Abstract

The template consists of a basic body divided longitudinally into two parts (2,3) with guide bores (1) whose arrangement is identical with that of a bone plate to be used and whose inner dias. are adapted to the dia. of the bone screws. For the introduction of the bores into the bone, a drill bush is insertable into a selected guide bore. The two parts are held together by releasable connections which are removed after introduction of the bone screws and mounting of the plate fixer. The drilling template provides the exact positioning of the bone bores and the checking, during the screwing in of the bone screws with collars, of the position of the bearing surface in the correct plane.

Description

The invention relates to a drilling jig for a plate fixator Fracture treatment of broken bones, as described in the preamble of Claim is described.

For the insertion of bone screws during bone fracture fixation a drilling jig for drilling the bone borehole known from a There is a drill bush with a handle and with external fixation with a Trocar is provided. After drilling the bone hole, the Drilling jig removed and the bone screw inserted. In In many cases, the creation of a bracket fixer, as used in the PL PS 1 45 631 or the DD-PS 2 58 361 is necessary. This consists of a bone plate with a series of holes for Bone screws and the corresponding ones Bone screws. These bone screws have one instead Screw head a collar and a threaded pin on one Nut is screwed on. The bone plate is spaced from Bone surface by first inserting the bone screws into the Bone fragments are screwed onto the bone plate Bundles are placed and fastened by means of the nuts. When putting on the boreholes, each borehole is exact according to the bone plate to be measured to avoid unnecessary tension in the bone. The required flatness of the contact surface of the bundles must be with a Check the ruler, the bone screws are closed accordingly correct. This type of clip fixer application requires a high level of experience and dexterity of the surgeon relatively time consuming.

The invention has for its object a drilling jig for one To develop plate fixators that depend on the bone plates to be used and the exact purpose Position the bone holes and when screwing in the Bone screws with collar control the position of the Support surfaces allowed in one plane, so that the Bone fracture fixation facilitates good reduction of the Bone fragments are reached and the operation time is shortened can.

According to the invention the object is achieved in that the drilling jig one longitudinally divided into two parts and detachable by one or more Connected basic body with a series of connections Guide bores, their arrangement with that of the Bone plate and its intended use is identical and their Inner diameter is adapted to the diameter of the bone screws, and from a drill bushing that can be inserted into the guide bores There are holes in the bone.

Preferably, sockets for Kirschner wires can be used. The longitudinal division runs in the middle of the Holes. The detachable connection can be made with one part firmly connected grub screw that with one in the other part arranged and opposite bore is engaged, and insist on a screw-on screw cap. Another Detachable connection can be made from a handle designed as a clip consist of one arranged in the ends of the two parts Dowel pin is centered and has a sliding clamping ring.

The advantages of the solution according to the invention are that on the one hand the bone screws in one by the dimensions of the Bone plate certain position to each other to make a good Achieve reduction and unnecessary tension in the bones avoid. On the other hand, the bone screws can be inserted in this way be that the contact surfaces of the bundles for the bone plates in lie on one level. This eliminates the otherwise necessary Correction work. For the application of compression is one Deviation of the contact surfaces from the level necessary over the Drilling jig can be determined and adjusted precisely. Here too elaborate measuring and control procedures are eliminated.

In der zugehörigen Zeichnung zeigtThe invention is intended to be explained below using an exemplary embodiment are explained in more detail. In the accompanying drawing shows

Fig. 1 die Bohrlehre in Seitenansicht und Fig. 1, the drilling jig in side view

Fig. 2 die Bohrlehre in Draufsicht. Fig. 2 shows the drilling jig in plan view.

The drilling jig consists of a basic body with a series of guide bores 1 . The length dimension of the base body is designed depending on the bone plates to be used. The arrangement and the spacing of the guide bores 1 corresponds to the “neutral” or “compression” application and to the individual bone plate types for the various skeletal sections. The base body is divided in the longitudinal direction in the middle of the guide bores 1 into two parts 2 and 3 . Parts 2 and 3 are held together by means of two releasable connections which are arranged in the end regions of the base body. In Fig. 2, a releasable connection is designed as follows: The part 2 is provided with a threaded pin 4 and a bore 5 , which are each arranged at the opposite ends. The part 3 is also provided with a threaded pin 6 and a bore 7 , the threaded pin 4 engaging with the bore 7 and the threaded pin 6 with the bore 5 when the base body is joined. A screw cap 8 can be screwed onto the threaded pins 4 and 6 . In a further embodiment, not shown, the releasable connection, which is arranged at both ends of the base body, consists of a grip piece designed as a resilient clip, which is centered in parts 2 and 3 by means of a dowel pin and opened by means of a clamping ring which can be displaced via the grip piece and can be closed.

The drilling jig also includes two sockets for Kirschner wires that are inserted into the outer guide holes. With the Kirschner wires, the drilling jig is over the breaking point fixed.

The guide bores 1 have an inner diameter which allows the bone screws 10 to be inserted into the fragments of the bone 9 to be guided when screwed into them. The same applies to the tap drills when cutting the thread in the bone holes beforehand. For the drilling of the bores into the bone 9 , a drill bushing 11 is provided with a handle, the outer diameter of which corresponds to the inner diameter of the guide bore 1 . To apply the plate fixator, whose bone plate is held at a distance from the bone or skin surface by the bone screws, the drilling jig is used in the clamped state. After the fragments have been reduced, the drilling jig is put on, the sockets are inserted into the outer guide bores, bone bores are made and the drilling jig is fixed using Kirschner wires. To drill the holes in the bones, the drill bushing is first inserted into the guide holes near the fracture and then further worked outwards. After removing the drill bushing, a thread can be inserted into the bone hole using a tap that is guided in the guide hole. The bone screw is then screwed in until its collar comes to rest on the surface of the base body. In this way, all bone screws are inserted into the free guide holes. Then the Kirschner wires and sockets are removed and the bone holes and the bone screws are inserted in their place. To generate compressions in the breaking point, the bundles of the bone screws are screwed in at some distance from the surface of the drilling jig. Now the bone plate can be attached to the bone screws using nuts or spacers, i.e. the clamp fixator can be set up. After its completion, the detachable connection is opened and the two parts of the drilling jig are removed from under the collars of the bone screws.
