Abstract
Description
The invention relates to a device for determining the optimal AV delay time corresponding to that in the upper handle of the first claim mentioned art.
When using a pacemaker of the DDD type the individual setting of the AV delay time Basis for optimization of hemodynamically advantageous loading conditions in cardiac stimulation. The exact setting the AV delay time is on the one hand for older people, where the stroke volume to a greater extent from the ven ticular filling quantity depends and on the other hand with frequent Heart defects, where there is already a slight increase in heart rate initial value by generating an attractive contribution to can lead to significant clinical improvements, from be of particular importance.
The currently used to set an optimal Devices used for AV delay time rework the method of Doppler echo cardiography. You assign the Disadvantage that their use is particularly time consuming is and the achievable results are also essential from experience - and thus from a subjective one estimate - depend on the attending physician.
It is also disadvantageous that those with the above results are achieved which not required for certain clinical applications have accuracy.
Aus der DE-OS 31 04 398 ist darüberhinaus ein Verfahren nebst Vorrichtung zur Maximierung des Schlagvolumens eines schrittmachergesteuerten Herzens bekannt, bei welchem die optimale AV-Verzögerungszeit in Abhängigkeit von der Ände rungstendenz aufeinanderfolgender Impedanzänderungen zwi schen zwei im Bereich des Herzmuskelgewebes angeordneter Elektroden nach attrialer und ventrikulärer Stimulation durch den Herzschrittmacher eingestellt wird.DE-OS 31 04 398 is also a method together with a device for maximizing the stroke volume of a pacemaker controlled heart, in which the optimal AV delay time depending on the change tendency of successive changes in impedance between two arranged in the area of the heart muscle tissue Electrodes after attractive and ventricular pacing is set by the pacemaker.
The main disadvantage of this method is that as device two, by an additional operati electrodes to be positioned in engagement are, which are only the detection of the impedance or Change in impedance when stimulating the be between them serve sensitive heart muscle tissue.
In clinical practice, however, there is a device for an easy to perform, inexpensive and well repro inducible determination or setting of the AV delay time desirable, which is uniform to the An is suitable in all medical facilities.
Based on the shortcomings of the prior art, the Invention based on the object of a universally applicable and easy-to-use device for determining the Specify AV delay time.
The task is characterized by the characteristics of the contractor spell 1 solved.
The invention includes the finding that a change tion of the amplitude of an arterial pulse wave both on a corresponding change in the left ventricular Stroke volume as well as a change in Aortic pressure pulse. Likewise, it is invented appropriately exploited the fact that changes in the aor ta pressure pulses well with changes in left ventricular Correlate stroke volume.
The AV delay time is the period between the Ab giving the stimulation impulses for the Attrium and the Ven particle is defined and is considered taking into account the respective ideal physical strain on the patient set that the ventricular stroke volume a maxi paint value.
The AV delay time generally has one in the Be range from 120 to 250 milliseconds, which needs to be set specifically for the patient. A pre-programmed by programming the pacemaker given value for the AV delay time can be for a Pa patients were optimal, but just for another patient still be allowed. The stimulation of the ventricle into one less than optimal time leads to a reduction in performance of the heart controlled by the pacemaker, which then only pumps a reduced amount of blood, causing the Function of important organs is impaired.
According to the preferred embodiment of the invention are in the device - taking advantage of the fact that an increase in the amplitude of an arterial pulse wave to a corresponding one, by means of a distance from the subsequent vascular stimulation time-optimized stimulation of the attraction-related, increase of the left ventricular stroke volume and vice versa is provided - means provided to change the Amplitude of the arterial pulse wave depending on the Detect and save AV delay time. The before direction also has resources with which through Comparison of the measured values of several measurements at differ AV delay times the maximum amount of changes determination of the amplitude of the arterial pulse wave and the associated value of the AV delay time is determined and is set as the optimal value for the specific patient.
The value of the maximum change in arterial pulse ampli tude corresponds to the attractive surcharge on stroke volume of the heart, which is lost when the attrium is not is stimulated, d. H. when the diastolic filling period at non-contracting attrium is constantly regulated. A Such a condition can be easily reached if a heart DDD pacemaker temporarily switched from DVI mode to VVI mode is switched. In the invention direction the necessary funds are provided for this.
To determine the optimal AV delay time, the Device according to the invention a computer unit where at the amplitudes of the arterial pulse wave related to the AV delay time in a first memory unit be cached and the difference between the last and the previous measured value or with the Measured value of the amplitude with non-contracting attrium in a comparator unit can be determined.
To check the determined value of the optimal AV delay time, the determination result is provided again with the pulse wave amplitudes compared to the as optimally determined value suddenly prolonged or suddenly Lich reduced AV delay time to compare.
To determine the change in amplitude of the arterial pulse According to a preferred embodiment, wave is invented device according to the invention provided an oximeter, which the oxygen saturation of hemoglobin spectrophotome trically recorded. A strong arterial pulse wave creates egg ne greater oxygen saturation of the arterial blood than a relatively weak one; H. by a non-optimal AV delay time conditional, pulse wave.
According to an advantageous further education of the Erfin The device has a finger oximeter, since a such device is very convenient to use and a sub jective evaluation of the measurement results in a favorable manner is not required.
According to a further variant of the invention, an after the transmission principle working finger oximeter see which be for the application of the device is particularly well suited.
The one to determine the optimal AV delay time of a DDD pacemaker device provided pulls a finger oximeter with an integrated Ana log / digital converter, whose signal output for evaluation processing the measured values and determining the optimum AV delay time connected to a programming device is.
In this programming device, the above be written memory means, the comparator and Computer unit and the means for switching the cardiac pacemaker from DVI mode to VVI mode. Two the programming device and the implanted Pacemaker is featured a telemetric connection see to the one hand the different, for the individual NEN measurement cycles required AV delay times and on on the other hand, the determined optimal AV delay time to be able to give.
According to a preferred embodiment of the invent device according to the invention has the programming device a multiplexer, with the help of a first Storage unit different values of AV delay time read out serially and telemetrically to that Pacemaker is transmitted. The multiplexer is clocking at the same time a second storage unit in which the Measured values of the ampli recorded by the finger oximeter tude of the arterial pulse wave. For the purpose of Determination of the maximum value of the amplitude change of the ar serial pulse wave in relation to the respectively set AV delay time is a comparator unit hen, on the one hand the current measured values and on the other hand, the previous measured value of the amplitudes the change can be fed. Out of yourself by comparison resulting difference in amplitude changes is the maxi Male amplitude change as maximum attractive surcharge to the stroke volume of the pacemaker-controlled heart indirectly. The value of the associated one AV delay time is the time period between at trialer and ventricular stimulation of the heart, at which reaches a maximum stroke volume.
According to an advantageous further development of the Erfin The second storage unit also contains the (through a patient-specific measurement) amplitudes value of the arterial pulse wave that arises, if the attrium has not been stimulated, d. H. if the Pacemaker from DVI mode to the short term VVI mode has been switched. The comparator unit then averages from this amplitude value and the respective ak actual measured value of the amplitude of the arterial pulse wave as Difference the absolute value of the attractive surcharge at Stroke volume, the maximum value at the optimal AV delay time is reached.
For setting the respective comparison mode at the Determining the optimal AV delay time is a first Switching device provided.
The programming device accordingly has a favorable gene further development of the invention, a second switch direction on which for the calculation of the maximum Change in amplitude of the arterial pulse wave provided Computer unit using a separate connecting line to the telemetry unit of the programming device is closable to the respectively determined optimal value the AV delay time to the pacemaker to be able to.

The device 1 has a sensor device 2 , in which a transmission finger oximeter 2.1 is provided. The measured amplitude values of the pulse wave are digitized in an analog / digital converter, transmitted to the programming device 3 via the data line 6 and further processed there in order to determine the optimum AV delay time.
The electrode line via which the stimulation pulses are transmitted from the pacemaker 4 to the heart in accordance with the respective AV delay time is designated by 15 .
In Fig. 2 is a block diagram of the programming device 3 shown in Fig. 1 in a schematic form Darge provides.
The programming device 3 has a first memory unit 9 , from which the signal equivalents of different AV delay times in the range from 120 to 250 ms can be read out by means of a multiplexer 8 connected via the control line 14 . These AV time signals he reach over the signal line 16 after passing through a first switching device 13 he a telemetry device 22 and are transmitted from there to the pacemaker (compare position 4 in Fig. 1). The value of the amplitude of the arterial pulse wave, which can be determined on the basis of this AV delay time using the oximetric sensor arrangement (compare position 2 in FIG. 1), is supplied to the programming device 3 via the signal line 6 and, in parallel, via the signal line 6.1 into a second, also stored by the multiplexer 8 , memory unit 10 AV delay time-related (the input of the AV delay time takes place via the signal line 16.1 ).
At the same time, the value of the pulse wave amplitude on the signal line 6.2 is the input signal of a comparator unit 11 in which each of the serially determined values of the arte rial pulse wave amplitude is compared with the value of the previously determined pulse wave amplitude which can be taken from the second memory unit 10 via the signal line 17 will. In the computer unit 12 , the largest amplitude (difference between two successive values equal to zero) of the arterial pulse wave AV delay time-related (the AV delay value input into the computer unit 12 is carried out by a standard software) the signal line 16.2 ) determined and then the optimal AV delay time determined.
By the second switching device 21 , the calculation mode for the optimal AV delay time can be changed. The second memory unit 10 also contains the amplitude value of the arterial pulse wave in the event that the attrium is not stimulated (switching the pacemaker from DVI mode to VVI mode) and the pulse wave is determined solely by the ventricular contraction of the amount of blood applied becomes. From the amplitude changes of the oxymetrically measured arterial pulse wave amplitudes, the maximum is determined by the computer unit as the maximum attractive contribution.
After computational determination of the optimum AV delay time, the first switching device 13 ak tivated by the computing unit 12 via the control line 19 and the value of the optimum AV-delay time can be telemetered to the pacemaker.
All other electronic components of the programming device 3 required for the adjustment of the management of the pacemaker are summarized in the assembly 23 .
The invention is not limited in its execution the preferred embodiments given above games.
