Die Erfindung bezieht sich auf einen Führungsstab für die schwere
Intubation gemäß dem Oberbegriff des Patentanspruches 1.The invention relates to a guide rod for the heavy

Führungsstäbe sind leicht biegsame Stäbe aus Kunststoff oder Metall.

Die Endotrachealtuben (a) sind biegsame Rohre, die zur Narkosebeatmung
von Patienten meistens durch den Mund in die Luftröhre geschoben werden.
Sie bestehen aus gewebefreundlichen Kunststoffen wie Gummi, Silastic
oder PVC. Die Endotrachealtuben garantieren freie Atemwege. Sie sind
in der überwiegenden Zahl mit einer aufblasbaren Blockermanschette 5
ausgestattet. Die Blockermanschette wird über einen kleinen Kanal, der
in die Rohrwand eingearbeitet ist, mit der Raumluft aufgeblasen. Der
Kanal geht in einen dünnen Schlauch 6 über, der sich außerhalb des
Patienten zu einem Kontrollballon 7 erweitert. Der Kontrollballon
endet mit einem Ventil und einem Ansatzstutzen (8) für eine Blockerspritze,
über die die Blockermanschette aufgeblasen wird. Das Ventil verhindert,
daß die Luft spontan aus dem Kontrollballon ausweicht. Die Führungsstäbe
sind ein paar Zentimeter länger als die Endotrachealtuben. Am proximalen
(aboralen) Ende bilden sie eine Biegung, daß sie nicht zu tief in den
Endotrachealtuben rutschen und daß man sie nach der Einführung des
Endotrachealtubuses in die Luftröhre wieder aus dem Tubus rausziehen

Die Führungsstäbe aus Metall (c), die einen kleinen Durchmesser haben,
enden distal (der Trachea zugewandt) mit einer Olive (2), die eingebaut
ist um eine Perforation im Hypopharyngs, Kehlkopf oder Luftröhre zu
verhindern. Proximal (aboral) haben die einen Ansatzstück (1) mit eine
Feststellschraube zur Längebegrenzung des Führungsstabes.
Die Führungsstäbe haben verschiedene Durchmesser, wobei jeder
Durchmesser als der Endotrachealtubus hat.
Vor eine Intubation kann man einen Führungsstab in ein PVC oder
Gummitubus einführen, für die Tuben nach Woodbridge ist das obligatorisch.
Mit der Hilfe eines Laryngoskops (ein Handgriff mit gebogenem Spatel,
an dessen Spitze sich eine Lichtquelle befindet; zur Darstellung und
Einsichtnahme des Hypopharyngs und des Kehlkopfeinganges) wird der
Endotrachealtubus in die Luftröhre eingeführt. Nach einer erfolgreichen
Intubation zieht man den Führungsstab aus dem Endotrachealtubus heraus,
und fixiert den Endotrachealtubus mit dem Pflaster an die Wange.
Anschließend wird der Endotrachealtubus an ein Narkosegerät angeschlossen.
In dem Fall, daß man mit einem Laryngoskop Hypopharyngs und den
Kehlkopfeingang darstellen kann, ist es nicht so schwer, den

erfolgt nun mit dem Führungsstab voran, über den der Endotrachealtubus
nach erfolgreicher Plazierung des Stabes sicher in die Luftröhre
geschoben werden kann. Der Führungsstab wird dann aus der Lufröhre
herausgezogen, der Tubus mit der Hilfe eines Pflasters an die Wange fixiert
