Description
技术领域
本发明应用于传统的内窥镜和开放式手术器械,还应用于机器人辅助手术。本发明还进一步涉及可调整的手术植入带,如用于治疗肥胖的束胃带。
背景技术
患有病理性肥胖的世界人口百分比正在持续增长。严重的肥胖者易于增加患心脏病、中风、糖尿病、肺病和意外事故的危险。由于病理性肥胖对患者生命的影响,人们正在研究治疗病理性肥胖的方法。
人们已尝试病理性肥胖的多种非手术治疗方法,但未获得实质上的永久成功。已尝试了饮食建议、行为修正、用金属丝绑束患者下鄂使其关闭以及药理学方法,但都不能医治该病症。在治疗该病症中还采用了通过非手术方法插入体内的机械装置,如利用胃气囊来充满胃部。但是这类装置不能长时间使用,因为它们经常会引起严重的疼痛,不得不定期取出,从而中断治疗。因此,医学界已研制了治疗病理性肥胖的手术方法。
大多数病理性肥胖的手术治疗方法可通常分为侧重于阻止食物吸收(吸收障碍),或者胃部束缚以使患者感到胀饱(胃部束缚)两类。最普通的吸收障碍和胃部束缚技术是胃部旁路。在这种技术的各种方案中,将胃水平分为两个分隔的胃囊,上部胃囊具有较小的食物容量。上部胃囊通过一个小的开口连接到小肠或空肠,这通过极大地减小可用的胃部而限制食物处理。由于食物绕过很大一部分肠,就大大地减少了食物吸收的量。
上述方法具有许多缺点。通常上述操作是在开放的手术环境中进行的。目前的微创技术对于外科医生来说难于掌握,且具有许多其它缺点。而且,这种手术激烈并不容易回复原状的想法会给患者带来很大的不安。此外,所有吸收障碍技术对患者随时存在危险并产生副作用,包括营养不良和倾倒综合征。
因此,许多患者和医生更愿意采用胃部束缚方法来治疗病理性肥胖。最普通的方法之一是植入可调整的束胃带。可调整束胃带的实例在已授权给Kuzmak的美国专利4,592,339、已授权给Kuzmak的美国专利RE36176、已授权给Kuzmak的美国专利5,226,429、已授权给Jacobson的美国专利6,102,922以及已授权给Vincent的美国专利5,601,604中公开,这些申请的全部内容在此引入作为参考。根据目前的实际应用,束胃带可操作地放置以环绕胃部。这将胃部分为上下两部分,在其间有一个开口。上部分,即胃囊相对较小,而下部分相对较大。胃的较小分割部分有效成为患者新的胃,从而需要极少的食物就可使患者感到胀饱。
束胃带一旦放置在胃部周围,其两端彼此固定在一起,并且,该束胃带通过将胃壁的一部分折叠在该束胃带上且用放置在其中的缝合线缝合被折叠的组织而使其安全地固定在适当的位置中,从而防止该束胃带滑脱以及防止被环绕的开口扩大。束胃带通常包括柔性的基本上不可延伸的部分,其具有连接在其上的可延伸的可膨胀部分。该可膨胀部分与远处注入位置-即端口-流体连通。膨胀流体向可膨胀部分的内部注入或从可膨胀部分的内部抽出以便在植入期间或植入之后调整开口的尺寸。通过扩大该开口,患者可在不感觉胀饱的情况下吃下更多的食物,但将不会较快地减轻体重。通过减小该开口的尺寸,则可发生相反的情况。医生定期调整开口的尺寸以调整体重降低的速度。
对于大多数用于诸如束胃带这样的装置中的流体注入端口,导管通过端口侧面上的连接管连接到端口上。这导致导管从端口向远端横向伸出。导管的该位置可导致医生在试图刺穿在端口上的流体贮存器时用针不小心刺穿导管。这会引起导管内泄漏,其需要采取措施进行修补。
在1963年12月10日授权给Schulte的美国专利NO.3,310,051说明了一种解决该问题的技术方案,其内容在此引入作为参考。该参考文献公开了一个端口,导管连接管以及导管在该端口处从背面向远侧延伸。但是,该装置的导管连接管伸出到该装置护罩之外。该伸出的导管连接管会刺穿或刺激周围组织。
发明内容
本发明提供一种用于可调整束胃带的可植入手术注入端口。该可植入手术注入端口具有护罩,该护罩具有带有凹部的远侧背部。该护罩还包括近端开口和流体贮存器。该端口还具有在开口附近连接到护罩上的针可穿透隔片。该端口还包括与贮存器流体连通的导管连接构件。该构件连接到背部的凹部上且从该处向远侧延伸,从而连接构件不会延伸至背部最远端部分的远侧。

具体实施方式
现在参照附图,其中在全部视图中相同的标号表示相同的部件,如上所述,图1中示出了上述引入的参考文献中所描述类型的可调整束胃带101。该束胃带101包括伸长的柔性可膨胀部分110,也可称为气囊部分,以及伸长柔性的且基本上不可延伸的带部120。正如图2所示以及如上所述的,当该束胃带101在使用时,其围绕胃111放置,且束胃带的两端相互固定在一起。
再回来参见图1,其示出了一种根据本发明制成的可手术植入的流体注入端口10。可膨胀部分110通过导管52与注入端口10流体连通。导管52具有连接到端口10上的近端53和连接到可调整束胃带101上的远端55。端口10在医疗领域的装置中具有广泛的应用,并且,不仅用于束胃带。例如,该端口还可用于进行药物传输的血管通道。

如图3和图4所示,可手术植入的注入端口10包括护罩12。护罩12可由多种材料制成,包括不锈钢、钛或聚合物材料。护罩12具有在远侧的背部14以及从背部14以一角度向近端延伸的圆周壁部16。壁部16形成近端开口18以及位于开口18和背部14之间的流体贮存器20。该端口包括在开口18附近连接到护罩上的针可穿透隔片22,从而覆盖该开口并且密封贮存器20。隔片22可由包括聚硅氧烷在内的多种材料制成。隔片22优选放置在足够靠近近端的位置,从而使贮存器20的深度足以暴露给诸如休伯(Huber)针的开放尖端,从而可以产生流体传输。优选地设置隔片22使其被针穿透时和当针抽出后自身密封。在一个实施方案中,隔片由聚硅氧烷制成,当其连接到护罩上时受到压缩。
端口10还包括与贮存器20流体连通的导管连接构件30,该连接构件连接到护罩12的背部14上,优选在其中心13上,且从贮存器20向远侧伸出。这种向远侧延伸的设置消除了导管从贮存器径向伸出从而会被医生刺穿的问题。构件30可以是一个独立的部件,其可通过焊接干涉配合、螺纹配合、粘合或其它连接方式连接到背部14上,或者它可以与背部14是一体的,如通过铸模。在图3所示的实施方案中,背部14包括连接构件30连接到其上的凹部。凹部15允许该连接构件从贮存器20向远侧伸出,从而使其不会延伸到背部14的最远端部分17的远侧。使连接构件30完全容纳在护罩内的优点是该构件不会侵蚀任何组织,且导管将保持垂直于该端口底部表面,从而不会承受任何可能最终导致导管故障的弯曲负载。
如图3所示,连接构件30包括一个或多个从其上延伸的径向伸出凸缘32。凸缘32的直径优选大于导管52(通常由聚硅氧烷或其它聚合物材料制成)的松弛直径。这样导管52就可以弹性扩展以在凸缘32上配合,从而提供更好的干涉配合并在导管52和构件30之间提供良好的流体密封。
在实际操作中,医生将端口10连接到患者身上。之后,他/她将导管连接到装置的背部,从而使导管与贮存器流体连通并从贮存器向远侧伸出。这消除了导管从贮存器径向伸出从而会被医生刺穿或由于90度弯曲在患者体内扭结的问题。对于本领域技术人员来说,上述发明对于其它类型的植入带具有同等应用是显而易见的。例如,那些用于治疗排泄物失禁的带。在美国专利6,461,292中说明了一种这样的带,其全部内容在此引入作为参考。这些带还可用于治疗尿失禁。在美国专利申请2003/0105358中说明了一种这样的带,其全部内容在此引入作为参考。这些带还可用于治疗胃灼热和/或返酸。在美国专利US6,470,892中说明了一种这样的带,其全部内容在此引入作为参考。这些带还可用于治疗阳萎。美国专利申请2003/0114729中说明了一种这样的带,其全部内容在此引入作为参考。

Claims (9)
1.一种用于可调整束胃带的可植入的手术注入端口,其包括:
a.护罩,其具有带凹部的远侧背部、近端开口和位于其间的流体贮存器;
b.针可穿透隔片,其在所述开口附近连接到所述护罩上;以及
c.与所述贮存器流体连通的导管连接构件,所述导管连接构件连接到所述背部的所述凹部上并从该处向远侧伸出,从而使该导管连接构件不会延伸到所述背部最远端部分的远侧。
2.如权利要求1所述的注入端口,其特征在于,所述导管连接构件连接到所述背部的中心。
3.如权利要求1所述的注入端口,其特征在于,所述导管连接构件包括径向延伸的凸缘。
4.如权利要求3所述的注入端口,其还包括导管,其中所述导管的内径小于所述径向延伸凸缘的外径。
5.如权利要求1所述的注入端口,其还包括导管,其中所述导管连接到所述导管连接构件上且从该导管连接构件向远侧延伸。
6.如权利要求1所述的注入端口,其特征在于,所述护罩包含钛。
7.如权利要求1所述的注入端口,其特征在于,所述隔片在由针穿透时和针抽出后自身密封。
8.如权利要求1所述的注入端口,其特征在于,所述隔片包含聚硅氧烷。
9.如权利要求1所述的注入端口,其特征在于,所述注入端口还包括用于将所述端口连接到患者身上的装置。
