Abstract
The invention relates to a method and device for placing implants (1) using a surgical template (11) which is made from tomographic cuts in the patient's jawbone (7). According to the invention, step drills (4) and calibrating drills (5), having a single standard diameter for each type of implant (1), are guided through drill bushings (18) which are inserted into bores (15) in the template (11) in order to produce any drilling sequence corresponding to an implant plan. The penetration depth of the drills (4, 5) is controlled by the height of the bores (15) or by the drill rings (21). The aforementioned template (11) bores (15) serve as a guide for the precise placement of the implants (1) owing to the adapted implant supports (3). Moreover, washers (23), which are mounted around the implant supports (3), limit compression i n relation to the implants (1) while said implants are being placed (20). The above-mentioned characteristics serve to limit the required number of drills (4, 5) and implant supports (3) to the longest models only. The inventive method and device are particularly suitable for computer-assisted implantolo gy systems.
Claims (34)
1) Method for the placement of dental implants (1), of the type comprising the following steps : a) placement (6) of a scannographic guide in the mouth of the patient, b) acquisition (8) by computer of the scanner data of said guide, as well as of the mandible or the maxilla (7) of said patient c) simulation (9) on computer of said mandible or said maxilla (7) starting from said data, d) generation (10) by computer, under control of the practitioner, of implant planning parameters based on said simulation (9), e) control (12) by computer based on the planning parameters of a device for the production of a template (11) featuring bore tubes (15) with predetermined inclinations and positions, f) securing (13) in said bore tubes (15) the guiding cylinders (14) of one single standard dimension predetermined in function of the type of said implants (1), g) insertion (16) in said cylinders (14) of drilling tubes (18) arranged so as to control the direction and the depth of the insertion of the drills (4,5), h) drilling (19) through said tubes (18) of holes (17) which are intended to receive the implants (1) into said mandible or said maxilla (7) by way of said drills (4,5) that are used successively i) placement (20) through said cylinders (14) by way of fixture mounts (3) of implants (1) in said holes (17), said method being characterized in that - the tubes (18) are produced beforehand (2) with one single standard inner diameter predetermined in function of the type of said implants (1), - production beforehand (2) of a first set of said drills (4,5), consisting of staged drills (4), of which the maximal standard diameter corresponds to the inner diameter of said tubes (18), - production beforehand (2) of a second set of said drills (4,5) consisting of calibration drills (5) of which the nominal standard diameter corresponds to the inner diameter of the tubes (18), - the drilling (19) consists of using for each of the holes (17) first one of the staged drills (4), then one of the calibration drills (5).
2) Method for the placement of dental implants (1) according to claim 1, characterized in that said placement (20) is guided by fixture mounts (3) gliding in said cylinders (14).
3) Method for the placement of dental implants (1) according to any one of the preceding claims characterized in that said parameters comprise the heights of said bore tubes (15) so as to control without additional means the depth of insertion of the drills (4,5) in the maxilla or the mandible (7).
4) Method for the placement of dental implants (1) according to any one of the preceding claims characterized in that a set of rings (21) with inside diameters corresponding to the diameter of said drills (4,5) is produced beforehand (2) and in that said method comprises one first intermediate step consisting of the fixing of at least one of said rings (21) on said drills (4,5) so as to control the depth of penetration.
5) Method for the placement of dental implants (1) according to any one of preceding claims 3 or 4 characterized in that the drilling (19) consists of using staged drills (4) and calibration drills (5) of one single standard length predetermined in function of the type of said implants (1), independent of the depth of the holes (17) to be obtained.
6) Method for the placement of dental implants (1) according to any one of the preceding claims characterized in that a set of washers (23) with and inside diameter corresponding to the diameter of the fixture mounts (3) is produced beforehand (2) and that said method comprises a second intermediate step consisting of the fixing of at least on of said washers (23) on said fixture mounts (3) so as to control the depth of the penetration.
7) Method for the placement of dental implants (1) according to claim 6 characterized in that the placement (20) consists of using fixture mounts (3) of one single standard length, predetermined in function of the type of implants (1), independent of the height of said implants (1) to be placed.
8) Method for the placement of dental implants (1) according to any one of the preceding claims characterized in that the insertion (16) is ensured by screwing.
9) Method for the placement of dental implants (1) according to any one of the preceding claims characterized in that the securing step(13) is ensured by pasting.
10) Device for the placement of dental implants, adapted for the application of the method as claimed in anyone of preceding claims 1 to 9, of the type comprising:
a) a scanning guide destined for being placed in the mouth of the patient, b) a first system for the acquisition on a computer of the data from said guide, and of the mandible or the maxilla (7) of the patient, c) a second system for the simulation on a computer of said mandible or said maxilla (7) starting from said data, d) a third system for the generation by computer of the implant planning parameters based on said data, e) a fourth system for the formation of a drilling template (11) controllable by computer, f) a fifth system for controlling by computer said fourth system starting from said parameters, g) bore tubes (15) in said drilling template (11) on with predetermined inclinations and positions fitted with guiding cylinders (14) of standard dimension predetermined in function of the type of said implants (1), h) drilling tubes (18) in coaxial arrangement in the upper part (15) of said cylinders (14), i) drills (4, 5) with diameters corresponding to the inside diameters of said drilling tubes (18), and j) fixture mounts (3), characterized in that said drilling tubes (18) all display a single predetermined standard inner diameter in function of said implants (1) and in that said drills (4, 5) include a first set of staged drills (4) and a second set of calibration drills (5) of which, respectively, the maximal standard diameter and the nominal standard diameter correspond to the inner diameter of said drilling tubes (18).
a) a scanning guide destined for being placed in the mouth of the patient, b) a first system for the acquisition on a computer of the data from said guide, and of the mandible or the maxilla (7) of the patient, c) a second system for the simulation on a computer of said mandible or said maxilla (7) starting from said data, d) a third system for the generation by computer of the implant planning parameters based on said data, e) a fourth system for the formation of a drilling template (11) controllable by computer, f) a fifth system for controlling by computer said fourth system starting from said parameters, g) bore tubes (15) in said drilling template (11) on with predetermined inclinations and positions fitted with guiding cylinders (14) of standard dimension predetermined in function of the type of said implants (1), h) drilling tubes (18) in coaxial arrangement in the upper part (15) of said cylinders (14), i) drills (4, 5) with diameters corresponding to the inside diameters of said drilling tubes (18), and j) fixture mounts (3), characterized in that said drilling tubes (18) all display a single predetermined standard inner diameter in function of said implants (1) and in that said drills (4, 5) include a first set of staged drills (4) and a second set of calibration drills (5) of which, respectively, the maximal standard diameter and the nominal standard diameter correspond to the inner diameter of said drilling tubes (18).
11) Device for the placement of dental implants (1) corresponding to claim 10, characterized in that each of the staged drills (4) displays successively, along its axis, from the pointed end to the other end - a first drilling section (30) with one single standard diameter predetermined by the type of said implants (1), - a second drilling section (31), adjacent to said first drilling section (30) with one single standard diameter predetermined by the type of said implants (1) and larger than the diameter of said first drilling section (30), - a smooth part (32) with one single standard diameter predetermined in function of the type of said implants (1) and corresponding to the standard inner diameter of said staged drills (18), - a flange (22), and - a standard blocking means for a handpiece (33)
12) Device for the placement of dental implants (1) according to claim 10 or 11, characterized in that each one of the calibration drills (5) displays along its axis from the pointed end to the other end:
- a first cutting section (34) with one single standard diameter predetermined by the type of said implants (1) and corresponding to the standard inner diameter of said drilling tubes (18), - a second cutting section (35) with a diameter noticeably inferior to the diameter of said first cutting section (34), - a smooth drill zone (36) with a single standard diameter predetermined by the type of said implants (1) and corresponding to the unique inner diameter of said drilling tubes (18), - a flange (22), and - a standard blocking means for a handpiece (33)
- a first cutting section (34) with one single standard diameter predetermined by the type of said implants (1) and corresponding to the standard inner diameter of said drilling tubes (18), - a second cutting section (35) with a diameter noticeably inferior to the diameter of said first cutting section (34), - a smooth drill zone (36) with a single standard diameter predetermined by the type of said implants (1) and corresponding to the unique inner diameter of said drilling tubes (18), - a flange (22), and - a standard blocking means for a handpiece (33)
13) Device for the placement of dental implants (1) according to any one of preceding claims 10 to 12, characterized in that each one of the fixture mounts (3) contains on one hand, a mandrel (37) which features, successively along its axis:
- a piece (39) complementary to a handpiece-connector - a flange (22) - a smooth section (24) of a single standard external diameter predetermined in function of the type of implants (1) and corresponding to the standard inside diameter of said cylinders (14) - a piece (41) complementary to the standard head (42) of said implants (1), and, on the other and, a screw (38) which passes through the mandrel (38) and is screwed in said head (42).
- a piece (39) complementary to a handpiece-connector - a flange (22) - a smooth section (24) of a single standard external diameter predetermined in function of the type of implants (1) and corresponding to the standard inside diameter of said cylinders (14) - a piece (41) complementary to the standard head (42) of said implants (1), and, on the other and, a screw (38) which passes through the mandrel (38) and is screwed in said head (42).
14) Device for the placement of dental implants (1) according to any one of preceding claims 10 to 13, characterized in that the length of the bore tubes (15) is variable in order to limit - without additional means - the penetration depth of the drills (4, 5) in the mandible or the maxilla (7).
15) Device for the placement of dental implants (1) according to any of preceding claims 11 or 12, characterized in that it additionally comprises a set of rings (21) with inside diameters corresponding to the diameter of the drills (4,5), said rings (21) intended to be slid over the smooth drill section (32) or smooth drill zone (36) of each of the drills (4,5) so as to control the depth of penetration.
16) Device for the placement of dental implants (1) according to any one of preceding claims 14 or 15, characterized in that the staged drills (4) and the calibration drills (5)
17 all have the same length, predetermined in function of the type of said implants (1), independent of the depth of the holes (17) to be obtained, thus representing universal drills.
17) Device for the placement of dental implants (1) according to claim 13, characterized in that it additionally comprises a set of washers (23)of an inside diameter corresponding to the diameter of the fixture mounts (3), said washers (23) intended to be slid over the smooth section (24) of each of said fixture mounts (3) so as to control the depth of penetration.
17) Device for the placement of dental implants (1) according to claim 13, characterized in that it additionally comprises a set of washers (23)of an inside diameter corresponding to the diameter of the fixture mounts (3), said washers (23) intended to be slid over the smooth section (24) of each of said fixture mounts (3) so as to control the depth of penetration.
18) Device for the placement of dental implants (1) according to claim 17, characterized in that the fixture mounts (3) all have the same length, predetermined in function of the typo of said implants (1), to be placed, thus representing universal fixture mounts.
19) Device for the placement of dental implants (1) according to any one of preceding claims 10 to 18, characterized in that the cylinders (14) and the tubes (18) comprise respectively an internal screw thread (25) and an external screw thread (26) featuring four helixes at a 90° angle with respect to each other.
20) Device for the placement of dental implants (1) according to any one of preceding claims 10 to 19, characterized in that each of the tubes (18) features a ring (27) comprising a tangential slot (29).
21) Device for the placement of dental implants (1) according to any one of preceding claims 10 to 20, characterized in that each of the tubes (18) features a ring (27) comprising four blind radial holes (28) at 90° with respect to each other.
22) Device for the placement of dental implants (1) according to any one of preceding claims 15 or 17, characterized in that the rings (21) and the washers (23) are made of a bio-compatible plastic material, preferably of polyoxymethacrylate (POM).
23) Device for the placement of dental implants (1) according to any one of preceding claims 10 to 22, characterized in that the cylinders (14) are made of a titanium alloy, preferably of TA6V.
24) Device for the placement of dental implants (1) according to any one of preceding claims 10 to 23, characterized in that the tubes (18) are made of steel, preferably of INOX 316L.
25) Device for the placement of dental implants (1) according to claim 13 or any one of preceding claims 14 to 24 depending from claim 13, characterized in that the mandrel (37) is made of a titanium alloy, preferably of TA6V.
26) Device for the placement of dental implants (1) according to any one of preceding claims 10 to 25, characterized in that the standard inside diameter of the tubes (18) is preferentially 3,20 mm, the corresponding standard maximal diameter of the staged drills (4) and the corresponding nominal standard diameter of the calibration drills (5) preferably being 3,15 mm.
27) Device for the placement of dental implants (1) according to claim 13 and any one of preceding claims 14 to 26 dependent from claim 13, characterized in that the standard inner diameter of the cylinders (14) is preferentially 4,20 mm and the standard outer diameter corresponding to the sleeve (24) is preferentially 4,15 mm.
28) Device for the placement of dental implants (1) according to claims 11 or 12 and any one of previous claims 13 to 27 dependent from claims 11 or 12, characterized in that the drill sections (30,31) or the cutting sections (34,35) extend preferentially over mm, 13 mm, 15 mm, or 18 mm in case of the universal drills.
29) Device for the placement of dental implants (1) according to claim 11 and any one of the preceding claims 12 to 28 depending from claim 11, characterized in that - the first drilling section (30) preferentially has a diameter of 2 mm and extends over 4mm, - the second drilling section (31) preferentially has a diameter of 3 mm and extends over 6mm, - the smooth section (32) preferentially has a diameter of 3,15 mm and extends over 5 mm.
30) Device for the placement of dental implants (1) according to claim 12 and any one of preceding claims 13 to 29 depending from claim 12, characterized in that:
- the first cutting section (34) preferentially has a diameter of 3,15 mm and extends over 4 mm, - the second cutting section (35) preferentially has a diameter of 3mm and extends over 6mm, - the smooth zone (36) preferentially has a diameter of 3,15 mm and extends over 5 mm.
- the first cutting section (34) preferentially has a diameter of 3,15 mm and extends over 4 mm, - the second cutting section (35) preferentially has a diameter of 3mm and extends over 6mm, - the smooth zone (36) preferentially has a diameter of 3,15 mm and extends over 5 mm.
31) Device for the placement of dental implants (1) according to claim 13 and any one of preceding claims 14 to 30 depending from claim 13, characterized in that the length of the sleeve (24) and the length of the screw (38) are preferentially, respectively 4,5 mm and 13,5 mm for a osseous fixture mount, 10,5 mm and 19,5 mm for a mucosal fixture mount, 10,0 mm et 19,0 mm for one of said "universal" osseous fixture mounts, and 15,0 mm et 24,0 mm for one of said "universal" mucosal fixture mounts.
32) Device for the placement of dental implants (1) according to claims 11, 12 or 13 and any one of preceding claims 14 to 31 depending on claims 11, 12 or 13, characterized in that the flange (22) preferentially has a thickness of 0,5 mm and a external diameter of 5,2 mm.
33) Device for the placement of dental implants (1) according to claim 15 and any one of preceding claims 16 to 32 depending on claim 15, characterized in that the inner diameter of the rings (21) is preferentially 3,10 mm, the external diameter is preferentially 5,2 mm, and their thickness is preferentially 0,5 mm or 1,5 mm.
34) Device for the placement of dental implants (1) according to claim 17 and any one of preceding claims 18 to 33 depending on claims 17, characterized in that the inner diameter of the washers (23) is preferentially 4,10 mm, the external diameter is preferentially 5,2 mm, and their thickness is preferentially 0,5 mm or 1,5 mm.
