Abstract

背景技术

发明内容

具体实施方式
本发明的复合伤口敷料装置通过使用真空储存器而促进伤口的愈合。真空储存器使伤口受到真空或低于大气压的压力,以便将包括液体渗出物的伤口流体有效地从伤口基底吸走,而并不连续使用真空源或泵。因此,根据伤口的性质和严重性,真空压力可以一次性或以变化间隔供给,直到复合伤口敷料由渗出液浸透或伤口愈合。当伤口敷料由渗出液浸透且伤口没有愈合时,特定层和/或所有层的复合伤口敷料可以更换,并可以重复施加低于大气压的压力的处理。
下面参考图1-3,本发明优选实施例的复合伤口敷料表示为具有以并列或重叠关系布置的多层的物品的形式。该多层包括但不局限于:底层或基层102、吸收/填塞层104、粘附层106和顶层108,该顶层108包括和/或确定了内部真空储存器110。
基层102与伤口基底“W”直接接触。基层102通常为多孔,从而允许低于大气压的压力能够通向伤口基底。在一个优选实施例中,基层包括“无粘附性”材料。这里使用的术语“无粘附性”是指材料并不粘附在伤口基底中和周围的组织上。这里使用的术语“多孔”是指材料包含大量小孔或孔隙,这些小孔或孔隙允许所有类型的伤口流体通过该材料而通向上面的敷料层。伤口流体通过多孔材料可以为单向,这样,伤口渗出液不会流回至伤口基底。这种方向流动特征可以为形成于材料层中的定向孔的形式,即吸收性与基层102不同的材料或者促进定向流动的选择特定材料的层叠材料。用作基层102的实例材料包括接触层,由Kendal Corp(TycoHealthcare的一个部门)出售的商标名为在可选实施例中,基层102可以包括粘附性材料。
此外,试剂例如水凝胶和药物可以粘在或涂覆在基层102上,以便降低伤口的生物负担,促进愈合以及降低与更换或除去敷料相关的疼痛。药物例如包括:抗菌剂、生长因子、抗生素、镇痛剂、清创剂等。而且,当使用镇痛剂时,镇痛剂可以包括这样的机构,该机构将能够在除去或更换敷料之前释放该药剂。释放机构的示例激发可以是温度变化。
靠近基层102或形成基层102的复合结构的层是填塞/吸收层104。伤口敷料100的填塞/吸收层104将吸收和捕获伤口流体和渗出液。示例吸收材料包括:泡沫材料、无纺复合织物、水凝胶、纤维素织物、超级吸收聚合物以及它们的组合。通常,吸收/填塞层104可以吸收大约100立方厘米(cc)或更多的伤口流体。优选是,吸收材料包括抗菌剂敷料,由Kendal Corp(TycoHealthcare的一个部门)出售的商标名为在一个优选实施例中,吸收/填塞层104可以预成形或形成为与伤口基底的不同形状相符。本领域技术人员应当知道,吸收/填塞层104可以形成为任意合适形状。对形状的仅有要求是,填塞/吸收层104适合治疗特殊形状的伤口。
另外,吸收/填塞层104可以用药物处理。药物例如包括:抗感染剂例如防腐剂或其它合适抗菌剂或抗菌剂的组合、聚六亚甲基双胍(下文中称为“PHMB”)、抗生素、镇痛剂、再生因子例如维生素、生长因子、营养物质等以及冲洗剂例如等渗压盐溶液。
继续参考图1-3,粘附层106至少包围伤口敷料100的周边,以便环绕伤口基底,从而提供环绕伤口基底“W”周边的密封。例如,密封机构可以为粘接在伤口基底“W”周围的层上的粘接剂,或者为直接施加在皮肤上的粘接剂。粘接剂必须可接受地粘在伤口基底“W”皮肤周围的组织“t”上,例如伤口周围区域,并可用于皮肤上,且没有接触恶化(例如,粘接剂应当优选是无刺激和不引起过敏的)。粘接剂可以半渗透以便允许接触的皮肤传递水汽,或者不可渗透。另外,粘接剂可以通过外部刺激(例如热或者给定的流体溶液或化学反应)来激励或去激励。粘接剂例如包括Kendall Corp.(TycoHealthcare的一个部门)出售的ULTEC Hydrocolloid Dressing。
粘附层106还可以为靠近吸收/填塞层104的整层形式,或者优选是环形或“圆环形”,如图所示。优选是,粘附层106并不粘接在吸收/填塞层104上,以便能够很容易地更换吸收/填塞层104。在优选实施例中,粘附层106至少粘接在基层102的周边。顶层108的周边部分108a又可以粘在粘附层106上,以便提供环绕伤口周边的密封。也可选择,粘附层106可以位于顶层108的周边部分108a上,并固定在伤口基底“W”周围的组织“t”上,且并不粘接在基层102上。还可选择,顶层108的周边部分108a可以包括粘接剂表面。应当知道,可除去的接触衬垫也可以用于在使用前保护粘附层106的粘接剂表面。
顶层或上层108通常密封伤口敷料100的顶部,并有助于在伤口敷料100内保持合适的真空水平。在一个优选实施例中,顶层108包括柔性透明敷料(由Kendall Corp.(TycoHealthcare的一个部门)制造,商标名为)。Polyskin为透明的半渗透性材料,它允许水汽和氧与伤口部位交换,并提供了对于细菌和流体污染物的屏障。也可选择,顶层108可以为不可渗透。还可选择,顶层108可以包括例如圆顶形状的弹性材料,例如弹性体材料。
顶层108限定了密封或封闭的真空储存器110。真空储存器110优选是保持合适的真空水平预定时间,该预定时间足够促使或完成伤口基底“W”的治愈,即从伤口基底“W”吸走伤口流体和渗出液,同时使伤口受到低于大气压的压力。真空可以根据需要重新施加,以便保持治疗效果。真空可以根据需要而连续或者间断。
最好如图1中所示,真空储存器110限定于顶层108的圆顶中。如图2中所示,一旦施加真空,上层108的圆顶通过在顶层108下面产生的真空或低于大气压的储存器110而朝着吸收/填塞层104向下拉动。通常,顶层108包括与真空储存器110流体连通的真空孔或连接器114。优选是,真空孔114包括单向阀(以参考标号116示意表示),该单向阀提供了抽吸单向流,并可以提供用于使复合伤口敷料100能够与真空源112连接的装置。单向阀116可以包含在真空孔114中,或者也可选择与真空源112“成一直线”。柔性管118与真空孔114和真空源112连接。管118提供了从真空源112对伤口的抽吸,并使得伤口流体能够从伤口敷料100传送。管118可以由PVC,基于硅酮的材料或其它柔性材料(聚合物)来制造。管118可以任选地包括连接到用于伤口排出物和碎屑的收集罐120上的连接件。因此,真空源112可以通过复合伤口敷料100和管118而将伤口流体吸入罐120中。在本发明的优选实施例中,罐120为便携式的,这样,病人将有运动的自由,而不需要限制于固定位置。罐120还可以装有吸收材料,以便吸收伤口流体和渗出液。
真空源112可以通过例如人工泵这样的装置而向伤口施加真空,该人工泵在共同转让的美国专利No.5549584(授予Gross)中公开,它的整个内容被本文参引。也可选择,真空源112可以包括自动泵。通常,真空水平在大约20mmHg至大约500mmHg之间的范围内,更优选是在大约40m mHg至大约125mmHg之间的范围内。自动泵可以是壁吸装置,例如可在急性或亚急性护理设备中可用的装置。自动泵可以为便携式泵的形式。便携式泵可以包括较小或小型泵,它保持或抽吸合适的治疗真空水平。在优选实施例中,泵是便携式、重量轻、电池操作的抽吸泵,该泵安装在管的远端上。通常,真空源112有调节装置,以便施加用于伤口愈合的优化真空压力。而且,真空源112优选包含用于当没有获得优化真空压力时检测系统中的泄漏的机构。优选是,真空源112还将包含指示器(未示出),以便当获得优化真空压力时进行指示。也可选择,为挤压球形式的手泵或脚泵可以用作真空源112。
优选是,泵用作真空源112。普通泵包括隔膜或音圈驱动类型,它可以供给直到50cc/分钟的可变真空。
下面参考图1-4,顶层108可以包括进入门122,以便提供通向伤口敷料100内部和/或伤口基底“W”的进口。门122可以是与顶层108形成一体的折片,或者可以是通过铰链等而与顶层108连接的单独部件。门122优选是可重新密封的,以便保持真空储存器110的完整性,并提供相对顶层108的密封。用于可释放地密封该门122的一种合适装置包括卡扣结构、舌片和槽结构、“zip结构、粘接剂、等。门122优选提供了通向伤口基底“W”的进口,以便使临床医生能够监测伤口的状态、改变吸收/填塞层104,或者对伤口施加附加医疗处理,例如所需的抗菌剂、生长因子、清创剂或其它伤口愈合剂。一旦完成合适处理,门122将相对于顶层108重新密封,以便保持真空储存器110的完整性。图4表示了当门122处于打开位置时通过该门122取出吸收/填塞层104。如上所述,新的吸收/填塞层104随后可以通过门122引入,以便从伤口基底“W”吸收渗出液。
下面参考图3和5并结合图1-2,伤口敷料100可以包括安装在顶层108上的视觉指示器装置124,以便提供在伤口敷料100中的真空压力的视觉指示。视觉指示器装置124可以包括至少一个电子位置指示器,用于检测顶层108和伤口基底的相对位置,因此检测在伤口敷料100中的真空状态。根据本实施例,顶层108可以通过构成顶层108的材料或通过包含在顶层108中的增强(例如弹性体)部件而具有弹性特征。
在图6所示的一个实施例中,视觉指示器装置124包括至少一个(优选是三个)位置敏感开关126a-126c以及自供电的电子信号模块128。模块128可以包括电子信号模块电路板130、电池电源132和至少一个换能器(包括与电路板130电连接138的三个发光二极管LED134a-c和/或扬声器136)。LED 134a-c的颜色分别是红色、黄色和绿色。位置敏感开关126a-126c各自有压力传感器,该压力传感器电桥接视觉指示器装置124的触点140a、140b。
在所示实施例中,三个位置开关126a-c安装在顶层108上。开关126a-c包括布置为一系列减小直径的环形同轴元件的开关板或接触臂。也可选择,开关可以是从顶层108向下悬垂的线性结构。开关126a-c的开关板或接触臂为在真空储存器110内从顶层108向下延伸预定长度(图4和5)。压力指示器124的外部开关126a的接触臂具有最大长度。中间开关126b的接触臂的长度小于外部环126a的接触臂的长度。内部开关126c的接触臂具有最小长度。开关126a-c的触点140a集成在接触臂中。开关126a-c的触点140b可以布置在吸收/填塞层104的顶表面上,或者集成在吸收/填塞层104中,大致与它们的相应接触臂纵向对齐。也可选择,位置传感器可以是磁邻近传感器。自供电电子信号模块130可以是用于当开关闭合时发出光和/或声音等的任意普通模块。
当顶层108通过真空储存器110中的真空压力而朝着它的真空状态向下拉动时(图2),内部开关126c与布置在吸收/填塞层中的各连接器或触点140b接触,从而使视觉指示器装置124通电,以便使得在安装于顶层108上的压力指示器装置中的绿LED 134c照亮。LED 134c的绿光指示真空储存器110的完全真空状态。如图所示,开关126a、126b还可以在顶层108的该真空状态中与它们的相应连接器接触。不过,可以设想电路板130将包括用于当开关126c与它的相应连接器接触时超越这两个开关的电接触的电路。
当真空压力降低,且顶层108的圆顶开始呈现它的正常状态(图1)时,例如通过上述顶层108的弹性特征,内部开关126c将失去与吸收/填塞层104的接触,同时中间开关126b保持/形成与它的相应触点140b的电接触。中间开关126b的电连接使得黄色LED 134b照亮。黄色LED134表示真空储存器110的局部真空或边际真空状态。当真空压力进一步减小且顶层108朝着图1的完全膨胀或正常状态运动,外部开关126a是与吸收/填塞层104内的相应触点140b接触的剩余开关。在这种情况下,红色LED 134a通电,并可由临床医生基本可见,从而提供真空储存器中的真空已经耗散或几乎耗散的警告(即低于大气压的压力将封闭和不再存在)。因此,真空源112可以人工或自动驱动,以便在真空储存器110中重新建立真空状态。进一步的真空损失将导致剩余开关126c失去它的触点,其中,不再有光可由病人看见。
还设想电路板130可以没有前述超越电路。因此,在敷料100的完全真空状态下,各绿色、黄色和红色LED 134a-134c都将照亮,而在部分真空状态下,黄色和红色LED 134a、134b将照亮,而在警告状态下,红色LED 134a将照亮。还设想当达到任意前述真空状态时,扬声器136将发出声音警告。
图7表示了视觉压力指示器124的可选实施例。根据该实施例,视觉压力指示器124安装在吸收/填塞层104上或以并列关系定位在吸收/填塞层104上。顶层108优选是透明的,以便能够通过顶层108观察视觉指示器124和真空储存器110内部。视觉压力指示器124的定位反转,或者布置在它的背面,这样,各开关126a-c向上朝着顶层108延伸。各连接器或触点140b包含在顶层108中。在图2的完全真空状态或情况下,顶层108的触点140b与开关126c接触,从而使LED 134c通电,以便向临床医生显示绿色。在愈合过程中,当真空储存器开始失去它的真空时,顶层108朝着它的打开状态(图1)运动,内部开关126c失去与顶层108的相应连接器140b的接触,从而导致与黄色LED 134b电接触,指示真空储存器110的局部真空状态。在真空储存器110中继续损失真空将使得连接器140b与外部开关126c电接触。LED 134c与外部开关126c的该接触通过存在红色来指示,并可以对应于在真空储存器110中失去真空或接近失去真空,从而促使临床医生驱动真空源112或采取其它临床措施。
图8表示了另一实施例,其中,LED 134a-134c补充有附加视觉标记。该视觉标记可以包括与真空储存器110中的真空状态相对应的各种标志。当达到合适水平的真空时,“微笑”标志152将照亮,从而指示合适的真空状态。局部真空状态将导致“注意三角形”标志154照亮。失去或几乎失去真空将导致“八边形”标志(没有真空)156照亮。本领域技术人员应当知道实现该目的的电子电路的设计。
图9-11表示了本发明的可选实施例。根据该实施例,视觉指示器装置124可以包括布置在顶层108底侧的一系列环200,202,204,例如印在顶层108上,并布置成同心关系,如图所示。这些环包括外部环200、中间环202和内部环204,颜色分别为红色、黄色和绿色。各颜色的环200,202,204定位成根据真空储存器110中的真空状态或情况而与吸收/填塞层104接触。在图10所示的完全真空情况下,全部环200,202,204都将与吸收/填塞层104接触,因此进行驱动,并可通过伤口敷料的顶部看见。当储存器内的真空耗散或减少时,顶层110从吸收/填塞层104向外拉动。在具有弹性圆顶的一个实施例中,当真空减小时,内部环204首先失去接触,随后分别是中间环和外部环202,200。当各颜色环200,202,204都失去与吸收/填塞层104的接触时,各环变得不能从伤口敷料100的上面看见,因此向临床医生指示在真空储存器中的真空状态。环200,202,204可以包括电子开关,以便以上面结合图1-6的实施例所述的方式来驱动。也可选择,各环200,202,204可以包括分析测试条装置,它例如可以检测预定分析物在(包含于吸收/填塞层104中的)渗出液中的存在。当与预定分析物接触时,各环200,202,204的测试条装置呈现一定颜色,例如红色、黄色或绿色。当各环200,202,204失去与预定分析物的接触时,各环的颜色可以褪去或呈现中性颜色。适用于本发明的颜色测试条装置的一个实例在授予Carroll等的美国专利No.7049130(公开日为2006年5月23日,该文献的整个内容被本文参引)中公开。本领域技术人员可以确定测试条的参数和特征,以便实现上述目的。
此外,图9-11的实施例的门122用于绕铰链208枢轴转动,以便进入真空储存器110中。门122的开口布置在伤口敷料102的周边附近,因此当门122沿铰链208枢轴转动或打开时将提供相对较大的进入开口。这有利于取出和更换吸收/填塞层104。
还可以考虑,伤口敷料装置可以包括用于刺激组织生长和/或愈合的外部装置或设备。例如,超声波换能器可以包含在伤口敷料装置中,以便施加机械能,用于组织的治疗,例如将热或振动能量引导至伤口区域以便刺激愈合,和/或通过真空促进除去渗出液,和/或通过皮肤将各种药物引入人体内。其它类型的传感器也可以考虑包含在伤口敷料装置中,包括氧、化学药品、微生物和/或温度传感器。检测伤口区域附近的氧将帮助临床医生确定伤口愈合的状态。出现升高温度可以指示感染。

Claims (19)
1.一种伤口敷料装置,它包括:
伤口敷料部件,该伤口敷料部件的尺寸设置成用于相对于伤口基底定位,伤口敷料部件包括内部真空储存器,并具有与真空储存器连通的孔,用于将低于大气压的压力施加给真空储存器,以便于伤口基底的治疗和从伤口基底中除去流体,伤口敷料部件包括与它相连的进入门,该进入门可选择地在基本封闭真空储存器的封闭位置和允许进入真空储存器的打开位置之间运动。
