Abstract

The essential feature of the device for fastening a catheter (10) on the skin surface is the formation of a hole (17) in a lower plaster (11) onto which an upper plaster (12) can be stuck. The hole (17) is positioned above a puncturing site (22) for insertion of a catheter (10) in the tissue, so that the puncturing site (22) can be visually checked after lifting the upper plaster (12). A foamed material plug (21) impregnated with a bactericidal substance projects through the hole (17) in the lower plaster (11) and onto the section (10a) of the catheter (10) emerging from the puncturing site (22) and protects it against bacterial invasion. <IMAGE>

Description

The invention relates to a device for Attach a catheter or cannula to the Skin surface consisting of a plaster and a plaster with a layer of pressure sensitive adhesive.

Securing the extracorporeal end of catheters and cannulas that come in after puncture through tissue a body cavity, e.g. a blood vessel are necessary for changes in location, Ab prevents kinking and constriction of the catheter and the lumen of the catheter for the liquid flow is unrestrictedly available. To Attachment of the catheter to the skin are made in one the simplest case used adhesive plasters. Here it is however awkward that in a deliberate situation  Modification of the catheter for reasons of strength often extensive bonding to the skin is solved, the Catheter removed from the adhesive plaster and then the catheter tube again on the patient's skin must be glued. Furthermore, it is a practiced one Practice the catheter at the skin exit site more loop around with a surgical thread and to sew this on the skin. However, this creates an increased risk that the catheter through the relative sharp thread when knotting or later by pulling and movements of the patient cut off and the in Vascular catheter section with the blood stream is washed into the heart, resulting in a life-threatening Situation leads.

To improve the attachment of a catheter a clip with two clamps was placed on the skin surface plates created (DE-GM 82 04 827), the bracket elements for clamping the catheter tube point and either sewn on the skin or with Plasters are stuck to the skin. The start and attaching such clips is cumbersome. For Catheters of different diameters have to be ver different types of clips with different widths Clamp elements manufactured and kept in stock and protection against bacterial contaminants These clips do not offer the puncture site.

To avoid bacterial contamination in infusion therapy bacteria-retentive 0.2 µm Filter used. However, such filters can only Protection against contamination inside the infusion systems. A much larger clinical trial However, interpretation has an ingress of germs, especially of  Skin germs, at the puncture site. Even a worry Wrong catheter care can cause germ migration at the Do not completely prevent the outside of the catheter. It is therefore the one in the known fixing system gangs mentioned type (DE-OS 36 43 985) provided that Under-plaster bactericidal on one or both sides to be fabric-coated. With this well-known before The first step is the plaster on the skin glued to the surface, then the plaster is attached pierced a sharp steel cannula, and on the steel cannula or one through the Steel cannula inserted catheter tube after removal the steel cannula around the passage point on the Ver plasters with a bacteria-proof adhesive seals. Then the top paving is on the through leadership and sealing point glued. This before walking is risky because piercing the bottom plasters particles are punched out into the blood can get on track. It is also a control of the Puncture site only after removing the plaster from the skin surface and related movement of the Catheter at its point of exit from the skin Lich. There is therefore a risk of avoiding it of trauma and contamination risks only is rarely checked, although a frequent check the puncture site, especially for longer periods Catheters, would be necessary.

The invention has for its object a simple Device to be used for fastening a Form catheter or cannula so that visual inspections of the puncture site possible without any problems are.

This object is achieved in that a hole is formed in the plaster.

In this way, the plaster can be placed on the inserted into the tissue, flat on the skin stick catheter or cannula that the Hole is above the puncture site, so that at Detachment of the plaster from the plaster The puncture site is exposed and visually checked can be. Then the top plaster is again glued to the plaster and the punctures position is closed again. In this process the catheter remains immobile under the plaster lie and changes its position on the skin exit point not. The tissue is protected. A flexible catheter can be from the puncture site U-shaped, so that the glued bottom paving two substantially parallel sections of the Catheter on the skin surface. It will this increases the risk of the catheter kinking the exit point decreased and by double Fixation ensures a better hold.

It is advantageously provided that the top plaster carries an elastic plug that goes into the hole of the plaster engages appropriately and that the plaster plaster has a layer of foam whose thickness corresponds essentially to the height of the stopper. The rubber elastic plugs penetrating the hole on the The surface of the pavement presses against the Catheter or the cannula at the puncture site and fixes it there without squeezing it. Furthermore the rubber-elastic plug as a means of ver Avoid bacterial contamination of the puncture be used. For this purpose, the  Plug a bactericidal ingredient that is long lasting effectiveness on the puncture site works. Chlorhexidine is particularly suitable for this, Poly (1-vinyl-2-pyrrolidone) iodine, colloidal silver or silver ions, preferably silver chloride or Silver nitrate, preferably in a matrix Polyethylene glycol, gelatin, collagen or elec tassel angel, are bound. The stopper acts substance-containing reservoir that enters the germ, especially of skin germs, at the puncture site prevents.

In an advantageous embodiment of the invention, this Place a flat film on the plaster on which the stopper is attached and which at an edge area with the Foldable foam layer of the under-plaster is bound. The result is a one-piece design from Ober pavement and plaster, which are also hand facilitated the device. The pressure sensitive adhesive layer of the top plaster film is equipped that multiple removal and sticking of the Overlap is possible, so that repeated Kon troll the puncture site with constant Ver final effect of the top plaster are carried out can. The rubber-elastic plug can be soaked again with bactericidal substance.

The device for attaching the extracorporeal end of a cannula or a flexible catheter 10 on the skin surface of a patient consists essentially of a rectangular plaster 11 , the one transverse edge 13 of which is foldably connected to a rectangular plaster 12 of the same size.

The under-plaster 11 has a foam layer 15 which is provided on the underside with a pressure-sensitive adhesive layer 16 which extends over its entire surface. A circular cylindrical hole 17 is formed on its longitudinal center line, preferably in the area of the one transverse half of the under-plaster 11 , which penetrates the foam layer 15 and the pressure-sensitive adhesive layer 16 and is open at both ends.

The top plaster 12 consists of a film 19 of much less thickness than the foam layer 15 of the plaster 11 . The film 19 is fixed to the transverse edge area 13 of the device behind a crease line 23 with the foam layer 15 . At the opposite transverse edge, the film 19 is elongated ver, this projection is a grip tab 20 . The lower surface area of the film 19 is permitted with the exception of the tab 20 and the lateral edge region 13 with a pressure-sensitive adhesive layer 18, the adhesive of the lower patch adheres to the smooth Oberläche of the foam layer 15 11, wherein the composition of the pressure sensitive adhesive layer 18 is chosen such that a multiple opening and closing of the device is possible. A circular cylindrical stopper 21 is attached to the pressure-sensitive adhesive layer 18 . The height of the plug 21 corresponds to the thickness of the foam material layer 15 and the shape and dimensions of the hole 17 of the plaster 11 are adapted so that the plug 21 projects through the hole 17 when the device is closed and fills it. The plug 21 is made of soft foam. It is soaked in a bactericidal substance with long-lasting effectiveness.

The flexible catheter tube 10 is inserted in the usual way after puncturing the skin through a puncture point 22, for example into a blood vessel and laid in a U-shaped arch. Then the under-plaster 11 is glued onto the two sections 10 a and 10 b of the catheter 10 in the manner shown in FIG. 1, after a protective cover sheet has been removed from its pressure-sensitive adhesive layer 16 . The plaster 11 is glued on so that the hole 17 is above the puncture site 22 and the two catheter sections 10 a and 10 b are fixed substantially parallel to each other. As FIG. 2 shows, the foam layer 15 is pressed in somewhat by the section 10 b of the catheter 10 . The strength of the foam material of the foam layer 15 is chosen so that the foam yields even under the pressure of very thin-walled flexible catheter 10 and does not restrict the catheter lumen. The same property must have the foam of the plug 21 , which bears through the hole 17 of the plaster 11 through against the top of the catheter 10 at the puncture site 22 .

The device is sterile packed with plaster and plaster 11 , 12 on top of each other. To be placed on a threaded catheter 10, the pressure-sensitive adhesive layer 16. Protective cover sheet is peeled off, and it is the overplaster lifted 12 some of the subsurface 11 to watch 22, the puncture site through the hole 17 of the lower patch 11 and the lower patch 11 accurately with the hole 17 to be able to attach over the puncture site 22 .

To check the puncture site 22 , the top plaster 12 is pulled up to the crease line 23 as often as required and then glued back onto the lower plaster 11 , so that the puncture site 22 is sealed by the stopper 21 .
