Polymermaterial mit einem Mindest-Härtewert von 65 N/mm2 hergestellt werden, und die Lagerauskleidung kann aus einem "weichen" elastomeren Polyurethan-Material mit einem Härtewert von 3,0 bis 9,0 N/mm2 hergestellt werden, und zwar bei Verwendung eines Härteuntersuchungsverfahrens BS 2782; PT3 Verfahren 365D. - Die Lagerauskleidung wird vorzugsweise an der Unterlage angeformt, um eine einzige Komponente zu bilden, und die Unterlage kann aus Polyurethan hergestellt werden, zum Beispiel Corothane 75D.
- Alternativ kann die Unterlage aus kohlenstofffaserverstärktem Kunststoffmaterial hergestellt werden, zum Beispiel aus Polybutylenterephthalat (CFR-PBT) oder einer Mischung von CFR-PBT mit Polyurethan.
- Außerdem kann das Unterlagematerial aus Polyetheretherketon (CFR-PEEK) hergestellt werden.
- Bei einer noch anderen Konstruktion kann das Unterlagematerial aus Polymethylmethacrylat hergestellt werden, wobei alle diese Materialien zum Formen geeignet sind.
- Vorzugsweise wird die Lagerauskleidung aus Corothane 80A hergestellt.
- Die innere Lageroberfläche der Lagerauskleidung kann über einen Teil derselben, der im Wesentlichen im Abstand gegenüber von einer Lücke oder Öffnung zwischen den Armen der Unterlage angeordnet ist, im Wesentlichen teilsphärisch sein.
- Vorzugsweise weist die Lagerauskleidung ähnlich wie die Unterlage unabhängige Arme mit einer Lücke oder Öffnung zwischen ihnen auf, und vorzugsweise ist die Lagerauskleidung von im Wesentlichen derselben Konfiguration wie die Unterlage.
- Alternativ kann die Lagerauskleidung im Wesentlichen halbkugelförmig sein und sich über die Lücke oder Öffnung zwischen den Armen der Auskleidung erstrecken.
- Die Erfindung kann auf verschiedene Weise ausgeführt werden, jedoch wird nun eine Ausführungsform, angewandt bei einer Hüftpfanne, beispielhaft und mit Bezugnahme auf die begleitenden Zeichnungen beschrieben, in denen:

8 eine Querschnittsansicht durch einen Teil des Elements ist. - Wie in der Zeichnung dargestellt, umfasst das erfindungsgemäße zementlose prothetische Lagerelement eine Unterlage
1 , die eine Lagerauskleidung2 (in den5 und6 am deutlichsten dargestellt) trägt bzw. abstützt. Die Lagerauskleidung2 weist eine Lageroberfläche3 auf. Die Unterlage ist aus einem verhältnismäßig starren Polymermaterial geformt, zum Beispiel Polyurethan in Form von Corothane 750D, CFR-PBT, CFR-PEEK oder Polymethylmethacrylat. Die Auskleidung kann aus Corothane 80A bestehen, und die Auskleidung und die Unterlage können durch eine in derEP 0 698 382 beschriebene Technik zusammen geformt werden, wobei die Auskleidung durch das Formverfahren mit der Unterlage verbunden wird. - Die allgemeine Gesamtform und Gesamtkonstruktion der Prothese ist ähnlich wie diejenige, die in der
EP 0 552 949 beschrieben ist, das heißt, der Hauptteil4 der Unterlage ist im Wesentlichen teilsphärisch und es sind zwei unabhängige Arme5 und6 vorhanden, die sich aus dem Hauptteil4 erstrecken. Die äußere Form der inneren Lagerauskleidung3 ist ebenfalls halbkugelförmig und passt in die Unterlage1 , jedoch ist die innere Oberfläche der Lagerkomponente nur über ihrem Hauptteil halbkugelförmig, über inneren Oberflächen7 und8 ihrer Arme9 und10 ist sie ausgespart. - Falls erwünscht, könnte die innere Lagerauskleidung
3 im Wesentlichen halbkugelförmig sein und lediglich eine dünne Trennlinie tragen, um die beiden Arme bereit zu stellen. - Die Arme
5 und6 und9 und10 sind im Abstand angeordnet, um zwischen ihnen eine Lücke oder Öffnung12 bereit zu stellen. Die Arme sind im Abstand um einen Bogen auf dem teilsphärischen Hauptteil der Unterlage und der Auskleidung angeordnet, wobei sie auf dem Rand13 auslaufen, und die Arme selbst und der Hauptteil sind zusammen im wesentlichen teilsphärisch. - Die Unterlage
1 umfasst somit eine im Wesentlichen teilsphärische Wand mit einem Rand13 , der von einer geformten Öffnung unterbrochen wird, um zwei im Abstand angeordnete Arme5 und6 bereit zu stellen. - Der Hauptteil
14 der Öffnung12 ist im Wesentlichen halbkreisförmig und weist eine Mundöffnung15 auf, welche die Unterbrechung im Rand13 liefert und welche von kleinerer Breite als der Rest14 der Öffnung ist. Die Unterlage1 ist daher im Wesentlichen hufeisenförmig. - Wegen ihrer Konstruktion und der verwendeten Materialien ist die Unterlage ausreichend flexibel, um eine Deformation des Acetabulums des Patienten hinzunehmen, ist jedoch steifer als die innere Lagerkomponente.
- Man hat gefunden, dass diese spezielle Form von Öffnung zweckmäßig und erfolgreich ist, und die Belastung wie erforderlich in das Becken übertragen wird; insbesondere stellt diese Form von Öffnung wirkungsvoll sicher, dass keine Last an einer unerwünschten Stelle in den Knochen übertragen wird.
- Die äußere Oberfläche der Unterlage
1 schließt eine Reihe von erhabenen Eingriffsmerkmalen ein, die in Richtungen vorgesehen sind, welche sich weg von und/oder hin zum äußeren Umfang13 der äußeren Oberfläche erstrecken. So liegen die erhabenen Eingriffsmerkmale in Form von überstehenden Straken bzw. Graten18 vor, die sich vom äußeren Umfang13 aus in radialer Richtung erstrecken und von dreieckigem Querschnitt sind, wie am deutlichsten in8 dargestellt. Zwischen den überstehenden Straken bzw. Graten ist die Oberfläche im Wesentlichen teilsphärisch. - Die äußere Oberfläche der Unterlage
1 trägt auch ein Paar überstehende Rippen19 , die sich in im Abstand angeordnete parallele chordale Richtungen erstrecken. - Die äußere Oberfläche der Unterlage wird mit einer Hydroxylapatit(HA)-Beschichtung plasmagespritzt, die osteokonduktiv ist und Knochenwachstum stimuliert.
- Um die Hüftpfanne in ein Acetabulum einzusetzen, wird zuerst der Knochen in die geeignete Form präpariert, und eine Reihe von dreieckigen Nuten werden vom Chirurgen in ihn eingekerbt, welche sich mit den Straken bzw. Graten auf der Pfanne decken. In ähnlicher Weise werden zwei chordale Nuten eingeschnitten, um die Rippen
19 aufzunehmen. - Die Pfanne wird nun in Position gebracht und leicht eingeschlagen, wobei die Strake bzw. Grate und Rippen sie genau so festhalten, wie es erforderlich ist. Das Gelenk wird nun wieder zusammengesetzt, und man hat gefunden, dass der Druck der Kugel der femoralen Prothese, wenn eine solche Prothese benutzt wird, oder der natürlichen Kugelform des Knochens, dahingehend wirkt, einen Druck auf die Pfanne auszuüben und sie in Position zu halten, während das Knochenwachstum stattfindet.
- Bei der beschriebenen Konstruktion wird die Erfindung an einer Hüftpfanne angewandt, jedoch kann sie an andere Arten von Prothesen angewandt werden, bei denen ihre vorteilhafte Konstruktion es gestattet, sie zu verwenden.
- Bei den oben beschriebenen Konstruktionen werden die Lagerauskleidung und die Unterlage zusammen geformt, so dass die Auskleidung durch das Formverfahren mit der Unterlage verbunden wird.
- Falls gewünscht, können jedoch die Lagerauskleidung und die Unterlage durch eine mechanische Verzahnung bzw. Verriegelung zwischen den beiden Komponenten zusammengehalten werden. So könnte die Auskleidung oder die Unterlage Elemente aufweisen, welche mit der anderen Komponente in Eingriff treten, oder sie könnten durch getrennte Mittel, wie Bolzen oder Schrauben oder eine beliebige andere geeignete mechanische Verzahnungs- bzw. Verriegelungsanordnung aneinander befestigt werden.
Claims (16)
- Zementloses prothetisches Lagerelement, umfassend eine aus einem starren Polymermaterial geformte Unterlage (
1 ), die an eine Lagerauskleidung (2 ) angeformt ist und diese abstützt, welche aus einem elastomeren Polyurethanmaterial hergestellt ist und welche eine Lageroberfläche (3 ) aufweist, dadurch gekennzeichnet, dass die Unterlage (1 ) einen teilsphärischen Hauptteil (4 ) und zwei unabhängige Arme (5 ) (6 ) umfasst, die daraus überstehen und die voneinander getrennt sind, wobei die Unterlage (1 ) mit einer äußeren Oberfläche geformt ist, die einen Rand (13 ) und geformte erhabene Eingriffsmerkmale aufweist, welche mindestens zwei im Abstand voneinander angeordnete, nach oben überstehende Eingriffsrippen (19 ) einschließen, die senkrecht zur Oberfläche sind und die sich vom Rand (13 ) der äußeren Oberfläche des teilsphärischen Hauptteils (4 ) weg in parallelen chordalen Richtungen nach innen zu den unabhängigen Armen (5 ) (6 ) hin erstrecken. - Zementloses prothetisches Lagerelement nach Anspruch 1, dadurch gekennzeichnet, dass die Eingriffsmerkmale auch überstehende Strake bzw. Grate (
18 ) einschließen. - Zementloses prothetisches Lagerelement nach Anspruch 1 oder Anspruch 2, dadurch gekennzeichnet, dass die Eingriffsmerkmale von im Wesentlichen dreieckigem Querschnitt sind.
- Zementloses prothetisches Lagerelement nach einem der Ansprüche 2 oder 3, dadurch gekennzeichnet, dass die äußere Oberfläche im Wesentlichen teilsphärisch ist und sich die überstehenden Strake bzw. Grate (
18 ) in radialen Richtungen erstrecken. - Zementloses prothetisches Lagerelement nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass das starre Polymermaterial einen Mindest-Härtewert von 65 N/mm2 aufweist, und die Lagerauskleidung aus einem "weichen" elastomeren Polyurethanmaterial mit einen Härtewert von 3,0 bis 9,0 N/mm2 hergestellt ist.
- Zementloses prothetisches Lagerelement nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass die Unterlage (
1 ) aus Polyurethan hergestellt ist. - Zementloses prothetisches Lagerelement nach Anspruch 6, dadurch gekennzeichnet, dass das Polyurethan Corothane 75D ist.
- Zementloses prothetisches Lagerelement nach einem der vorangehenden Ansprüche 1 bis 5, dadurch gekennzeichnet, dass die Unterlage aus kohlenstofffaserverstärktem Polybutylenterephthalat (CFR-PBT) hergestellt ist.
- Zementloses prothetisches Lagerelement nach einem der vorangehenden Ansprüche 1 bis 5, dadurch gekennzeichnet, dass die Unterlage (
1 ) aus Polyetheretherketon (CFR-PEEK) hergestellt ist. - Zementloses prothetisches Lagerelement nach einem der vorangehenden Ansprüche 1 bis 5, dadurch gekennzeichnet, dass die Unterlage (
1 ) aus Polymethylmethacrylat hergestellt ist. - Zementloses prothetisches Lagerelement nach einem der vorangehenden Ansprüche 1 bis 10, dadurch gekennzeichnet, dass die Lagerauskleidung (
2 ) aus Corothane 80A hergestellt ist. - Zementloses prothetisches Lagerelement nach einem der vorangehenden Ansprüche 1 bis 11, dadurch gekennzeichnet, dass es in Form einer Hüftpfanne vorliegt.
- Zementloses prothetisches Lagerelement nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass die innere Lageroberfläche (
3 ) über einen Teil derselben, der im Wesentlichen im Abstand gegenüber von einer Lücke oder Öffnung (12 ) zwischen den Armen (5 ) (6 ) der Unterlage (1 ) angeordnet ist, im Wesentlichen teilsphärisch ist. - Zementloses prothetisches Lagerelement nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass die Lagerauskleidung (
2 ) ähnlich wie die Unterlage unabhängige Arme (5 ) (6 ) mit einer Lücke oder Öffnung (12 ) zwischen ihnen aufweist. - Zementloses prothetisches Lagerelement nach Anspruch 14, dadurch gekennzeichnet, dass die Lagerauskleidung (
2 ) im Wesentlichen von derselben Konfiguration wie die Unterlage (1 ) ist. - Zementloses prothetisches Lagerelement nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, dass die Lagerauskleidung (
2 ) im Wesentlichen halbkugelförmig ist und sich über die Lücke oder Öffnung zwischen den Armen (5 ) (6 ) der Unterlage (1 ) erstreckt.
