Abstract
Description
The present invention relates to a method for detection of specific antibodies in a biological sample Use of recombinantly produced antigens derived from solid supports immobilized transformed host cells can be expressed, after appropriate pre-adsorption of anti-host cell antigens non-specific antibodies the specific antibodies for example using immunofluorescence appropriately labeled secondary antibodies against the specific antibodies are detected.
An essential area in medical laboratory diagnostics forms the detection of antibodies against infectious agents, Allergens and autoantigens. All of these investigations require the provision of biological material in the form of body fluids (such as blood, cerebrospinal fluid, stool) or Tissue.
The detection of (auto) antibodies takes place, among other things using serological methods. Basically, doing so using known antigens specific antibodies in Patient material demonstrated. As part of this serological Processes include microscopic techniques (indirect immunofluorescence) enzymatic methods (Enzyme immunoassays), isotope techniques (radioimmunoassay) or Blot techniques used. These methods are common quickly feasible and partly because of their Automation found a wide application.
In the field of autoantibody diagnostics and detection a variety of antibodies against infectious agents indirect immunofluorescence plays an important role. there are antigens (from viruses, bacteria, eukaryotes) in the form of smears, frozen sections or cleaned up biochemically characterized substances on slides immobilized and in a first incubation step with a incubated the patient's antibody-containing sample. Are in it specific antibodies, they bind to the antigens Structures. In the second incubation step, anti-human Antibodies added to the first antibody tie. These anti-human antibodies are covalent to one Fluorescent dye (fluorescein isothiocyanate) coupled, and under the fluorescence microscope, the bound first antibody "visible".
Despite the wide range of diagnostic options available Numerous antibody detection methods are possible not yet practically feasible, not sufficient selective or specific or for use in the Routine diagnostics too complex.
For example, antibodies are used in syphilis diagnostics against Treponema pallidum, a strictly anaerobically growing gram-negative stick that belongs to the family Spirochaetaceae heard, by means of indirect immunofluorescence, enzyme or Radioimmunoassay and immunoblot detected. For the Enzyme immunoassays and immunoblots are the antigens from in Rabbit testicles grown Treponema pallidum cells cleaned or used as full lysate. With insulation and processing of the treponemen exists for the executing Personnel also face the risk of infection. recombinant Antigens produced (in particular TpN15, TpN17, TpN44.5 [TmpA] and TpN47; Nomenclature according to Norris, 1993) are published in ELISA and used in blot techniques.
In the area of indirect immunofluorescence, inactivated Treponema pallidum bacteria for the detection of antibodies against the treponemes and thus used by syphilis. Also For this procedure, the bacteria in rabbit testicles bred and isolated from it. The received Cell suspensions are immobilized on slides and such used to detect the antibodies. Because of a strong Cross reactivity of the antibodies with antigens of the human-pathogenic strain Treponema phagedenis must pre-adsorption of in the patient sera beforehand Contained antibodies against the cellular components of this tribe are directed.
Since when using immunofluorescence for syphilis Evidence so far on the use and killing of animals is dependent on what is extremely important in terms of animal welfare is undesirable, and not for laboratory staff insignificant risk potential when dealing with the pathogenic bacterial strains Treponema pallidum exist, adhere this method has significant disadvantages. So far however no alternative methods with comparable Specificity available.
Another example where no fast and reliable detection method has been found is the autoimmune hepatitis in the context of chronic Liver diseases.
Autoimmune hepatitis is a chronic inflammation that has a good chance of healing if it is recognized in time, untreated, however, for cirrhosis and a few years later for Death leads. A meaningful and timely diagnosis is therefore desirable, especially if one assumes that Approx. 100,000 new patients worldwide annually at the Autoimmune hepatitis. By initiating a low dosed immunosuppressive therapy is even at advanced stage of disease a long standing, common achieve lifelong remission.
The relevant autoantibodies for the diagnosis of autoimmune Liver diseases are diagnosed using indirect immunofluorescence, Enzyme or radioimmunoassay and immunoblot detected. As Target antigens are animal or human preparations used and recombinantly produced antigens come in the detection of Autoantibodies (in ELISA and immunoblot) against liver and Kidney microsomes and against the soluble liver-pancreas antigen (SLA / LP) are used.
Autoantibody serology is often particularly large Meaning, it should be noted that also Autoantibody negative cases can occur. When determining the Autoantibody profile of chronic hepatitis there are antibodies against a number of cellular Components (e.g. antibodies against the cell nucleus, antibodies against smooth muscles, antineutrophilic antibodies, Antibodies against liver and kidney microsomes). Mann's et al. (1987) described autoantibodies against soluble Liver pancreas antigen (SLA / LP; SLA = soluble liver antigen) on the other hand, appears almost exclusively in patients with autoimmune hepatitis to occur. By Wies et al. (2000) the target antigen SLA was identified and demonstrated that the pancreas, which is also used as a diagnostic marker Antigen is identical to SLA. Autoantibodies to soluble Liver pancreatic antigen are thus the diagnostic ones Leading antibody in autoimmune hepatitis. Although their Prevalence is only 10-30%, its predictive value is attached almost 100%.
In principle, indirect immunofluorescence is safest method for determining the autoantibody profile chronic liver disease, since it is suitable, almost all clinical pictures based on their specific Distinguish fluorescence patterns. So far, however, none fluorescence pattern clearly assigned to the SLA / LP be demonstrated, whereby the presence of a Autoimmune hepatitis through additional other procedures diagnosed or excluded.
A full differential diagnosis is mainly because of this necessary because the clinic of autoimmune hepatitis and infectious hepatitis closely resemble the therapies are diametrical. While with autoimmune hepatitis Treated immunosuppressively in chronic Hepatitis the immune system z. B. stimulated with interferon become.
For this reason, it has been necessary in addition to the profile determined with indirect immunofluorescence routinely perform SLA / LP detection using an ELISA perform. The full assessment of whether a is autoimmune or chronic hepatitis process and cost intensive as well as time consuming.
The proof poses another, as yet unsolved problem of infections with the human pathogenic Epstein-Barr virus (EBV), the causative agent of infectious mononucleosis (Whistling glandular fever). So far, the process using immunofluorescence by using human cells (Tumor cell lines) uses the parts of the EBV genome in itself wear. These cells produce alongside their own and viral ones Proteins among other things that used to diagnose a late Infection phase with the Epstein-Barr virus used EBNA- 1 (Epstein-Barr Nuclear Antigen-1; Hille et al., 1993). The However, EBV diagnosis is not always with this procedure Sufficiently secure since you are not between specific ones Antibodies against the EBNA-1 located in the cell nucleus and in the non-specific sample present against the cell nucleus can distinguish directed autoantibodies. For this Reasons close regularly if the results are positive other tests (e.g. ELISA and / or Western blot).
The problems described in the assessment and evaluation always appear where the fluorescent patterns of Antibodies against antigens of the pathogen with those of Autoantibodies are identical or similar. Antinukleoläre Autoantibodies (ANA) are bothersome in EBV- (EBNA-1) and CMV diagnostics (CMV: cytomegalovirus) and in several Antibody detection against bacterial antigens found in the The nucleus of the infected cell. Bother against it antimitochondrial antibodies (AMA, fluorescence of the Cytoplasm) e.g. B. with parainfluenza virus, HSV (Herpes Simplex Virus), rubella virus, VZV (varicella-zoster virus), RSV (rous- Sarcoma Virus), Influenza A and B, Coxsackie A and B, Echo Virus, Bartonella etc.
The object of the present invention is therefore a general applicable method for the detection of specific To provide antibodies in biological samples, that for a wide application in the field of infection, Allergy and (auto) antibody diagnostics is suitable and the significant disadvantages of those used in the prior art Procedure does not have. In particular, it is the job of Invention, reliable and by laboratory personnel comparatively easy to carry out develop with the autoimmune hepatitis specifically detected can be and a clear distinction from others Hepatitis is possible. Furthermore, the Invention in particular also a specific syphilis detection allow the use and killing of animals avoids and without the risk of a possible infection with Treponema pallidum in the production of bacteria in the Rabbit testicles and the subsequent isolation of the germs can be carried out. In addition, the process should be suitable, one compared to the prior art enable more specific diagnosis of EBV infections.
According to the invention, the object is achieved by a method in which one recombinantly for the detection of specific antibodies manufactured antigens used by on solid supports immobilized, transformed host cells be, whereby after appropriate pre-adsorption of against the antigens of the host cells are directed non-specific Antibodies in the sample to be examined the specific Antibodies for example using immunofluorescence Use appropriately labeled secondary antibodies that directed against the specific antibodies.
The process according to the invention has the advantage that by the recombinant production of the antigens for the Antibody to be detected specific binding partner is presented. By providing this new method becomes the spectrum of diagnostic detection methods expanded, which in many, so far unclear cases have clearer disease diagnoses made.
Furthermore, it is possible according to the invention, not only antigens use that so far in other diagnostic procedures were used. By overproducing the antigen also a sufficient number of binding partners for the antibodies be provided, resulting in a bond of so many Antibodies cause detection that is often more sensitive becomes possible than is the case in the prior art. On this way, samples can now also be tested and Detect antibodies that are currently one Routine diagnostics withdrawn. By presenting the specific antigen / antigens or fragments of the specific antigen / antigens (without cross-reactions inducing components) will also affect the specificity of the Evidence such as B. in the field of diagnostics against Allergic directed antibodies, increased.
Man benutzt Genbanken (i. d. The method according to the present invention has this addition, the particular advantage of using several, each expressing a recombinant antigen Cells (or a cell that contains several antigens at the same time expressed) a screening is made possible. The structure this procedure corresponds to the "screening" for new ones Antigens. One uses gene banks (usually cDNA banks), clones the nucleic acids into expression plasmids and expresses the corresponding proteins (or parts of the Proteins). Individual clones are e.g. B. on a membrane immobilized, and you can use antibodies against one of these proteins are directed to the corresponding clone and finally identify the antigen. The implementation of the Method would be identical to indirect immunofluorescence, with the difference that it was not searched for new antigens but several, already known antigens side by side be immobilized and the sample for antibodies against these antigens "screen". This type of "mass screening" is so far only possible in connection with the ELISA, however this method has the disadvantage that per Incubation field only one pathogen can be tested at a time, while this limitation for the inventive method does not exist. Through the use of immunofluorescence now for the first time a comparatively easy to use, automated process provided.
Furthermore, the invention is characterized by its scope because numerous established expression systems, such as. B. for E. coli, exist and practically any antigens can be expressed recombinantly.

It is also possible to have several specific ones in parallel Detect antibodies in a sample by using multiple used various recombinant antigens expressed by the host cells provided in step (e) be, the respective, the different antigens expressing host cells at different locations (Reaction fields) immobilized on the solid support, so that only one antigen is presented in each reaction field.
In step (g), several different ones can be used at the same time detect secondary antibodies, which are only directed to one Immunoglobulin class bind, with the secondary antibodies attached different markers are bound and one of them specific antibodies by detection of the corresponding Markers demonstrates.
According to a particular embodiment using Several antigens are carried out step (g) several times in succession or with different secondary antibodies (IgA, IgG and / or IgM) in parallel, the incubation with the secondary antibodies for each reaction field separately he follows.
Alternatively, step (g) can be repeated several times or with different secondary antibodies (IgA, IgG and / or IgM) in parallel and different secondary Use antibodies to only one at a time Immunoglobulin class bind, with the secondary antibodies attached different markers are bound. You can also get a Antigen on a reaction field simultaneously with secondary ones Antibodies against different immunoglobulin classes incubate with the secondary antibodies on different markers are bound.
- 1. An incubation field, equipped with several different recombinant antigens, is used to investigate a class of pathogens (eg pathogens of sexually transmitted diseases) and one receives a positive or negative result. In the positive case, individual examinations should follow.
- 2. A miniaturization of the approach is also possible, in which the cells for the individual antigens are applied as "dots" next to one another in a reaction field and these are evaluated separately in the analysis and can thus be distinguished from one another.
As the person skilled in the art will recognize, this method can be carried out in different variants depending on the task.
The biological sample used is a human or animal sample, preferably serum, plasma, Blood, cerebrospinal fluid, urine, sputum, stool, cerebrospinal fluid, Ascites or amniotic fluid is taking the sample if necessary (i.e., if necessary) previously in a for the incubation in step (f) has been converted into a suitable form.
According to step (d) of the method, the expression of Antigen / the antigen one or more nucleic acid sequences introduced into the host cells. This can e.g. B. by Incubation of the host cells with a virus is done when the Host cells then the desired viral antigen express. Of course, those for the ones you want Antigen coding nucleic acid sequences also in each one or more suitable expression vectors be cloned. These methods are known to the person skilled in the art well known.
In the context of the present invention, the host cells, generally expressing the desired antigen (s) referred to as "transformed" host cells without the Invention to the narrow meaning of this term restrict. The name is only intended to differentiate of the antigen-expressing host cells from the corresponding ones Serve cells that do not express antigen.
In those used in the method according to the invention Host cells are prokaryotic or eukaryotic cells, especially those for which already well established expression systems for recombinant Production of proteins exist. E. coli cells, cells the genus Saccharomyces, insect cells, especially the Genus Spodoptera frugiperda (Sf), human or animal Cells (especially hamster cells, such as Chinese Hamster Ovary (CHO) cells) are particularly preferred.
The one used to bind to the specific antibodies Antigen is a viral, prokaryotic and / or eukaryotic protein, the term "eukaryotic Protein "parasitic (e.g. worms) or vegetable proteins (Allergens). It is preferably a viral, bacterial and / or human protein, whichever Host cells are not used for expression glycosylated or glycosylated or otherwise modified can be. The modifications that the organisms make to the There are many ways to attach proteins. Get as a result one next to phosphorylated proteins, mucopeptides, glyco- and Lipoproteins. There are also modifications of individual amino acids possible (hydroxylations with hydroxyproline, etc.).
Surprisingly, it has been shown that the invention Procedures especially for separate or joint verification from EBNA-1, from SLA / LP and / or from Treponema pallidum- Proteins and thus for the detection of EBV infections, from Syphilis and autoimmune hepatitis is suitable. The in Relation to the disadvantages known in the prior art are avoided in the method of the present invention.
According to a particular embodiment of the invention, this is Procedure a procedure for the diagnosis of autoimmune hepatitis in the Context of chronic liver disease, in which one as biological sample used human serum and the antigen SLA (soluble liver antigen).
According to a further, special embodiment of the invention is the procedure a procedure for diagnosing EBV Infections in which a biological sample is human serum used and the antigen is EBNA-1.
Vorzugsweise weisen SLA und EBNA-1 die in SEQ ID NO: 2 (SLA; vgl. DE-A-198 05 815) und EBNA-1 die SEQ ID NO: 4 (EBNA-1) dargestellten Aminosäuresequenzen auf. SLA and EBNA-1 preferably have the properties shown in SEQ ID NO: 2 (SLA; see. DE-A-198 05 815) and EBNA-1 SEQ ID NO: 4 (EBNA-1) amino acid sequences shown.
Als Nukleinsäuresequenzen, die für SLA und EBNA-1 kodieren, werden beispielsweise die erfindungsgemäß bevorzugten, in SEQ ID NO: 1 (SLA) und SEQ ID NO: 3 (EBNA-1) dargestellten Sequenzen verwendet. As nucleic acid sequences coding for SLA and EBNA-1, for example, the preferred according to the invention, in SEQ ID NO: 1 (SLA) and SEQ ID NO: 3 (EBNA-1) Sequences used.
According to a further, special embodiment of the invention The method is a method of diagnosing syphilis in which one as a biological sample human serum and the recombinant Antigen is TpN15, TpN17, TpN44.5 [TmpA] and / or TpN47.

For the skilled person it goes without saying that in addition to the explicitly named and disclosed nucleic acid sequences others due to the degeneration of the genetic code Sequences can be used for the same proteins encode. Modifications are also conceivable, such as the Exchange, deletion and / or insertion of individual or several amino acids that are not necessarily one Lead to change in antigenicity. With knowledge of respective epitopes can also make parts (fragments) larger Use proteins, reducing the specificity of the detection can be increased. Peptides from MOMP- Antigen (major outer membrane protein) from Chlamydia pneumoniae are listed. These are enough for you type-specific ELISA and to avoid cross-reactions with z. B. Chlamydia trachomatis.
To detect the binding of the specific antibodies to the Antigen or antigens are marked against specific antibody directed secondary antibodies used. Chromogens, for example, come as markers, Fluorescent dyes (such as fluorescein isothiocyanate), Luminescent dyes, radioactive markers, metals (such as Gold) and / or enzymes in question.

Important in the absorption of host-specific antibodies is the production of a preadorbent, its antigen Ingredients with which the recombinant protein expressing host cells must be identical. To do this will reach the host cell without the plasmidal or chromosomally inserted coding DNA for the recombinant Antigen cultivated identically to the respective one Expression strain.
Escherichia coli cells are streaked from a glycerol culture on an LB agar plate and incubated overnight at 37 ° C. (according to Sambrook et al., 1989). An occasionally grown clone is transferred to 50 ml LB medium and incubated again at 37 ° C. with vigorous shaking overnight. 20 ml of this preculture are inoculated into 1000 ml of fresh LB medium and cultivated to an optical density of approximately 2 (measured at a wavelength of 600 nm) (approximately 4 hours). The absorbance of an E. coli suspension at this wavelength is proportional to the cell density. The cell suspension is centrifuged for 15 minutes at 4000 rpm (rotor: Beckman JA-17) and the cell pellet obtained in a total of 30 ml PBS / Tween buffer (for 1 liter: 8.5 g NaCl, 1.42 g Na 2 HPO 4 , 0.29 g NaH 2 PO 4 , 0.02% Tween-20) resuspended. This cell suspension is treated with ultrasound for 5 minutes (Branson, Sonifier Cell Disruptor B15), the cells lysing and the antigens available for the absorption of the host-specific antibodies. This Escherichia coli lysate, known as pre-adsorbent, is aliquoted, frozen at -20 ° C and can thus be stored for a period of one year.
10 µl of this preadorbent are mixed with 10 µl antibody-containing sample (ratio 1: 1) mixed, 80 µl PBS / Tween buffer added and for 30 minutes Incubated at room temperature. After the pre-adsorption of the E. coli-specific antibodies will be the solution accordingly the respective test requirement diluted and for the analysis of the specific antibodies used.
A nucleic acid coding for a recombinant antigen E. coli strain contained is from a glycerol culture (Standard according to Sambrook et al., 1989) on one Spread out LB-agar plate containing ampicillin (100 µg / ml ampicillin) and incubated overnight at 37 ° C in the incubator. Of the on colonies grown the next day become one in 5 ml LB medium containing ampicillin (100 µg / ml ampicillin) in one Test tube inoculated and this pre-culture is made overnight incubated with shaking in a 37 ° C warm air shaker. At the the following day, 1 ml of the preculture in 100 ml Medium containing ampicillin transferred (100 µg / ml ampicillin, in Erlenmeyer flask) and again at 37 ° C with shaking incubated. With an optical density between 0.5 and 1.2 (measured at a wavelength of 600 nm) becomes induction the expression of the recombinant protein IPTG (isopropyl-β- thiogalactoside) added in a final concentration of 1 mM and the culture for a further 4 hours with shaking at 37 ° C incubated. The antigen-containing cell suspension now has one optical density of about 2 and can now be used in diagnostics be used.
1 ml of a recombinant antigen containing E. coli cell suspension (C) is centrifuged for 5 minutes with a table centrifuge in the Eppendorf reaction vessel at 13,000 rpm. The bacterial pellet obtained is resuspended in 1 ml PBS buffer (for 1 liter: 8.5 g NaCl, 1.42 g Na 2 HPO 4 , 0.29 g NaH 2 PO 4 ). 100 µl of this suspension are spread on a glass slide (26 × 76 mm) and air dried. The slide with the dried cells is immersed in methanol for 10 minutes and then in acetone for 10 seconds. Finally, the slide is allowed to air dry again and the cells are either used immediately as substrates in indirect immunofluorescence for the detection of antibodies or are stored in liquid nitrogen. The substrates can be stored there for several years.

Treponema pallidum was infected from Rabbit testicles and the genomic DNA by a modified proteinase K / SDS method isolated (Sambrook et al., 1989). The purification was carried out using an agarose gel analyzed. The gene amplification was done with the help specific primer at the 5 'end of each Coding sequences for restriction endonucleases were present, so that the 5 'overhangs that arise after digestion the incorporation of the amplificates into the NcoI / XhoI Cloning site of the plasmid enabled. The 3'- Terminal primers were designed to be directly connected a stop codon followed the coding sequence, which the Translation was aborted and the in this plasmid did not encode part of the histidine tag recombinant protein.

The size of the PCR products was over native Checked agarose gel electrophoresis.
Restriction and ligation of vectors and PCR products were carried out according to standard protocols (Sambrook et al., 1989) taking into account the manufacturer's information on the respective enzymes (restriction endonucleases, MBI fermentas; T4 DNA ligase and alkaline phosphatase, Roche). To Transformation of the ligation products into E. coli cells was done the correct incorporation of the PCR products into the plasmid Restriction analysis checked and tested in expression. The overexpressed proteins (procedure see example 1, C) were via SDS polyacrylamide gel electrophoresis and Coomassie staining and detection in a Western blot characterized sera tested.
Overexpression of Treponema pallidum proteins expressing cells takes place according to that in Example 1C. specified general protocol.
To detect the specific for Treponema pallidum A human sample was obtained from antibodies (clinical characterized human serum; Treponemen positive) and with that in Example 1A above. mentioned lysate among the in Example 1B. described conditions mixed and incubated.
After the incubation with the preadorbent (Example 1B.), The antisera are present in a dilution of 1:10. These are either used directly in the immunofluorescence detection or diluted 1:10 again with PBS / Tween buffer (final dilution 1: 100). 25 µl of the diluted samples are pipetted onto each slide and incubated for 30 minutes at room temperature. The entire slide is then washed with PBS / Tween buffer. This removes unbound antibodies and other components of the sample and the preadorbent. The detection of the bound specific antibodies is carried out by adding 25 µl of FITC-coupled anti-human IgG antibodies in PBS / Tween buffer for 30 minutes (anti-IgA or anti-IgM antibodies can then be used here). The slides are washed again with PBS / Tween buffer (to remove non-specifically bound FITC-IgG conjugates). Before the final check and assessment under the fluorescence microscope, 3 drops of 90% glycerol are pipetted onto each slide and the slide is covered with a cover slip. The result of the incubations is shown by way of example on TpN17 ( FIG. 1).
Fig. 1 (AD) shows the fluorescence micrographs of T. pallidum TpN17 expressing E. coli cells after incubation with one positive (C and D) and one negative serum (A and B). Without pre-adsorption, antibodies from both sera used bind to the corresponding antigens (A and C). Accordingly, the T. pallidum negative serum contains antibodies against components from E. coli (A) which, in the case of the positive serum, may also be present in addition to the anti-TpN17-specific antibodies (C). The fluorescence image after the pre-adsorption, however, shows a pattern that can be clearly evaluated (B and D). In the case of the negative serum, the fluorescence almost completely disappears (B), whereas with the T. pallidum positive serum used the fluorescence is weaker, but clearly provides a positive signal (D). The same results were obtained for the other 3 Treponemen antigens TpN15, TpN44.5 and TpN47 tested as for TpN17.
The advantages of this method have already been mentioned. To belong to the then no longer necessary cultivation of Treponemes in rabbit testicles, no risk of infection for that laboratory staff isolating the treponemes, no pre-adsorption more with another strain (Treponema phagedenis). Furthermore, the cultivation of E. coli cells is simple and delivers large quantities in a short time Antigens. Because the cells each have only one apathogenic antigen expressing from T. pallidum there is no risk of infection more.
The SLA antigen and its use in diagnostics autoimmune liver disease is in the patent DE 198 05 815 C1 described. The cloning and expression of the SLA antigen is in the associated publication (Wies et al., 2000) detailed. For use in immunofluorescence the cells producing the antigen are as described cultivated and fixed.
The expression vector in this case is pQE-31 (Qiagen), the Overexpression strain E. coli M15 (pREP).
For the detection of antibodies against the soluble liver Pancreatic antigen, a human serum was used and with that in Example 1A above. mentioned lysate among the in Example 1B. described conditions mixed and incubated.
After incubation with the pre-adsorbent (see Example 1B.) anti-SLA / LP antibodies were detected accordingly that in Example 2B. for Treponema pallidum Protocol.
The decisive advantage of this approach according to the invention lies in the merging of the different ones Diagnostics of chronic liver disease used Detection method by indirect immunofluorescence too one process. Further investigations using ELISA or Western blot would be used to identify the Autoimmune hepatitis no longer necessary. Whether the antigenicity through our new approach (the detection of the recombinant SLA in immunofluorescence) is improved or not, we cannot say at the present time.
