相关技术描述
临床研究和实践已显示,在组织部位附近提供减压能增加及加快组织部位处新组织的生长。此种现象的应用有很多,但减压在处理伤口和组织部位方面的应用已经特别成功。此种治疗(在医疗机构中经常被称为“负压伤口疗法”、“减压疗法”或“真空疗法”)提供多种益处,包括更快的愈合以及增加肉芽组织的生长。

在说明性实施方式中,任何水凝胶或结合剂可以被施加到孔302以及敷料器110,以用于密封组织部位或与组织部位接触。如图4和图6所示,第二衬垫部分104通常放置成基本覆盖孔302,位于消毒盖布108和敷料器110之间。在图5中,孔302可如本文所描述地基本由织物层106覆盖。孔302的尺寸可以变化以适应较大的伤口,但是在一个实施方式中,大约10mm到大约20mm的尺寸对于小伤口是有利的。
参照图4,展示了透气性界面系统100的说明性实施方式。孔302展示为设置穿过敷料器110。另外,透气性界面系统100可以进一步包括界面402,该界面402有利于第一衬垫部分102和/或织物层106以及减压导管112之间流体相通。参照图5,展示了透气性界面系统500的另一个说明性实施方式。该透气性界面系统500可以包括位于消毒盖布108和敷料器110之间的第一衬垫部分102、第二衬垫部分104以及织物层106的不同布置。在此说明性实施方式中,织物层106位于第一衬垫部分102的底部表面126和第二衬垫部分104的底部表面128以及敷料器110的顶部表面132之间。该第一衬垫部分102和第二衬垫部分104位于织物层106的顶部上或被放置在织物层106的顶部上。在此说明性实施方式中,织物层106基本覆盖孔302。参照图6,展示了透气性界面系统600的另一个说明性实施方式。在此说明性实施方式中,织物层106位于第二衬垫部分104的顶部表面124和第一衬垫部分102的顶部表面130以及消毒盖布108的底部表面136之间。
在透气性界面系统100、200、500和600中的任何一个中,减压导管112可放置成与第一衬垫部分102和/或织物层106直接接触。该减压导管112可以通过在透气性界面系统100的端部114附近将减压导管112直接插入第一衬垫部分102或织物层106的任何一个而放置成与第一衬垫部分102或织物层106直接接触。在另一个说明性实施方式中,透气性界面系统100、200、500和600可以进一步包括如图4所示的界面402以有利于第一衬垫部分102和/或织物层106以及减压导管112之间流体相通和流动。但在另一个说明性实施方式中,减压导管112可以不与第一衬垫部分102和/或织物层106直接接触,但可以另外与第一衬垫部分102和/或织物层106流体相通。
在一个说明性实施方式中,第二衬垫部分104的侧面120在第二衬垫部分104的顶部表面124和底部表面128之间延伸。第二衬垫部分104的底部表面128可具有可以覆盖敷料器110的端部116的顶部表面132的基本全部或敷料器110的端部116的顶部表面132的一部分的表面区域。另外,第一衬垫部分102的侧面122在第一衬垫部分102的顶部表面130和底部表面126之间延伸。第一衬垫部分102的底部表面126可具有可以覆盖敷料器110的顶部表面132的端部114的基本全部或敷料器110的顶部表面132的端部114的一部分的表面区域。
敷料器110可以具有足以对本文所描述的组织部位提供有效覆盖以及功能所需的任何尺寸。一方面,敷料器110包括可以优选地与组织部位接触的底部表面134。敷料器110的端部116可以具有与敷料器110的端部114的形状不同的表面区域。例如,如图1中所示的端部116的表面区域展示了基本圆形形状的表面区域。然而,敷料器110的端部116的形状可为任何所需的形状,对称、不对称或另外的形状,以提供本文所描述的组织部位的覆盖以及功能。在一个说明性实施方式中,敷料器110的端部114可以具有接近于矩形形状的表面区域;但是,敷料器110的端部114也可以为任何所需的形状,对称、不对称或另外的形状,以提供本文所描述的覆盖以及功能。
优选地,消毒盖布108的底部表面136覆盖第一衬垫部分102、织物层106以及第二衬垫部分104,并将第一衬垫部分102、织物层106以及第二衬垫部分104固定到敷料器110的顶部表面132。一方面,敷料器110和消毒盖布108基本上围绕它们各自形状的周边或边缘而密封在一起。优选地,敷料器110和消毒盖布108将组织部位与其周围环境隔绝,并在如本文所描述的施加减压时在该组织部位保持减压。该敷料器110可以使用任何合适的结合剂比如丙烯酸粘合剂或水凝胶固定到消毒盖布108。另外,敷料器110可以通过其它公知的方法比如例如结合、粘合剂、焊接、紧固和烧结接合到消毒盖布108。一般地,水凝胶或其它组织友好的结合剂可以被施加到组织侧或敷料器110的底部表面134,然后,其被放进组织部位或与组织部位的周边接触以将敷料固定到组织部位。
在说明性实施方式中,第一衬垫部分102和第二衬垫部分104可以为本领域已知的适合于减压组织治疗的材料,该材料的尺寸和形状可以变化以适应本文所描述的各种尺寸和形状的组织部位。优选地,第一衬垫部分102和第二衬垫部分104包括多个流通道(fluid channel)或流路径,以促进来自组织部位或通往组织部位的减压或流体的分配。在一个说明性实施方式中,第一衬垫部分102和第二衬垫部分104为包括互相连接的起流通道作用的小室或孔的多孔泡沫。除上文外,第一衬垫部分102和第二衬垫部分104也可以为比如由一系列聚合物形成的开放小室、网状泡沫的材料,上述聚合物包括但不限于:聚氨基甲酸酯、聚烯烃、乙酸乙烯酯、聚乙烯醇以及它们的共聚物。另外,第一和第二衬垫部分102、104可以为纺织或非纺材料,包括三维织物结构。衬垫也可以由烧结聚合物,包括比如烧结聚烯烃、乙烯乙酸乙烯酯以及含氟聚合物的材料制成。第一衬垫部分102和第二衬垫部分104也可以为任何类型的开放小室的网状泡沫(open-cell reticulated foam),比如由Kinetic Concepts,Inc.of SanAntonio,Texas.制造的和WhitefoamTM。如果使用了开放小室泡沫,则孔隙度可以变化,但优选为大约400微米至600微米。可选择地,纱布或适合于特定生物用途的任何其它材料可用于构造第一衬垫部分102和第二衬垫部分104。在某个说明性实施方式中,第一衬垫部分102和第二衬垫部分104可构造为单独的整体衬垫。在另一个说明性实施方式中,第一衬垫部分102和第二衬垫部分104可以为多部件或多层衬垫部分。优选地,第一衬垫部分102和第二衬垫部分104的厚度为从大约1mm到大约50mm,并且在一个执行方式中为从大约5mm到大约20mm,但是可以使用任何厚度。
在说明性实施方式中,织物层106可以为本领域已知的纺织或非纺织物材料,织物材料的尺寸和形状可以变化以适应本文所描述的各种尺寸和形状的组织部位。其可由在暴露于流体时保持其结构完整性的任何纤维材料比如聚酰胺、聚烯烃、尼龙、聚酯、涂有聚氨基甲酸酯的聚酰胺、任何聚合物网、非纺(空气层)融化吹制聚合物(non-woven(air layed)meltblown polymer)或柔韧的烧结聚合物构造。织物层106也可以为覆盖有粘合剂或水凝胶以有利于结合到组织部位的织物,其中该织物层106延伸到敷料器110以外。该材料可以被纺织在一起以形成尺寸适合的层,或其可以为任何类型的尺寸合适的开放小室网结构。如图1中所示,织物层106也可以被折叠并包括缝线140以提供附加的通道和结构支撑。折叠的织物层可以围绕边缘沿中间纵向向下缝合,如图2中所描绘的,或者为其任何组合。作为缝线的选择,折叠的织物层可以使用丙烯酸粘合剂或任何其它合适的结合剂固定。织物层106也可以包括通过任何已知方法接合在一起的若干个重叠层。织物层106的厚度可以为从大约1mm到大约50mm,或可选择地为从大约5mm到大约20mm,但是可以使用任何厚度。
消毒盖布108可以为具有足够高的湿气传输率(“MTVR”)的柔韧材料以排除组织浸渍,一般大于600mg/m2/day。一方面,塑料和热塑性塑料为用于消毒盖布108的合适材料的例子。并且同消毒盖布108一样,敷料器110一般由具有足够高的MTVR的任何柔韧材料比如塑料和热塑性塑料构造以排除组织部位的浸渍。
减压导管112可以代表可以通过其传送气体、液体、凝胶或其它流体的任何导管管道、线路或路径,并且可以具有多于一个的内部腔管。虽然该减压导管112可以为不柔韧的,但优选其应足够柔韧以便于使用并使患者舒适。减压导管被构造成连接到减压源以提供减压输送。
在说明性实施方式中,透气性界面系统100、200、500和600可以为用于严重程度低的小组织部位的重量轻、低型界面系统,但是这些原理可容易地由本领域技术人员延伸到更大、更外延的组织部位,以及多种其它类型的组织治疗。
再次参照图1-6,孔302设置在组织部位上方,并且减压源704通过减压导管112将减压传输到透气性界面系统100、200、500和600。孔302可以为如所展示的单个孔,或任何数量或多个洞、开口、孔、狭缝或所需的类似物以提供组织部位以及第一衬垫部分102、第二衬垫部分104和织物层106之间的减压和流体输送的分配。如上文所述,第一衬垫部分102和第二衬垫部分104可以包括允许减压被分配到透气性界面系统100、200、500和600的各处的路径或通道,并且该路径或通道允许流体通过孔302从组织部位移除。织物层106的纺织结构或网结构提供附加的流体路径,该附加的流体路径不易受可施加到透气性界面系统100、200、500和600的压缩负荷下折叠的影响,比如由于患者在床上翻滚或以其它方式移动引起敷料的压缩时遇到的那些情况。附加的流体路径也减少将减压分配到组织部位所需要的时间。如下面所描述的,测试已显示,由减压源产生的压力变化用构造成和透气性界面系统100、200、500和600一样的敷料更快地相通到组织部位。
参照图7,显示了结合透气性界面系统的新颖特征的减压组织治疗系统700的说明性实施方式。减压组织治疗系统700包括类似于本文描述的其它透气性界面系统的透气性界面系统701,该透气性界面系统701被施加到组织部位702以用于治疗。透气性界面系统701通过减压导管112流体地连接到减压源704。在某些实施方式中,减压组织治疗系统700还可包括用于收集从702提取的流体和其它非气态流出物的罐706。
参照图8,展示了对比常规敷料和实质上如上文所描述的透气性界面系统100的压力传输测试结果的图表。在这些测试中,减压被施加到每个透气性界面系统100并且水被泵送通过每个透气性界面系统100,同时透气性界面系统100受到一系列压缩力。压力测量在压缩力的两侧进行,以确定每个样品的性能。如图8所展示的,结果展示,如上文所描述的透气性界面系统100使得跨越压缩负荷的压力相通到比常规敷料更大的程度。
水的流量被设置成大约20mls/hr,并且从大约0N到大约500-930N的压缩力被施加到常规敷料和透气性界面系统100。Y轴802代表在泵或敷料/透气性界面系统100处测量的减压或真空的量。X轴804代表从测试开始所消耗的持续时间。线806代表用于常规敷料的泵处的减压的量,而线808代表敷料的相对侧处的减压的量。如可以从图8看到的,大约900N的压缩力被施加到常规敷料,并且敷料处可测量的减压的量为大约0mmHg,如线808所展示的。在事件814开始时,压缩力被释放,因此敷料处可测量的减压的量增加到大约120mm Hg。在事件814结束时,压缩力施加525N的量,而可测量的减压的量回落到接近0mmHg。在此相同事件的过程中,如线806所示,敷料的泵侧的可测量的减压保持在大约125mm Hg。这显示,对于处于压缩力下的常规敷料,通过敷料的减压的量大约为0mmHg。类似地,对于事件816、818和820,压缩力被释放并重新以大约250N而施加。从图8可以看到,其随后是基本相同的结果。即,一施加压缩力,通过常规敷料的可测量的减压的量就落到0mmHg或接近0mmHg。
相反地,线810代表用于透气性界面系统100的泵处的减压的量,而线812代表敷料的相对侧的减压的量。如上文所描述的,大约900N的压缩力被施加到常规敷料,并且敷料处可测量的减压的量大约为50mm Hg,如线812所示。在事件814开始时,压缩力被释放,因此敷料处可测量的减压的量增加到大约120mm Hg。在事件814结束时,压缩力被施加525N的量,并且可测量的减压的量被减少到接近50mm Hg。在此相同事件的过程中,如线810所示,敷料的泵侧的可测量的减压保持在大约125mmHg。这显示,对于处于压缩力下的透气性界面系统100,减压的量仍然可观。类似地,对于事件816、818和820,压缩力被释放并被重新施加大约250N。从图8可以看到,随后的结果甚至更好。即,一施加大约250N的压缩力,通过常规敷料的可测量的减压的量增加到大约70mm Hg到100mm Hg之间。
参照图9,展示的另一图表比较了当在干情况下受到间歇施加的减压时常规敷料和实质上如上文图8所描述的透气性界面系统100的响应时间。图9中展示的响应时间展示了在置于此种情况时常规敷料的响应比上文描述的透气性界面系统100的响应慢得多。
在压缩力的两侧进行压力测量以确定与如上文所描述的透气性界面系统100相比较的常规敷料的响应时间。如图9所示,结果展示,对于间歇施加和释放的减压,如上文所描述的透气性界面系统100实现较快的响应时间。Y轴902代表在泵或敷料/透气性界面系统100处测量的减压或真空的量。X轴904代表从测试开始所消耗的持续时间。线906代表用于常规敷料的泵处的减压的量,而线908代表敷料的相对侧处的减压的量。线910代表用于透气性界面系统100的泵处测量的减压的量,而线912代表在透气性界面系统100的相对侧测量的减压的量。
如可以从图9中看到的,减压在大约0mmHg和125mmHg之间断断续续地循环。线906和910相当紧密地彼此配对,这展示了在减压循环时在对敷料/透气性界面系统100的泵侧处进行测量的过程中可测量的减压有些许差别。在敷料的另一侧,线908展示了在常规敷料处达到已施加的减压的延迟时间。这可以视为线908对其形状具有代表在它达到完全减压之前逐渐累积的弧度。相反地,线912展示了当减压断断续续地循环时的急剧过渡,因此代表了透气性界面系统100提供了比常规敷料改进了的流体传输及对减压的响应。
从前述应明白,已提供了具有显著优势的本发明。虽然本发明已展示了其形式中的仅仅几个,但本发明是不受限制的,而是可以做出各种改变和修改而不背离于其精神。
Claims (8)
1.一种减压组织治疗系统,包括:
敷料器,其具有孔;
第一衬垫部分;
第二衬垫部分,其基本覆盖所述孔,并且基本与所述第一衬垫部分相邻地安置;
织物层,其至少部分地位于所述第二衬垫部分和所述敷料器之间,所述织物层包括由纤维材料制成的纺织或非纺织物;
消毒盖布,其基本覆盖所述第一衬垫部分、所述第二衬垫部分、所述织物层和所述敷料器;以及
减压导管,其与所述第一衬垫部分和所述织物层中的至少一个流体相通,以对所述孔提供减压。
2.如权利要求1所述的减压组织治疗系统,还包括流体地连接在所述减压导管与减压源之间的罐。
3.如权利要求1所述的减压组织治疗系统,其中,所述织物层至少部分地位于所述第一衬垫部分和所述消毒盖布之间。
4.如权利要求1所述的减压组织治疗系统,还包括位于所述第一衬垫部分和所述织物层中的所述至少一个与所述减压导管之间以利于流体相通的界面。
5.如权利要求1所述的减压组织治疗系统,其中,所述第一衬垫部分和所述第二衬垫部分选自由开放小室的网状泡沫、烧结聚合物、乙烯乙酸乙烯酯、含氟聚合物、聚氨基甲酸酯、聚烯烃、乙酸乙烯酯、聚乙烯醇以及它们的共聚物组成的组。
6.如权利要求1所述的减压组织治疗系统,其中,所述织物层选自由聚酰胺、涂有聚氨基甲酸酯的聚酰胺、聚合物网、非纺融化吹制聚合物以及柔韧的烧结聚合物组成的组。
7.如权利要求1所述的减压组织治疗系统,其中,所述第一衬垫部分、所述第二衬垫部分以及所述织物层的厚度为从5mm到20mm。
8.如权利要求1所述的减压组织治疗系统,其中,所述织物层包括接合在一起的多个重叠层。
