The invention relates to an implant for bridging defects of the spinal cord or peripheral nerves with an essentially parallel to the longitudinal axis of the implant running canal or several for receiving sprouting connective and nerve tissue as well as blood vessels.
An implant of the aforementioned type is in the earlier patent application P 33 16 468 proposed by the applicant.
The implant of the invention is used in the event of injuries or constrictions peripheral nerves or spinal cord (interruption, irreversible crushing or Scar tissue) as a guiding or bridging rail for the locally limited traumatized neural structures. Newly sprouting nerve fibers can grow through the canals, so that a connection is established between the previously separated nerve endings can. The implant of the invention leads to the spinal cord after surgical removal a 4 mm long spinal cord segment 1. to reduce the post-traumatic edema of the nervous tissue that is both proximal and distal to the site of trauma adjoins, 2. to reduce scarring, especially of the transversely growing scar cords, in that the channels or capillaries of the implant act as a guide rail from sprouting nerves as well as from the connective tissue and the newly formed ones Vessels are used, 3. to a significant reduction in post-traumatic Necrosis in the proximal and especially in the distal nerve stump or spinal cord stump, 4. to improve neural regeneration through being in the guardrail targeted growth of sprouting axons, connective tissue and blood vessels, 5. to protect the sprouting axons from sprouting connective tissue fibers and especially in those cases in which the sheathing of the implant is slower as the internal structures of the implant is resorbed, 6. to one; reduction the formation of neuromas or fibromas at the severing points of the spinal cord or the peripheral nerves and 7. to a new formation of a circular connective tissue Wrapping structure.
The implant of the invention is characterized in that the channel or stretched monofilament fibers or threads selling the channels in the longitudinal direction exhibit.
The implant as such can consist of a tube from a physiological compatible material, e.g. a silicone tube, or several, bundled and consist of hoses encased in a larger hose.
The hose material can have an inner diameter of 0.6-10 mm and have an outside diameter of 0.9-14 mm with wall thicknesses between 0.3 and 4 mm. The material should have a natural porosity, whereby the Depending on the material, pore sizes are between 10 µm and 2 mm. If a bunch of Hoses is used, this can also be from a film with appropriate Properties to be enveloped. The individual plastic hoses can be used for hose bundles be fixed with a plastic adhesive customary in the medical field, so that the implant itself has a stable order.
As a plastic material, plastics from soft to medium can be used Consistency can be used that is flexible, temperature resistant and sterilizable are. They can be absorbable or non-absorbable as required. The plastics or the tubes can still have X-ray markings so that a later Position control is possible.
Instead of the aforementioned hoses or hose bundles, for the implant of the invention can also be used with suitable plant structures, in particular made of non-lignified, vegetable, glucose-containing materials, such as they can be found e.g. inside a corn cob.
These herbal implants have the advantage that they are additionally have numerous micro-channels in addition to the larger, longitudinal channels, which can serve to transport nutrients. The plant structure material can can also be used within individual hoses.
The plant material can continue to run in the longitudinal direction Channels in addition to the microcapillary present in the plant structure System to be created; these channels can at least partially with the monofilament Threads are equipped.
Those in the ducts of the tubes or the plant structures arranged monofilament threads, which consist of a in whole or in part resorbable microsurgical sutures are available in stretched, but not provided in an overly tensioned arrangement. They are used for support the sprouting connective and nerve tissue as well as blood vessels, so that the implant the invention acts like a guide rail for the sprouting tissue. The absorption time the material of the threads can be the same or different; the threads can too have different thicknesses.
The threads are fixed in the canals by knotting the protruding thread ends end at the canal or by fixing to e.g. cross-shaped seams arranged at the channel ends.
The implants of the invention can also additionally have an outer pocket forming slowly absorbable film for the peripherally sprouting connective tissue as Have guardrail; such wrapping films are in the earlier application P 33 16 468 described.
Further features and advantageous designs of implants of the invention emerge from the claims, the following description and the Drawings.
Es zeigen: Fig. 1 eine erste Ausführungsform eines Implantats der Erfindung im Querschnitt; Fig. 2 eine zweite Ausführungsform eines Implantats der Erfindung im Querschnitt; Fig. 3 eine dritte Ausführungsform eines Implantats der Erfindung im Querschnitt; Fig. 4 eine vierte Ausführungsform eines Implantats der Erfindung im Querschnitt.1 shows a first embodiment of an implant in FIG Invention in cross section; Fig. 2 shows a second embodiment of an implant of Invention in cross section; 3 shows a third embodiment of an implant from Invention in cross section; 4 shows a fourth embodiment of a Implant of the invention in cross section.
The implant according to Figure 1 consists of a plastic or. Silicone hose 1, in the channel 2 of which there are a multitude of longitudinal, elongated, punctured surgical threads 3 shown are provided. Such an implant is suitable e.g. to bridge defects of peripheral nerves; the implant diameter therefore depends on the diameter of the nerve. The representation of the implant according to FIG. 1, like those of FIGS. 2 and 3, is thus enlarged.
The main structure of the implant according to FIG. 2 consists of the interior 4 of a corn cob with the more or less regularly arranged channels 2. The channels are partially filled with longitudinal monofilament threads 3. the Microchannels present in the inner material are not shown.
The implant according to Figure 3 comprises a plurality of smaller and larger plastic hoses 6.7 in a bundled arrangement, which are located within a Plastic casing 8 are located. The ones formed by the plastic tubes 6.7 Channels are partially filled with threads 2.
The implant according to FIG. 4 also has the larger and smaller ones Plastic tubing 6, 7, but essentially comprises two layers from the outer film 1 as a cover and an inner film 8. One such Construction is particularly beneficial for spinal cord implants, as in the -complicated neuronal Conduction systems of the spinal cord at the point of trauma die appropriate sprouting nerve fibers can find connection again with the right fibers.
Structures according to Figures 3 and 4 can be produced by large and small tubes are placed next to each other on a flat sheet or glued and then the film rolls up and at their seams glued.
