Description

The invention relates to an intravenous filter as well as a device and a method for preparation Preparation of the filter for the operation.

The invention can be used for prophylaxis of thrombo embolism of the pulmonary artery through the transcutaneous Implantation of the intravenous filter with access ver. via the clavicle or thigh vein be applied.

Das ganze Filter hat eine große Eigenmasse (US-PS 39 52 747).An intravenous filter is already known Treatment and prophylaxis of thromboembolism Pulmonary artery, which has six wire elements, each of which is curved and complicated Has shape. The wire elements are in one Socket firmly clamped. For assembling the Filters is a capsule with a cylindrical shape turns. The filter assembly is done by hand leads. Each wire element has a length of approximately 5 cm. The whole filter has a large weight (U.S. Patent 3,952,747).

The well-known intravenous filter is in its construction very complicated and bulky and has a large Dead weight on, making for its implantation surgically or the thigh vein must be exposed.

In addition, the structure and dimensions allow solutions of this filter not always, the prophylaxis of the Thromboembolism of the pulmonary artery through his Perform implantation in the inferior vena cava  especially in cases where the top of the floating thrombosis under the renal veins locally is settled. In addition, there can be a safe hold on the vein wall because of the considerable weight of the Filters is not guaranteed, a dislocation of the filter mostly in the distal direction of the lower one Vena cava occur.

An intravenous filter is also known a cylindrical socket with the internal thread and a holding device specified in this version for the retention of thromboembolism. The Holding device consists of a silicone film with Bores, with radial, from a Metal-made springs are reinforced.

The springs are in the socket with one end firmly clamped and protrude with their free end beyond the limits of the silicone film, whereby the free spring end to fix the filter with the help the feathers on the wall of the inferior vena cava is pointed.

To implant the filter in the lower cavity Reaching vein becomes a device for his Preparation used for the operation. The Vorrich tung has a guide consisting of a cylinder part and Truncated cone is assembled, and an applicator, that from a capsule to house the filter, a guide catheter connected to this, a handle and a clamp. The Stalk is inside the catheter and the capsule arranged and ver with a screw thread see to be able to hold the filter back.

When assembling the filter, the stem comes with its non-threaded end through the capsule and pulled the catheter through. After that it will Filter screwed onto the threaded end of the stem and through the truncated cone of the guide in their Cylinder part pulled in, the filter together is set. The holes of the capsule and the Guide cylinder part are combined and the filter becomes further inside by withdrawing the stem inserted the capsule, which then with the pushed filter out of the guide becomes. The assembly ends with the determination of the Stalk by a clamp (US-PS 35 40 431).

This filter has the disadvantage that blood flow interrupted in the inferior vena cava because of a closure occurs due to a filter thrombosis, what the geometric shape of the filter and the existing "dead" space on its proximal surface is conditional. It is also a reliable fix of the filter in the vein is not guaranteed. Furthermore, the Filters implanted surgically due to its design will.

With the help of the known device and the method for assembling the filter for the operation placed it inside the capsule so that its Implantation only through the right internal throttle line, d. H. is made possible by a "retrograde method". It is also a major effort for that Collapse of the filter required. When inserting The filter inside the capsule can crack out occur in the silicone film, resulting in a repeated Thromboembolism of the pulmonary artery can result.

Another intra belongs to the state of the art venous filter, which has a socket in which resilient rods made of wire are attached to the to form a network. At a distance from the frame the sticks are connected together. Then go them apart again. The end of every free stick chens is provided with a support element. The Socket has a hook for assembly the filter before it is implanted. In the area of the divergence of the spring sticks another hook is provided. The springy chopsticks have a pre between two attachment points given curvature caused by an angular turn of the Chopsticks are secured.

The filter is located within the inferior vena cava the support elements at the free ends of the Chopsticks set. The mesh of chopsticks over covers the clear width of the inferior vena cava causing thromboembolism to advance within it is prevented (see for example company brochure "Cook", Günther vena cava filter set, No. 6 000.4.85).

This known filter is with its length from 7 to 12 cm very bulky and has two "dead" rooms Fastening points of the spring sticks on the favor the formation of thrombi. The definition of the filter inside the inferior vena cava is excluded not sure and can perforate the veins wall and the hollow surrounding organs.

For assembling the filter for the operation uses a device that uses a guide catheter, a device for introducing the catheter, a hook to fix and another hook to go back  hold has.

The retention hook is passed through the catheter pulled, with the hook on the socket of the filter in Brought in and returned to the inside of the catheter pulled, after which the filter is ready for the operation is.

The invention has for its object an intra venous filter as well as a device and a ver drive for the preparation of the filter for the To develop surgery that has a perforation of the Vein wall as well as the hollow surrounding organs during exclude the definition of the filter.

The task is solved with respect to the filter in that in the intravenous filters for treatment and / or prophylaxis pulmonary artery thromboembolism, one cylindrical socket and one in this cylindrical Mounted device for retaining the Thromboembolic contains that consists in the form of a sentence springy chopsticks are made, each with one end fixed in the cylindrical socket is pinched and fixable on the wall of the inferior vena cava is, according to the invention, the resilient rods in the cylindrical socket are clamped in such a way that they have a near the cylindrical frame form cylindrical surface that gradually turns into a conical Surface merges, while the free end of each resilient rod carries an L-shaped gripper, which faces the cylindrical frame and in is directed outwards with respect to the conical surface.

Any L-shaped gripper can with respect to the axis  of the intravenous filter. The feathers the chopsticks can have different lengths.

It is advisable that each spring de chopsticks a variable along its length Has cross-section, being in the area of attachment of the spring rod in the cylindrical frame it has a round cross section while it is on the route adjacent to the L-shaped gripper has a flat shape. The intermediate section of the Chopsticks with a round and flat shape unifying cross section.

The spring sticks and the cylindrical frame can be biologically inert from one and the same Material.

Properly the most convenient is the number of springy Chopsticks considering the inside diameter of the inferior vena cava and to enable the back consider one still dangerous for the life of the sick thromboembolus of the smallest permissible size to choose between ten and sixteen.

The object is achieved with respect to the device in that in the device for preparing the intravenous Filters for the operation, one from a cylindrical existing partial tube and a truncated cone Guide tube and a Capsule for housing the intravenous filter, a guide catheter connected to the capsule inside the guide catheter and capsule housed and a threaded nose for restraint applicator provided with a stem of the intravenous filter and a forceps clamp interacting with the stem  contains, according to the invention, the guide tube with the Capsule of the applicator a separable connection forms, the applicator capsule with the cylindri The partial tube of the guide tube is united and with the same interior has diameter.

The task is solved with regard to the method by that in the procedure for preparing the intra venous filter for the operation, which be in it stands that the stem through the guide catheter and the capsule pulled through is put on the stem of the intravenous filter screws and this into the applicator capsule is drawn in, according to the invention, before the intra venous filter is screwed on, the cylindrical Partial tube of the guide tube with the applicator capsule ver is united, the threaded nose of the stem through the guide tube pulled outwards and then onto this the intravenous filter is screwed on, after which the intravenous filter through the truncated cone and the cylindrical tube part the guide tube into the interior of the applicator capsule and from the applicator capsule the guide tube is removed.

The technical solution according to the invention enables a reduction in the risk of injury to the surgical intervention and a shortening of the after period of operation and the time of rehabilitation the sick, since the filter starts earlier and fuller recanalization of a thrombver stuffed venous channel favored.

The intravenous filter for the treatment and / or prophylaxis of thromboembolism of the pulmonary artery has a cylindrical version 1 ( FIG. 1), which has a thread 2 , and a device 3 for retaining thromboembolism. This locking device 3 consists of a set of resilient rods 4 , each of which is firmly clamped with one end in the socket 1 . The number of resilient rods 4 is chosen with due to the inner diameter of the inferior vena cava and the reluctance of a thromboembolus of the smallest permissible size, which is still dangerous for the life of the patient, with the filter between ten and sixteen pieces. If the number of rods 4 is less than ten, thromboembolism can no longer be safely retained, whereas if the number of rods is more than sixteen, the rods 4 cannot be installed in the cylindrical socket with regard to their fixing, since the socket does not have a diameter has more than 3.2 mm.

Fig. 2 shows a variant of the intravenous filter with twelve resilient rods 4 , which has the most beneficial effect in terms of the number of rods.

The resilient rods 4 are laid near the socket 1 so that they form a portion of a cylindrical surface ( Fig. 1), which converts when the rods 4 come together into a truncated cone surface. At the free ends of each rod 4 , an L-shaped gripper 5 is provided to fix the filter within the vein.

The gripper 5 faces the socket 1 and is directed outwards with respect to the conical surface. Each gripper 5 is also bent by a certain angle with respect to the axis of the filter, as shown in FIG. 2, where the rods have different lengths and alternately form two rows of rods. Each rod 4 has a variable cross-section along its length ( Fig. 3). In the area of its attachment within the socket 1 , the cross-section 6 has a round shape ( Fig. 4). The rod section resting on the gripper 5 has a cross section in flat shape 7 ( FIG. 5). The round rod cross-section 6 gradually merges with the flat shape 7 at the intermediate section of the rod 4 .

The cylindrical socket 1 and the resilient rods 4 are produced from one and the same biologically inert material, for example from stainless steels, which have a modulus of elasticity in the order of 205,000 N / mm 2 . By using one and the same material for the manufacture of the cylindrical socket 1 and the resilient rod 4 , an electrochemical reaction in the patient's organism is avoided, which could lead to the destruction of the intravenous filter introduced.

To prepare the intravenous filter for an operation, a special device is used, which has a guide tube 8 ( FIG. 6) and an applicator. The applicator has a capsule for receiving the intravenous filter 10, a rigidly fixed to the capsule 9 guide catheter 11, an inside of the catheter 11 and the capsule 9 through solid and a threaded nose 13 propertied for retaining the filter 10 handle 2 as well as with the connecting piece 15 of the catheter 11 interacting forceps clamp 14 .

The guide tube 8 forms a separable connection with the capsule 9 , so that the guide tube 8 can be easily removed from the capsule 9 .

The pliers clamp 14 has a slot 16 through which any angular adjustment of the handle 12 is avoided during the operation, a collet 17 and a nut 18th

The guide tube 8 consists of a cylindrical partial tube 19 and a truncated cone 20 . The capsule 9 is connected to the cylindrical tube 19 and has the same inner diameter.

Fig. 7 shows the device with retracted into the capsule filter 10 9. The stem 12 ( FIGS. 6, 7) is pulled through the capsule 9 and the guide catheter 11 with its one end 21 . The cylin drical tube 19 of the guide tube 8 is united with the open end of the capsule 9 , the inner space 22 of the guide tube 8 being an extension of the interior 23 of the capsule 9 . The threaded nose 13 of the stem 12 protrudes beyond the limits of the guide tube 8 . The socket 1 of the filter 10 is screwed onto the threaded nose 13 . The resilient rods 4 are to be turned with their grippers 5 to the truncated cone 20 of the guide tube 8 when the filter 10 is implanted through the right internal throttle or clavicle vein (not shown in the drawing) and must be positioned away from the truncated cone 20 of the guide tube 8 6 when the filter 10 is implanted through the femoral veins, as shown in FIG .

By withdrawing the stem 12 , the filter 10 is pulled through the truncated cone 20 and the cylindrical part tube 19 of the guide tube 8 into the interior of the capsule 9 ( FIG. 7). Then the guide tube 8 is removed from the capsule 9 . The handle 12 can now be firmly clamped by rotation with the pliers clamp 14 in order to prevent the filter 10 from being accidentally unscrewed in the course of the operation.

Nach einer transkutanen Punktion bei örtlicher Be täubung wird in die Schlüsselbein- oder Oberschenkel vene eine in der Zeichnung nicht gezeigte Kanüle ein geführt, durch welche die Applikatorkapsel 9, über wacht mit Hilfe von Röntgenstrahlung und Fernseh monitor bis auf die Höhe des Körperrandes des zweiten Lendenwirbels gezogen wird.Täubung after a transcutaneous puncture under local loading a cannula, not shown in the drawing, vein into the subclavian or femoral a guided through which the Applikatorkapsel 9, monitored with the aid of X-ray radiation and television monitor to the height of the body edge of the second lumbar vertebra is pulled.

The filter 10 is then pushed out of the capsule 9 with the aid of the stem 12 and fixed within the lower vena cava with the subsequent upward pull of the entire applicator. In the following step, the applicator is separated from the implanted filter 10 by a few turns of the forceps clamp 14 counterclockwise and removed from the vein together with the cannula. The holder 1 of the filter 10 is located proximally with respect to the grippers 5 , while the bent rods 4 , which are symmetrical to the center and firmly clamped in the holder 1 , form a localization zone for a thromboembolus. Finally, an aseptic adhesive strip is placed on the puncture wound.

Fig. 8 shows the position of the filter 10 within half of the inferior vena cava 24 , in which the tip of the conical surface of the rods 4 and the socket 1 of the filter 10 are placed one behind the other in the direction of the blood flow indicated by the arrow B. Due to their shape, the L-shaped grippers 5 penetrate only through the inner surface 25 of the wall 26 of the vein 24 , without the wall being perforated during the operation and in the postoperative period.

When the thromboembolus 27 is located with the filter 10 , the grippers 5 do not move with respect to the wall 26 of the vein 24 .

The use of the present invention gives the Possibility of x-ray endovascular prophylaxis of the thrombus boembolie of a pulmonary artery after the little injuring Puncture procedure for the extremely serious patients with the to carry out floating thrombosis of the conduction veins. When the intravenous fil ters, method and the device for its preparation the time of the surgical intervention can be used for implantation shorten by 5 to 6 times what happens when the patient leaves also has a beneficial effect on the length of stay reduce the patient's treatment by 4 to 5 times and maintain the patient's ability to work since the occlusion of the inferior vena cava with its sequelae after Implantation of the intravenous filter according to the invention if not occurs.

The results of the clinical sub searches of the intravenous filter according to the invention guided.

Under local anesthesia is after performing diagnostic angiographic examinations such as heart sounding, angiopulmonography declining iliac calography percutaneous implantation of the intravenous filter Prophylaxis and treatment of massive thromboembolism Lung artery in 39 patients with the embologic throma boses of the iliofemoral and iliocal segments, including even after the declining procedure, there are 27 implantations (through the left clavicle vein 21, through the right Jugular vein six) and according to the antegrade procedure 12 implan tations (through the right femoral vein nine, through the left femoral vein three). Among the operators ten patients are 17 women (43.6%) and 22 men (56.4%) between the ages of 19 and 82.

In Abhängigkeit vom Durchmesser des infrarenalen Teils der unteren Hohlvene, der durch Durchführung der rückläu figen Voroperationsiliakalography ermittelt wird, wurden die intravenösen Filter der folgenden Abmessungsgrößen implan tiert: Depending on the diameter of the infrarenal part the inferior vena cava, which is carried out by performing the return preoperational iliacalography is determined, the intravenous filter of the following dimensions implan animals:

  • - das intravenöse Filter 23 mm bei 2 Patienten;- the intravenous filter 23 mm in 2 patients;
  • - das intravenöse Filter 28 mm bei 23 Patienten;- the 28 mm intravenous filter in 23 patients;

This angiographic intervention, i.e. H. the percutaneous Implantation of the intravenous filter lasted for the ope ill patients for 8 to 10 minutes and was considered final Eating phase of the diagnostic X-ray contrast tests like probing the right compartments of the heart, angio pulmonology and retrograde iliac calography performed.

The intravenous filter was passed through before implantation sterilized his autoclave. This sterilization of the Implantation device was in a 6% solution of the Hydrogen peroxide.

To learn the condition of the iliocal segment a control iliacography was performed in all patients Time from 1 to 6 months. The performed Ana lysis of the next and distant results of the endovascular scale prophylaxis and treatment of thromboemboilia Lung artery confirms increased reliability of the Procedure of percutaneous implantation of the intravenous fil ter. In the investigations we carried out, one occurred repeated thromboembolism of the pulmonary artery only in 1. operated on patients (2.5%) whose root cause is an In traoperation error - an incorrect definition of the fil ters - was, causing the filter to be implanted repeatedly had to. Postoperative mortality among the Surveillance patients 7.4%, mostly with comorbidities gene or with sequelae of the mas sive thromboembolism of the pulmonary artery.

Analysis of clinical angiographic and section data of the 39 operated patients confirmed that the intravenous filter is not clogged, no development of a pressure gradient in the vena cava and in 94.5% Do not under any circumstances destroy the blood flow in the line disturbs.
