Abstract

The surgical bone core extraction instrument has a cutter (4) enclosing shell (8) with parallel support feet (15) on which the cutter is movable. The shell has axially parallel incisions (9) which define tongues (10) which are elastically biased against the cutter shaft.

Description

The invention relates to an instrument for removing dowel-like cartilage-bone blocks with a circular Lindriger cutting device, rotatable about its longitudinal axis stuff, the front edge of which forms a cutting edge.

Such an instrument is, for example, from the DE 42 08 089 C2 known, it is in this document described a ring drill that ring to manufacture shaped holes in bone is used, for example wise for the treatment of compression fractures.

Such instruments are also used to make egg an undamaged bone area a dowel-like Block can be seen with a knot on its surface skin layer and in the lower part a bony Has base. Such cartilage-bone blocks are in damaged bone areas in correspondingly pre formed holes to insert cartilage islands there form from which the cartilage growth on a ge damaged bone surface.

It is important that the cartilage layer is on the bony base in as uniform a thickness as possible is present and as perpendicular as possible to the longitudinal axis of the  Cartilage-bone block runs. Especially with ar throscopic removal of the cartilage-bone blocks it is extremely difficult to put the cutting tool in the necessary way perpendicular to the bones surface because the view through a very small incision is usually limited.

It is an object of the invention, a generic In training instruments so that even with arthroscopic Block removal on a vertical as possible the bone surface in the area of the sampling point is possible is.

According to the invention, this object is achieved by an instrument of the type described above, which is known is drawn by a surrounding the cutting tool Sleeve with three against each other in the circumferential direction sets on their front edge arranged axially parallel Support feet that move longitudinally on the cutting tool is stored and from an advanced position lung, in which the support feet the cutting edge of the cutting protrude tool into a retracted position is slidable in the cutting edge of the cutting unit Stands over the support feet, with the sleeve its end facing away from the support feet through paraxial lele incisions formed, elastic against that Cutting tool bears spring tongues.

With the help of such on the cutting tool axis parallel sliding sleeve it is possible when on set the instrument to advance the sleeve as far ben that the support feet protrude beyond the cutting edge.

The instrument can be perpendicular to the surface bring up to the tapping point, taking the three supports feet come to rest on the surface when the in instrument stands exactly perpendicular to the surface. Also when viewed arthroscopically with a narrowed Ge view, it is possible to determine whether all three Support feet rest on the bone surface as all three support feet when viewed from the side without further ado are visible, the support foot behind it through the Window between the two front support feet by. The surgeon thus has the option of before place the cutting edge on the surface of the bone Instrument exactly perpendicular to this surface judge. Only after this alignment is the cutting tool against the surface, the Support feet remain supported on the bone surface and in the subsequent drilling process together with the sleeve holding it on the cutting tool slowly push back so that the actual cutting process is not hindered in any way. It results from the spring tongues a certain fixing of the sleeve the cutting tool so that the sleeve in the once set axial position remains until under Overcoming the frictional force in another position is moved.

From US-A-4 931 056 it is known per se at Publ of the skull using a twist drill To lead tripods, which on the top of the skull is set and has a guide sleeve through which the drill reaches through. It is this  Tripod, however, is an independent component based on the skull is put on and through which the Boh is inserted through, this arrangement is one Completely unused for endoscopic operations is suitable, since it forms a separate component Do not tripod through the narrow access openings in the Body can be introduced at endoscopic Operations are available. In the fiction This is, however, a straightforward cutting tool possible because the sleeve with the axially parallel Ver support feet directly on the cutting tool is held slidably, a radial enlargement of the Cutting tool hardly occurs, and moreover ensures that the support feet on the cutting unit are held themselves, i.e. together with the Cutting tool inserted into the body and ge there can be handled.

It is beneficial if in the retracted position the spring tongues and the cutting tool by dipping protrusions in recesses ne springy locking connection.

In particular, it can be provided that the cutting unit Wears a circumferential groove in the inward facing projections of the spring tongues in the retracted Immerse the position of the sleeve in a springy manner.

This makes it possible for the sleeve to be captive the cutting tool is held in a retracted  ne position to move and in this position also to fix, so that the sleeve the use of the Cutting tool does not bother when not needed becomes.

It is advantageous if the support feet in the circumferential direction compared to their angular distance a small order have catch extension, so that the space a sufficiently large one between adjacent support feet Observation window forms. The support feet can for example in the circumferential direction over an angle stretch of only a maximum of 30% of the angular extent of such a window.

It is also beneficial if the length of the support feet between its free end and the front edge of the sleeve corresponds at least to the diameter of the sleeve, so that the height of the window is also adequate has a reliable observation of the upper bone surface and the support feet attached to it continuous

In a preferred embodiment, that the cutting tool is adjacent to the cutting edge Depth scaling bears, this scale can be compared to the Sleeve that always with the support feet during the cutting process remains supported on the bone surface together be observed so that the surgeon immediately fixed how deep the cutting edge of the cutting tool protrudes beyond the level created by the support feet is spanned.

In a particularly preferred embodiment provided that when withdrawn or removed Place a push-on sleeve on the cutting tool is slidable radially outwards over the cutting protrudes and their postponement by a Stop is limited.

In particular, the push-on sleeve is so far on that Slidable cutting tool that it in the area of Cutting edge of cutting tools ends.

Such a push-on sleeve acts as a support for the Cutting tool if this is used to insert the won a block of cartilage-bone in a prepared boh tion is placed on this. The radially outward edge of the adjoining the cutting tool The plug sleeve is supported on the edge of the prepared boh tion and prevents the cutting tool from entering the submerged this hole. It is then easily possible Lich, from the back the inside of the cutting the cartilage-bone block held in the front prepared hole to press in without that Cutting tool enters the hole and be this damage or a the insertion movement of the block be prevents wedge action.

The instrument can carry a cutting tool Have twist grip.

In addition, the inside of the cutting tool can be longitudinal a plunger can be slidably mounted, the preference protrudes in the axial direction over the rotary handle.

The instrument 1 shown in the drawing comprises a handle part 2 with a quick coupling 3 , in which an elongated, circular cylindrical cutting tool 4 is rotatably inserted. This cutting tool 4 has the shape of an elongated hollow cylinder and ends in a circular cutting edge 5 . With the help of the handle part 2 , which grips a transverse handle 6 , the cutting tool 4 can be rotated so that the cutting edge 5 on a bone 7 cuts out a zy-cylindrical cartilage-bone block from the bone 7 .

On the cylindrical cutting tool 4 , a surrounding, cylindrical sleeve 8 is longitudinally displaceable GE, which forms a number of spring tongues 10 arranged in the circumferential direction at its end facing the handle part 2 by axially parallel incisions 9 . These are bent in the area of their free ends 11 from the formation of a projection 12 inwards. When the sleeve 8 is pushed back, these projections 12 dip into a circumferential groove 13 on the cutting tool 4 , which is arranged adjacent to the handle part 2 . As a result, the sleeve 8 can be fixed in a retracted position, from which it can be displaced in the direction of the cutting edge 5 of the cutting tool 4 while overcoming a certain disengagement force for the projections 12 .

On the opposite side of the spring tongues 10 are in the wall of the sleeve 8 at the end in the circumferential direction offset three rectangular windows 14 cut, so that between these windows 14 a total of three narrow, axially parallel support feet 15 remain, which form a at the free end Support surface 16 end in a thickening 17 ( Fig. 2).

The sleeve 8 can be pushed so far on the cutting tool 4 that the support feet 15 protrude beyond the cutting de 5 . In this position of the sleeve 8 , the direction of the instrument 1 can be aligned with respect to the surface of the bone 7 from which a block of bone is to be removed. For this purpose, the support feet 15 advanced over the cutting edge 5 are placed on the surface of the bone 7 in such a way that all three support feet 15 lie on the surface. Since all support feet 15 end in a plane perpendicular to the longitudinal axis of the instrument 1 , is there ensured that the instrument 1 is aligned perpendicular to the surface of the bone 7 . It can be easily observed from the side, since all three support feet 15 can always be seen directly or through the window 14 .

After the alignment of the instrument 1 , the cutting tool 4 is advanced by means of the handle part 2 with a rotational movement against the bone 7 until the cutting edge 5 lies against the bone. The sleeve 8 moves on the cutting tool 4 in the direction of the handle part 2 , but the support feet 15 remain on the bone 7 in the system. As the cutting tool 4 is further rotated and pushed forward, the cutting edge 5 penetrates into the bone 7 and cuts out of this a block which is cartilaginous in the area of the surface and bones in areas below it. The depth of penetration of the cutting edge can be read off by a scale 18 which is attached to the outside of the cutting tool 4 and which when the cutting tool 4 penetrates the bone 7 changes its position relative to the sleeve 8 pushed back by the bone 7 . The operator therefore has a measure of the penetration depth of the cutting tool 4 at any time by comparing the scale 18 on the one hand and the position of the sleeve 8 on the other hand.

After completion of the cutting process, there is a cartilage-bone block 19 in the interior of the cutting tool 4 . The cutting tool 4 can now be used immediately for inserting this bone-cartilage block 19 into a prepared bore 20 of another bone 27 . For this purpose, when the sleeve 8 is pushed back or removed, a push-on sleeve 21 is pushed onto the cutting tool 4 , which is preferably made of plastic. The delay depth of the plug-on sleeve 21 is limited by the fact that the end portion 22 of the receptacle 21 narrows conically towards the free end. This conically narrowing end section 22 bears against the likewise conically shaped cutting edge 5 , so that the completely pushed-on push-on sleeve 21 takes a defined position on the cutting tool 4 , in which its lower edge 23 is essentially aligned with the cutting edge 5 , if necessary, this is slightly exceeded ( Fig. 4).

The outer diameter of the push-on sleeve 21 is larger than that of the cutting tool 4 and also larger than that of the bore 20 , and so the edge 23 of the push-on sleeve 21 is supported on the bone 27 and thus prevents the cutting tool 4 from entering the bore 20 .

In this position the bone-cartilage block 19 may be means of a plunger to be pushed 24 of cutting tool 4 out into the bore 20, this plunger 24 is connected via a rod 25 which constitutes the handle portion 2 by, with an operating handle 26, which enables a sensitive advancement of the plunger 24 . For this purpose, the rod 25 can also carry a scale 28 which indicates the insertion depth of the plunger 24 .

The push-on sleeve 21 can be held on the cutting tool 4 by a frictional engagement, for this purpose it carries in the region of the end portion 22 opposite edge 29 an inwardly projecting annular shoulder 30th
