Abstract

可生物吸收的缝合线可以由在经过给定时间后在组织中分解的材料制成,取决于材料的该给定时间可以是从10天至8周。因此,缝合线用于身体的很多内部组织中。在大部分情况下,3周足以使得伤口牢固闭合。这时将不再需要缝合线,且它消失将是有利的,因为没有外来材料留在身体内,且病人不需要除去缝合线。在很少的情况下,可生物吸收的缝合线可能引起发炎和由身体排斥,而不是吸收。可生物吸收的缝合线首先由哺乳动物的肠制造。例如,内脏缝合线可以由专门准备的牛或绵羊的肠,并可以是未处理(普通肠线)、用铬盐鞣制以便增加缝合线在体内的保持时间(铬肠线)、或者热处理以便产生更快速的吸收(快速肠线)。由于担心传播疾病(例如疯牛病),因此内脏从已经检验的原料中获取,以便确定用于缝合线材料的天然聚合物并不携带病毒性疾病。可生物吸收的缝合线可以由合成聚合物纤维来制造,它可以是单丝或编织的。

发明内容
这里提供了缝合线,用于在施加于组织上的处理过程中使用,还提供了用于形成该缝合线的方法。根据一个实施例,缝合线包括细长缝合线本体和多个保持器,这些保持器相互间隔开,并在缝合线本体的第一端和第二端之间从细长缝合线本体伸出。在特殊实施例中,保持器是双向保持器。

具体实施方式
在介绍本发明之前,首先提出在下文中使用的某些术语的定义可能有利于理解本发明。
“自保持系统”是指自保持缝合线和用于将该缝合线部署在组织中的装置。该部署装置包括但不局限于:缝合线针头和其它部署装置以及在缝合线自身上的、用于穿透组织的足够刚性和尖锐端部。

“缝合线部署端”是指缝合线的、要部署至组织中的端部;缝合线的一端或者两端可以是缝合线部署端。缝合线部署端可以安装在部署装置例如缝合线针头上,或者可以足够尖锐和刚性,以便通过它自身穿透组织。
“已装备的缝合线”是指在至少一个缝合线部署端上有缝合线针头的缝合线。
“针头连接”是指针头连接在需要用它来部署至组织中的缝合线上,并可以包括方法例如卷曲、弯头、使用粘接剂等。缝合线连接在针头上的点称为弯头。
“缝合线针头”是指用于将缝合线部署至组织中的针头,它可以有多种不同形状、形式和组分。有两种主要类型的针头:创伤针头和无创伤针头。创伤针头有槽道或钻孔端(也就是孔或眼),并与缝合线丝线分别提供,并在现场穿线。无创伤针头是无眼的,并在工厂通过模制而连接在缝合线上,因此,缝合线材料插入在针头钝端处的槽道中,然后,该针头变形至最终形状,以便将缝合线和针头保持在一起。这样,无创伤针头并不需要在现场的额外时间来穿线,且在针头连接部位的缝合线端比针头本体更小。在创伤针头中,丝线从两侧的针头孔出来,且缝合线在它经过组织时通常在一定程度上撕开组织。更现代的缝合线模制有无创伤针头。无创伤针头可以永久性地模制至缝合线上,或者可以设计成通过尖锐的直拖拉件而离开缝合线。这些“突然离开”通常用于中断缝合线,其中,各缝合线只经过一次,然后打结。对于不中断的有倒钩缝合线,这些无创伤针头很理想。
缝合线针头还可以根据它们的尖端几何形状来分类。例如,针头可以为(i)“锥形”,因此针头本体为圆形,并朝着尖端平滑地逐渐变细;(ii)“切割”,因此针头本体为三角形,并有在内侧的削尖切割刃;(iii)“反向切割”,因此切割刃在外侧;(iv)“套管针尖端”或“锥形切割”,因此针头本体为圆形和锥形,但是终止于较小的三角形切割尖端;(v)“钝”尖端,用于缝合易碎的组织;(vi)“侧部切割”或“刮刀尖端”,因此针的顶部和底部为平的,具有沿前部至一侧的切割刃(通常用于眼外科手术)。
缝合线针头还可以为各种形状,包括:(i)直线形;(ii)半弯曲或滑板形;(iii)1/4圆形;(iv)3/8圆弧形;(v)1/2圆弧形;(vi)5/8圆弧形;(v)以及复合曲线形。
缝合针头,例如在美国专利No.6322581和6214030(授予日本的Mani,Inc.)、5464422(授予W.L. Gore,Newark德国)、5941899、5425746、5306288和5156615(授予US Surgical Corp.,Norwalk,CT)、5312422(授予Linvatec Corp.,Largo,FL)以及7063716(授予TycoHealthcare,North Haven,CT)中介绍。其它缝合针头例如在美国专利No.6129741、5897572、567675和5693072中介绍。这里所述的缝合线可以通过各种针头类型(包括但不局限于:弯曲的、直线形的、较长的、较短的、微型等)、针头切割表面(包括但不局限于:切割、锥形等)以及针头连接技术(包括但不局限于:钻孔端部、卷曲等)来部署。而且,这里所述的缝合线自身可以包括足够刚性和尖锐的端部,以便满足与针头一起部署的需要而进行分配。
“针头直径”是指缝合线部署针头在该针头的最宽点处的直径。尽管术语“直径”通常与圆形周边相关联,但是应当知道,这里表示与任意形状的周边相关联的截面尺寸。
“伤口闭合”是指用于闭合伤口的外科手术处理过程。创伤(特别是皮肤或其它外部或内部表面进行切割、撕裂、穿刺或以其它方式被破坏时的创伤)称为伤口。伤口通常在任何组织的完整性受损时产生(例如皮肤破裂或烧伤、肌肉撕裂或者骨折)。伤口可以由动作(例如射击、跌倒或外科手术处理过程)、感染疾病或者通过下面的医疗状态而引起。外科手术伤口闭合通过邻接或紧密拉近这些伤口(在该伤口处,组织已经撕裂、切割或以其它方式分开)的边缘而有利于生物的愈合。外科手术伤口闭合直接并列或拉近组织层,这用于减小桥接在伤口的两个边缘之间的间隙而需要形成的新组织的容积。闭合也可以用于功能和美观目的。这些目的包括通过拉近皮下组织而消除死区,通过仔细的表皮对齐而减少疤痕形成,以及通过精确翻转皮肤边缘而避免凹陷疤痕。
“组织升高处理过程”是指用于使得组织从更低高度重新定位至更高高度(即使得组织沿与重力方向相反的方向运动)。脸的保持韧带将脸的软组织支承在正常解剖位置。不过,随着年龄的增长,重力作用在该组织和底侧韧带上产生向下拉力,且脂肪下降至在表面和深层脸部筋膜之间的平面中,从而使得脸部组织下垂。脸升高处理过程设计成升高这些下垂组织,是更普通类型的、称为组织升高处理过程的医疗处理过程的一个实例。通常,组织升高处理过程使得外观变化(该外观变化由于经过一段时间的重力效果而产生)和时间效果(时间效果使得组织下垂,例如遗传效果)反向。应当知道,组织也可以在并不升高的情况下重新定位,在某些处理过程中,组织横向(远离中线)、向中间(朝着中线)或向下(降低)重新定位,以便恢复对称(即重新定位成使得身体的左侧和右侧“对称”)。
“医疗装置”或“植入件”是指为了恢复生理功能、降低/缓和与疾病相关联的病症和/或修复/代替损坏或病变的器官和组织而布置在体内的任何物体。尽管通常包括在外部的生物相容合成材料(例如医疗等级的不锈钢、钛和其它金属、聚合物例如聚氨酯、硅、PLA、PLGA和其它材料),但是一些医疗装置和植入件包括来自于动物(例如“异种移植物”如整个动物的器官;动物组织例如心脏瓣、天然出现或化学变化的分子,例如胶原蛋白、透明质酸、蛋白质、碳水化合物等)、人供体(例如“异源移植”如整个器官、组织如骨移植件、皮肤移植件等),或者来自病人自身(例如“自体移植件”如隐静脉移植件、皮肤移植件、腱/韧带/肌肉移植件)的材料。可以在与本发明结合的处理过程中使用的医疗装置包括但不局限于:矫形植入件(人造关节、韧带和腱;螺钉、板和其它可植入硬件)、牙齿植入件、血管内植入件(动脉和静脉脉管旁路移植件、血液透析进入植入件;自体的和合成的)、皮肤移植件(自体的、合成的)、管、排水道、可植入组织填充剂、泵、旁路、密封剂、外科手术网(例如、疝修理网、组织支架)、瘘管处理、脊骨植入件(例如仿真的椎间盘、脊柱融合术装置等)等等。
自保持缝合线
自保持缝合线(包括有倒钩缝合线)与普通缝合线的区别在于它们有大量组织保持器(例如倒钩),该组织保持器在部署后锚固在组织中,并阻止缝合线沿与保持器的朝向方向相反的方向运动,因此不需要系绳结来将相邻组织固定在一起(“无绳结”闭合)。通过消除系绳结,可以消除相关联的并发症,包括但不局限于:(i)喷溅(spitting)(当缝合线(通常为绳结)在皮下闭合后推动穿过皮肤时的症状);(ii)感染(细菌通常能够附在由绳结产生的空间内并在该空间内生长);(iii)体积/质量(在包括绳结的部分处,大量缝合线材料留在伤口中;(iv)滑移(绳结可能滑动或者解开);以及(v)刺激(绳结作为在伤口中的大块“外来体”)。与系绳结相关联的缝合线环可能导致局部缺血(它们产生拉伸点,该拉伸点可能勒住组织,并限制血液流向该区域)和增加在外科手术伤口处裂开或破裂的危险。系绳结也是有很大强度的劳动,并可能包括在闭合外科手术伤口时耗费的时间的明显较大百分数。附加手术处理时间不仅对于病人不利(并发症的发生率随着在麻醉下的时间增加而升高),而且还增加了手术的总体成本(很多外科手术处理过程估计成本在每分钟手术时间为$15和$30之间。因此,无绳结缝合线不仅能够提高病人的临床结果,而且还节省了与延长外科手术和随后处理相关联的时间和成本。
用于伤口闭合的自保持系统也导致伤口边缘的更好接近,使得拉伸力沿伤口的长度均匀分布(减少了可能破裂或导致局部缺血的拉伸区域),减少了保留在伤口中的缝合线材料体积(通过消除绳结),并减少了喷溅(缝合线材料(通常为绳结)穿过皮肤表面挤出)。所有这些特征都认为与通过普通缝合线或钉进行的伤口闭合相比减少了疤痕形成、提高了美容性,并增加了伤口强度。
自保持缝合线即使在没有向缝合线施加拉伸时也将组织锚固和保持就位的能力也是优于普通缝合线的特征。当在拉伸下闭合伤口时,该优点自身以多种方式证明:(i)大量的保持器可以使得拉伸力沿缝合线的整个长度分散(提供了数百个“锚固”点,与间断打结的缝合线相反,该间断打结的缝合线使得拉伸力集中在离散点;这产生了良好的美容结果,并减小了缝合线“滑动”或拉过的可能性);(ii)可以以均匀方式闭合复杂几何形状的伤口(圆形、弧形、锯齿形边缘),比间断的缝合线可以获得的结果更精确和正确;(iii)它们不需要“第三只手”,在普通缝合和系绳结的过程中通常需要“第三只手”来保持横过伤口的拉伸力(以便防止在打结过程中暂时释放拉伸时“滑动”);(iv)它们在系绳结技术上很困难的处理过程中很有利,例如在深层伤口或腹腔镜处理过程中;以及(v)它们可以用于在最后闭合之前拉近和保持伤口。因此,自保持缝合线使得在解剖上较紧或较深的位置中更容易处理(例如骨盆、腹板和胸部),并在腹腔镜和最小侵入处理过程中更容易拉近组织;所有这些都是不必通过绳结来固定闭合。与普通缝合线相比,更大精度使得自保持缝合线能用于更复杂的闭合(例如有直径不匹配、更大缺陷或荷包口缝合的闭合)。
自保持缝合线自身还有利于各种专业应用:例如,它们适用于组织升高处理,其中,组织离开它的在先位置,并重新定位在新的解剖位置(这通常在美容处理过程中进行,其中“下垂”的组织升高并固定在更“年轻”的位置;或者当使得“离开位置”的组织返回它的正确解剖位置时)。这样的处理过程包括脸升高、眉毛升高、胸升高、臀部升高等。
自保持缝合线可以是单向,具有沿缝合线丝线长度沿一个方向定向的一个或多个保持器;或者为双向,通常有沿丝线的一部分沿一个方向定向的一个或多个保持器,随后在丝线的其余部分上有沿另一方向(通常相反方向)定向的一个或多个保持器(如美国专利No.5931855和6241747中所述的有倒钩保持器)。
尽管保持器可以有任意数目的连续或间断结构,但是最通常的形式包括在一端的针头,随后是“远离”针头凸出的倒钩,直到到达缝合线的转变点(通常为中点);在该转变点处,倒钩的结构沿缝合线丝线的剩余长度在到达相对端处的第二针头之前自身颠倒180°(即这时倒钩朝向相反方向)(因此在缝合线的该部分上的倒钩也背离最近的针头)。换言之,在双向自保持缝合线点的两“半”上的倒钩都指向中间,转变段(没有保持器)置于它们之间,且针头安装在各端。
尽管自保持缝合线有多个优点,但是仍然需要和希望改进该缝合线的设计,这样,可以消除各种普通限制。
B.双向保持器
图1表示了根据本发明实施例的自保持缝合线100的一部分的透视图,该自保持缝合线100包括细长的线状缝合线本体102和从该缝合线本体102凸出的多个保持器104。缝合线本体102可以包括两个端部,或者超过两个端部(即三个端部、四个端部或更多)。图1中所示的自保持缝合线100还没有穿过病人的组织部署。
如图1中所示,保持器104(例如在外科手术处理过程中)在穿过组织部署之前为大致平的。如图1中所示,在穿过组织部署之前,该保持器可以与缝合线本体102大致垂直。保持器104可以称为双向保持器104,因为它们可以沿两个方向部署,但是一旦沿一个方向部署后将阻止沿相反方向运动,如后面所述。有利的是,这能够在外科手术处理过程中来确定沿一个方向部署的双向保持器104的数目以及沿相反方向部署的双向保持器104的数目。更具体地说,在穿过组织部署之前,各双向保持器104可沿第一方向和大致与该第一方向相反的第二方向来穿过组织部署。不过,一旦双向保持器104沿第一方向和第二方向中的一个方向穿过组织部署,双向保持器104将在细长缝合线本体102沿第一方向和第二方向中的一个方向拉动时基本屈服于细长缝合线本体102在组织内的运动,并将阻止细长缝合线本体102沿第一方向和第二方向中的另一方向运动。
如图2A和2B的正视图所示,保持器104可以为圆形形状(图2A),或者可以有其它形状,例如但不局限于:八边形(图2B)、六边形、正方形等。各保持器104包括槽106,该槽106从中心朝着保持器的外部周边或边缘径向延伸。槽106可以从中心至外部边缘连续,或者可以不连续或间断(类似于穿孔怎样包括间断的孔)。槽106可以切入保持器104中(并因此为刻痕),或者作为模制和/或镦头处理的一部分而形成,例如在形成保持器104的同时。可以使用比图2A-2C中所示更多或更少的槽106。还有,如下面所述,图2A-2C中所示的各槽可以由多对并排的槽或者三个或更多并排的槽来代替。
图3A-3F表示了槽106的各种实施例,该图3A-3F表示了保持器104的可选截面部分。各保持器104有两个大致平面形的侧部,这两个侧部可以称为第一侧和第二侧、左侧和右侧等。如图3A和3D中所示,只有保持器104的一侧可以包括槽106,或者如图3C和3F中所示,只有保持器104的另一侧可以包括槽106。也可选择,如图3B和3E中所示,保持器104的两侧都可以包括槽106。当保持器104的两侧都包括槽时,在一侧上的槽106可以是在另一侧的槽的镜像。也可选择,槽106可以在保持器104的两侧,它们并不相互镜像,如图2C中所示,其中,实线106表示在一侧的槽,虚线106表示在另一侧的槽。应当知道,图3A-3C中的槽106的形状与图3D-3F中的槽106的形状不同。一对槽可以并排,如图3G中所示,或者三个或更多槽可以并排,以便形成槽的正弦波,如图3H中所示。在特定实施例中,槽可以有均匀的厚度和/或深度。在另一实施例中,槽可以有变化的厚度和/或深度,以便允许在各保持器104的中心附近有更大变形。对于更特别的实例,各槽可以在保持器的中心或开口116附近有最大厚度和/或深度,且在保持器的外部边缘处最窄和/或最浅。尽管在图3A-3H中槽106表示为大致三角形形状,但是也可以为其它形状,例如但不局限于:正方形、U形等。
如图1和4A中所示,保持器104的侧部首先大致垂直于缝合线本体102。这也意味着槽106首先大致垂直于缝合线本体102。不过,一旦缝合线用于外科手术处理过程中且穿过病人的组织时,由病人的组织提供的阻力将使得保持器104收缩,如图4B和4C中所示。图4B表示了保持器104在它最初开始收缩时。图4C表示了保持器104在它已经收缩之后,即处于它的收缩位置。槽106用作折叠线或收缩线,这样,保持器104以控制方式收缩。收缩可以以稍微手风琴的形式产生,其中,折叠沿槽106前后交替。
根据保持器穿过组织部署的方向,各保持器104可以沿两个方向中的任意一个收缩。保持器104可以沿槽的方向收缩,但是也可以沿与槽相反的方向收缩。换句话说,当槽在保持器104的右侧,且具有保持器的缝合线本体102沿向左方向拉动穿过组织(即部署)时,保持器104将向右收缩,如图4B和4C中所示。不过,当相同的缝合线本体102最初沿向右方向拉动穿过组织(即部署)时,保持器104将向左收缩,尽管槽只是在保持器104的右侧。换句话说,各保持器104沿与保持器穿过组织部署的方向相反的方向收缩。
一旦处于收缩位置(例如如图4C中所示),保持器104在缝合线100沿使得保持器收缩的缝合线部署方向拉动时将基本屈服于细长缝合线本体102在组织内的运动,并当缝合线100沿与缝合线部署方向相反的方向拉动时阻止运动。例如,由图4C可知,收缩的保持器104可以沿箭头方向很容易地拉过组织,但是将阻止沿与箭头相反方向的运动。更概括地说,一旦双向保持器104由于保持器沿第一方向穿过组织部署而收缩,该保持器104就将在细长缝合线本体102沿第一方向拉动时基本屈服于细长缝合线本体102在组织内的运动,并将阻止细长缝合线本体102沿与第一方向大致相反的第二方向的运动。更通常是,各双向保持器104可以沿两个方向中的任意一个方向部署,且一旦部署,将屈服于沿使得保持器穿过组织部署的方向的运动,并阻止沿与部署方向相反的方向的运动。
包括双向保持器104的自保持缝合线100可以单向或双向使用。当要单向使用时,自保持缝合线可以包括:为尖端或有针头的端部,以便当由该端部拉动时允许穿过和通过组织;以及相对的端部,该相对端部在某些实施例中包括锚固件,用于在初始插入尖端与组织接合,以便限制缝合线的运动。当要双向使用时,缝合线的多于一个的端部可以包括尖端或针头,用于组织穿透。换句话说,双向缝合线可以在缝合线丝线的各端部处装备有针头。
通常,双向自保持缝合线包括沿缝合线的一部分朝着一个部署方向延伸的一组保持器以及分组的沿缝合线的另一部分朝着相反部署方向延伸的相反保持器。还有,通常,双向自保持缝合线包括位于两组保持器之间的转变段。这样的普通双向自保持缝合线的问题是外科医生需要在部署缝合线时认识到该转变段的位置。而且,当转变段处于缝合线的中间时,在某些情况下,在转变段一侧的缝合线长度可以比外科医生所希望的更短。
另外,普通双向自保持缝合线的转变点的标记并不总是令人满意。当前双向自保持缝合线的缺点是实现转变点的有效标记有附加的制造和技术障碍。而且,转变点并不总是处于某些医生所希望的位置和/或适合或最佳地用于某些处理过程的位置。
包括双向保持器104的双向自保持缝合线100克服了普通双向自保持缝合线的前述缺陷。这是因为包括双向保持器104的双向自保持缝合线100能够沿任意方向部署,而且一旦部署将阻止沿与部署方向相反的方向运动。例如,当缝合线包括N个间隔开的保持器104时,从全部到零的任意数目(即M,0≤M≤N)的保持器104可以通过将全部或一部分缝合线沿第一方向拉动穿过病人的组织而沿一方向收缩,而剩余的保持器104(即N-M)可以通过将剩余部分的缝合线沿第二方向拉动穿过病人的组织而沿相反方向收缩。然后,即一旦收缩,各保持器104将阻止沿与它的部署方向相反的方向运动。例如,M个保持器104可以阻止沿第一方向的运动,而N-M个保持器104可以阻止沿与第一方向相反的第二方向运动,其中,变量M可以很快选择。换句话说,沿一方向部署的双向保持器104的数目和沿相反方向部署的双向保持器104的数目可以在外科手术处理过程中决定。这可以由图5中所示。而且,与普通的双向自保持缝合线(其中,通常在可沿相反方向部署的两组保持器之间包括转变段)不同,包括双向保持器104的双向自保持缝合线100不需要包括转变段。另外,不管所希望和/或最佳的转变点如何,包括双向保持器104的相同双向自保持缝合线100都可以高效和有效地使用。另外,当缝合线包括在缝合线本体两端处的针头时,医生可以在处理过程中在任意时间改变转变点。例如,医生可以通过一个针头进行缝线,然后,这时不管已经使用了多少缝合线,当他这样做时,医生都可以开始用另一针头来对剩余的缝合线进行缝线。这优于具有固定的对称或不对称转变段的缝合线,因为转变段可以是医生对于任意处理过程在处理过程中的任意时间选择的任意位置。
图6A和6B表示了保持器104可以包括沿保持器的外边缘的凸起108。凸起108用于增加保持器104与组织的接合。图6C表示了图6A的一个保持器104在它穿过组织部署和收缩之后的情况。由图6C可知,当缝合线本体102沿与箭头相反的方向拉动时,凸起将刺入病人的组织中,从而进一步阻止沿与箭头相反方向运动。如图6D和6E中所示,凸起108可以是四边形面610的部分。四边形面610可以是理想的长菱形,如图6E所示。通过改变各四边形(和可能的长菱形)面610的尺寸,可以获得不同的最终几何形状。
图7表示了保持器104可以包括一个或多个通气孔110,该通气孔110在保持器穿过病人的组织部署时防止空气由保持器104捕获。这很有利,因为认为当并不在保持器104后面捕获空气时组织将更快愈合。而且,通气孔用于与病人组织的进一步锚固。
保持器104可以预先形成,然后连接在缝合线本体102上。在一个实施例中,各双向保持器104包括穿过的开口116。缝合线本体102可以穿过保持器104的开口116,且保持器104可以连接在缝合线本体102上,使得保持器104相互间隔开。在一个实施例中,保持器104可以使用粘接剂来连接在缝合线本体102上。可以使用的示例粘接剂包括但不局限于:氰基丙烯酸盐(例如氰基丙烯酸辛酯)、纤维蛋白密封剂、动物胶、基于合成N-羟基琥珀醯亚胺的胶粘剂和丙烯酸胶粘剂。也可选择,可以使用外部起动的粘接剂,例如来自可聚合组的材料,如丙烯酸和甲基丙酸烯官能度(functionalities)、丙烯酰胺、甲基丙烯酰胺、衣康和苯乙烯组,它们将通过暴露于高频辐射(例如紫外线光或其它高频波)而有粘接剂性质。可以使用的其它粘接剂包括永久性硅室温硫化胶粘剂、自由基产生胶粘剂如过氧化苯甲酰等。在其它实施例中,保持器104通过使得保持器104熔化在缝合线本体102上而连接在该缝合线本体102上。换句话说,保持器可以热粘接在缝合线本体102上。在一些实施例中,保持器104可以通过使得保持器104利用焊接(例如但不局限于超声波焊接或激光焊接)熔合在缝合线本体102上而连接在该缝合线本体102上。换句话说,保持器104可以焊接粘接在缝合线本体102上。也可选择,保持器104可以利用溶剂来连接在缝合线本体102上,例如通过将保持器104的一部分局部或完全溶解在缝合线本体102中和/或相反。换句话说,保持器可以溶剂连接在缝合线本体102上。
也可选择,保持器104可以形成为缝合线本体102的整体部分(即与缝合线本体成一体结构),例如使用挤出、模制和/或镦头处理。图8、9和10A-10C将用于解释可以用于制造包括双向保持器104的自保持缝合线100的示例方法。
参考图8,单丝220可以通过挤出和精压而形成。如图8中所示,挤出机210在料斗214中接收聚合物212的球粒。聚合物进行熔化,并由螺旋传送器216推动通过挤出模具218,以便形成熔融聚合物212的丝220。丝220最初有在挤出模具218中的孔的形状的均匀截面。挤出的丝经过气隙219,在该气隙219中,丝进行冷却,且聚合物212稍微凝固。挤出的丝220经过精压机230,在该精压机230中,丝220在两个辊232、234之间经过。在精压处理之前和/或之后,丝可以选择地淬火、拉制和/或回火。辊232、234有花纹表面,该花纹表面将丝220的材料压成图9中所示的合适形状。该精压处理可以在聚合物熔点的20-80%之间的温度下进行。在优选实施例中,精压处理在高于聚合物的玻璃转变温度和低于熔化开始期温度的温度下进行。在图8所示的结构中,各辊232、234有在它的外表面上的半球形凹入部分236,这样,当丝220经过该辊时,它获得图9中所示的形状。
如图9中所示,所形成的丝220有间断的半球形节结230。如图10A-10C所示,模具或模250可以用于在模制和/或镦头处理中形成、精压或以其它方式制造如上面参考图1-7所述的双向保持器104。在保持器104中的槽106可以利用模具或模250产生,或者以后切割或机械加工至保持器104中,例如使用刀片等。
上述图2A、2B、6A和6B表示了双向保持器104的可选形状。它们只是用于双向保持器104的一些可能形状,而并不是用于限制。换句话说,其它形状的双向保持器104也可以,并在本发明的范围内。

C.单向保持器
与凸起108类似的凸起也可以布置在单向保持器上,如图11A-11E中所示。在图11A中,自保持缝合线1100包括保持器1104,该保持器1104为锥形,并包括凸起1108。在图11A-11D中,凸起1108从锥形保持器1104的主体的端部1112伸出。保持器1104在缝合线沿由箭头表示的缝合线部署的方向拉动时屈服于细长缝合线本体在组织内的运动,并当缝合线沿与缝合线部署方向相反的方向(与箭头相反的方向)拉动时阻止运动。当缝合线1100沿与箭头相反的方向拉动时,凸起1108将刺入病人的组织中,从而甚至进一步阻止沿与箭头相反的方向运动。
图11B-11E表示了保持器1104的一些变化形式。图11B的保持器1104与图8的保持器1104类似,但是包括更大量的凸起1108。图11C表示了保持器1104的凸起1108的角度可以与保持器1104的锥形主体的壁1114的角度不同(例如,大约为钝角)。图11D的保持器1104与图11C的保持器1104类似,但是包括更大量的凸起1108。图11C还表示了保持器1104可以包括在倾斜壁1114中的一个或多个通气孔1110,其中,当保持器用于通过病人的组织时,该通气孔防止空气由保持器1104捕获。这样很有利,因为可以认为,当并不在保持器1104后面捕获空气时组织将更快愈合。而且,通气孔用于与组织的进一步锚固。该通气孔1110可以包含于图11A-11E的任意保持器1104中。通气也可以通过使得锥形保持器由网状、穿孔或多孔材料形成而提供。图11E的保持器1104包括凸起1108,该凸起1108从保持器1104的锥形主体的倾斜壁1114伸出。在各实施例中,保持器1104在缝合线沿第一缝合线部署方向拉动时屈服于细长缝合线本体在组织内的运动,并当缝合线沿与缝合线部署方向相反的方向拉动时阻止运动。当保持器1104沿与它们的部署方向相反的方向拉动时,凸起1108将刺入病人的组织中,从而甚至进一步阻止沿与部署方向相反的方向运动。
尽管锥形保持器1104可以有圆形截面,但是该截面可以选择为椭圆形或者某些其它形状。还有,应当知道,代替弯曲的倾斜壁1114,它们可以为多面体的面。而且,术语锥形的意思是包括截头锥形。
保持器1104可以预先形成,然后连接在缝合线本体1102上。在一个实施例中,各保持器1104包括穿过的开口1116。缝合线本体1102可以穿过保持器1104的开口1116,且保持器1104可以连接在缝合线本体1102上,使得保持器1104相互间隔开。保持器可以使用粘接剂、通过热粘接、通过焊接连接、通过溶剂连接等来连接在缝合线本体1102上。也可选择,保持器1104可以形成为缝合线本体1102的整体部分(即与缝合线本体成一件结构),例如使用挤出、模制和/或镦头处理,例如以上面参考图8-10C所述类似的方式形成。
保持器1104的壁倾斜,这样,保持器在缝合线沿一个缝合线部署方向拉动时基本屈服于细长缝合线本体在组织中的运动,并当缝合线沿与缝合线部署方向相反的方向拉动时阻止运动。自保持缝合线1100可以是单向的或双向的。当为单向缝合线时,自保持缝合线可以包括:有尖头或有针头的端部,以便当由该端部拉动时能够穿透和通过组织;以及相对端,该相对端在一些实施例中包括锚固件,用于在初始插入点与组织接合,以便限制缝合线的运动。当为双向时,自保持缝合线1100可以包括分组的沿缝合线的一部分朝着一个部署方向延伸的保持器以及分组的沿缝合线的另外部分朝着相反的部署方向延伸的保持器。因此,当植入这样的双向缝合线时,两组保持器都与组织接合,保持器可以阻止缝合线沿任一方向通过组织的运动。还有,双向缝合线可以在缝合线丝线的各端装备有针头。双向缝合线也可以有位于两组保持器之间的转变段。

D.制造自保持缝合线的材料
上述缝合线本体和保持器可以由任意合适的可生物相容材料来制造,并可以通过任意合适的可生物相容材料来进一步处理,不管是增加缝合线的强度、弹性、寿命或其它质量要求,还是使得缝合线装备成实现附加功能(除了将组织连接在一起、重新定位组织或将外来材料连接在组织上)。
上述保持器还可以包括进一步促进组织接合的材料。例如,由组织接合促进材料形成保持器可以提高使得缝合线保持就位的能力。一类这样的组织接合促进材料是多孔聚合物,该多孔聚合物可以被挤出以形成缝合线本体,包括微孔聚合物,且聚合物被挤出有气泡(不管是可生物吸收还是不可生物吸收)。由这样的材料合成的保持器可以有三维栅格结构,它增加了组织接合表面面积,并使得组织渗透至缝合线本体自身中,从而具有促使缝合线成功使用的主结构。而且,通过优化孔的尺寸,可以促进纤维细胞的生长,进一步有利于保持器锚固在组织中。也可选择,亲纤维涂层或药剂可以用于促使更多纤维组织包围保持器,并因此更好地接合。可以用于形成保持器204和/或可以施加在保持器上以便促进组织生长的示例亲纤维材料在美国专利No.7166570中所述,该美国专利No.7166570的标题为“Medicalimplants and fibrosis-inducing agents”,该文献在此通过参引而加入。
一种这样的微孔聚合物是ePTFE(膨胀聚四氟乙烯)。包括ePTFE(和相关微孔材料)的自保持缝合线非常适合需要较强和永久性升高的用途(例如胸部升高、脸升高和其它组织重新定位处理过程),因为缝合线的组织渗透导致提高了缝合线和周围组织的固定和移入,从而良好地保持该升高和有更长寿命。
另外这里所述的自保持缝合线可以提供有用于促进愈合和防止不希望效果(例如疤痕形成、感染、疼痛等)的成分。这可以以各种方式来实现,例如包括:(a)通过将配方直接固定在缝合线上(例如通过向缝合线喷涂聚合物/药物薄膜,或者通过将缝合线浸入聚合物/药物溶液中);(b)通过用物质例如水凝胶涂覆缝合线,依次该水凝胶将吸收组分;(c)在多丝缝合线的情况下通过将配方涂覆的线(或者形成线的聚合物自身)交织在缝合线结构中;(d)通过将缝合线插入套筒或网内,该套筒或网包括或涂覆有配方;或者(e)缝合线自身由组分构成。这样的组分可以包括但不局限于:抗增生剂、抗血管产生剂、抗感染剂、纤维引诱剂、抗疤痕剂、光滑剂、回声(echogenic)剂、消炎剂、细胞周期抑制剂、止痛剂和抗微管剂。例如,组分可以在形成保持器之前施加在缝合线上,这样,当保持器接合时,接合表面基本没有涂层。同样,当缝合线引入时,被缝合的组织与缝合线的涂覆表面接触,但是当保持器接合时,保持器的未涂覆表面与组织接触。也可选择,当希望完全涂覆而不是选择地涂覆缝合线时,缝合线可以在缝合线上形成保持器之后或在形成过程中进行涂覆。在还一可选实施例中,缝合线可以通过只使得缝合线的选择部分受到涂覆而在保持器的形成过程中或形成之后有选择地涂覆。缝合线要用于的特殊目的或者组分的特殊目的可以确定完全涂覆是适合的还是选择地涂覆是合适的:例如,对于光滑涂层,可能希望选择地涂覆缝合线,例如使得缝合线的组织接合表面未涂覆,以便防止损害这些表面的组织接合功能。另一方面,包括组分例如抗感染剂的涂层可以合适地施加在整个缝合线上,而包括组分例如纤维形成剂的涂层可以合适地施加在全部或部分缝合线上(例如组织接合表面)。缝合线的目的也可以确定施加在缝合线上的涂层的种类:例如,具有防增生涂层的自保持缝合线可以用于闭合肿瘤切除部位,而具有纤维形成涂层的自保持缝合线可以在组织中用于重新定位处理过程,而具有防疤痕涂层的自保持缝合线可以用于皮肤上的伤口闭合。还有,缝合线的结构可以影响涂层的选择和范围;例如,具有膨胀段的缝合线可以包括在膨胀段上的纤维形成引诱组分,以便使得该段在组织中进一步固定就位。涂层还可以包括多个组分,这些组分在一起,或者在缝合线的不同部分,其中,多个组分可以选择为用于不同目的(例如止痛剂、抗感染剂和抗疤痕剂的组合),或者用于它们的协同效果。
E.临床使用
除了在前述部分中所述的普通伤口闭合和软组织修复用途,自保持缝合线可以用于多种其它适应症。
这里所述的自保持缝合线可以用于各种牙齿处理过程,即口腔和上颌面的外科手术处理过程,因此可以称为“自保持牙齿缝合线”。上述处理过程包括但不局限于:口腔外科手术(例如除去嵌塞或破坏的牙齿)、提供骨增大的外科手术、修复牙齿脸变形的外科手术、修复下面的创伤(例如面骨骨折和损伤)、牙质原和非牙质原创伤的外科手术治疗、改造外科手术、兔唇或腭裂的修复、先天性颅面变形以及脸部感觉外科手术。自保持牙齿缝合线可以是可降解或不可降解,尺寸范围通常可以为从USP 2-0至USP 6-0。
这里所述的自保持缝合线还可以用于组织重新定位外科手术处理过程,因此可以称为“自保持组织重新定位缝合线”。这样的外科手术处理过程包括但不局限于:脸部提升、颈部提升、眉毛提升、股提升和胸部提升。用于组织重新定位处理过程的自保持缝合线可以根据重新定位的组织而变化;例如,具有更大和更加间隔开的保持器的缝合线可以合适地用于相对较软的组织,例如脂肪组织。
这里所述的自保持缝合线还可以用于在外科手术显微镜下进行的微外科手术处理过程(因此可以称为“自保持微缝合线”)。这样的外科手术处理过程包括但不局限于:外周神经的重新附着和修复、脊柱微外科手术、手的微外科手术、各种塑性微外科手术处理过程(例如脸的重构)、男性或女性生殖系统的微外科手术、以及各种重构微外科手术。当其它选择(例如初始闭合、辅助目的的治愈、皮肤移植、局部折片传递和远侧折片传递)不合适时,微外科手术重构用于解决复杂的重构外科手术问题。自保持微缝合线具有非常小的口径,通常小到USP 9-0或USP 10-0,并可以连接有相应尺寸的针头。它们可以是可降解的或不可降解的。
这里所述的自保持缝合线可以用于在类似小口径范围内用于眼外科手术处理过程,因此称为“眼自保持缝合线”。这样的处理过程包括但不局限于:角膜成形术、白内障和各种玻璃质视网膜微外科手术处理过程。眼自保持缝合线可以是可降解的或不可降解的,并连接有相应小口径的针头。
自保持缝合线可以在各种兽医用途中用于动物康复的很多种外科手术和创伤治疗目的。
尽管通过本发明的几个示例实施例详细表示和介绍了本发明的实施例,但是本领域技术人员应当知道,它并不是将本发明限制于这里所述的特殊实施例。在本质上不脱离本发明的新颖教导和优点的情况下,特别是根据前述教导,可以对所述实施例进行各种变化、省略和添加。因此,它将覆盖所有这些变化、省略、添加和等效物,它们可以包含在由下面的权利要求确定的本发明范围内。
Claims (10)
1.一种在施加于组织上的处理过程中使用的缝合线,包括
细长缝合线本体,该细长缝合线本体包括第一端和第二端;以及
多个可转变保持器,这些保持器相互间隔开,并在所述第一端和第二端之间从所述细长缝合线本体上伸出;
其中,在穿过组织部署之前,各所述可转变保持器可沿第一方向和与该第一方向大致相反的第二方向穿过组织来部署;以及
各所述可转变保持器的形状可以根据该保持器穿过组织部署的方向而以两种方式中的任意一种来转变;
一旦所述保持器的形状由于该保持器沿第一方向穿过组织部署而转变,在沿第一方向拉动细长缝合线本体时该保持器将基本屈服于该细长缝合线本体在组织内的运动,并将阻止细长缝合线本体沿与第一方向大致相反的第二方向运动;以及
一旦所述保持器的形状由于该保持器沿第二方向穿过组织部署而转变,在沿第二方向拉动细长缝合线本体时该保持器将基本屈服于细长缝合线本体在组织内的运动,并将阻止细长缝合线本体沿第一方向运动。
2.根据权利要求1所述的缝合线,其中:沿一个方向部署的所述可转变保持器的数目和沿大致相反方向部署的所述可转变保持器的数目可以在处理过程中决定。
3.一种在施加于组织上的处理过程中使用的缝合线,包括
细长缝合线本体,该细长缝合线本体包括第一端和第二端;以及
多个可转变保持器,这些保持器相互间隔开,并在所述第一端和第二端之间从所述细长缝合线本体上伸出;
其中,当保持器穿过组织部署时,各所述可转变保持器的形状转变。
4.根据权利要求3所述的缝合线,其中:当所述保持器的形状由于该保持器沿第一方向穿过组织部署而转变,在沿第一方向拉动细长缝合线本体时该保持器将基本屈服于该细长缝合线本体在组织内的运动,并将阻止细长缝合线本体沿与第一方向大致相反的第二方向运动。
5.根据权利要求4所述的缝合线,其中:当所述保持器的形状由于该保持器沿第二方向穿过组织部署而转变,在沿第二方向拉动细长缝合线本体时该保持器将基本屈服于细长缝合线本体在组织内的运动,并将阻止细长缝合线本体沿第一方向运动。
6.根据权利要求3所述的缝合线,其中:各所述可转变保持器可沿彼此大致相反的两个方向穿过组织来部署,但是一旦保持器的形状由于该保持器沿一个方向穿过组织来部署,该保持器就将阻止沿大致相反方向运动。
7.一种在施加于组织上的处理过程中使用的缝合线,包括
细长缝合线本体,该细长缝合线本体包括第一端和第二端;以及
多个保持器,这些保持器相互间隔开,并在所述第一端和第二端之间从所述细长缝合线本体上伸出;
其中,
各所述保持器包括槽或者穿孔,当保持器穿过组织部署时,该保持器可以沿该槽或者穿孔弯曲;
在穿过组织部署之前,各所述保持器可沿第一方向和与该第一方向大致相反的第二方向穿过组织来部署;
一旦所述保持器沿第一方向穿过组织来部署,在沿第一方向拉动细长缝合线本体时该保持器将基本屈服于该细长缝合线本体在组织内的运动,并将阻止细长缝合线本体沿与第一方向大致相反的第二方向运动;以及
一旦所述保持器沿第二方向穿过组织来部署,在沿第二方向拉动细长缝合线本体时该保持器将基本屈服于细长缝合线本体在组织内的运动,并将阻止细长缝合线本体沿第一方向运动。
8.根据权利要求7所述的缝合线,其中:沿一个方向部署的所述保持器的数目和沿基本相反的方向部署的所述保持器的数目可以在处理过程中决定。
9.根据权利要求7所述的缝合线,其中:多少缝合线沿第一方向穿过组织部署和多少缝合线沿第二方向穿过组织部署可以在处理过程中决定,因为缝合线没有预定的转变段或转变点。
10.根据权利要求7所述的缝合线,其中:所述保持器的所述槽或者穿孔从该保持器安装在所述细长缝合线本体上的位置径向延伸至保持器的外边缘。
